A child with unilateral dysplasia of the superior articular facet of the axis is described. This congenital anomaly, due to incomplete ossification, may be a cause of torticollis, although no signs of instability were noted.

In a 34-year-old man with an acute traumatic spondylolisthesis (ATS) of the axis, the presenting physical sign was severe torticollis associated with reactive spasm of the right sternomastoid muscle. Computed axial tomography (CAT) was useful both in assessing the nature of ATS (which involved more displacement of the axis fracture and adjacent soft-tissues on the right side of the neck) and in ascertaining reduction, realignment, and healing. The reduction was sustained by halo-vest immobilization.

BACKGROUND: Occipital plate fixation has been shown to improve outcomes in cervical spine fusion. There is a paucity of literature describing occipital plate fixation, especially in the pediatric population. The authors reviewed a case series of 34 patients at a pediatric hospital who underwent cervical spine fusion with occipital plate fixation between 2003 and 2016. This study describes how occipital plates aid the cervical spine union in a case series of diverse, complex pediatric patients.
METHODS: Our orthopaedic database at our institution was queried for patients undergoing an instrume

Most hangman's fractures are treated conservatively. If surgery is indicated, an anterior approach using a C2/C3 graft and plate fusion is usually preferred. Another surgical method according to Judet is direct transpedicular osteosynthesis by the dorsal approach. This surgery is frequently rejected because of the high risk of spinal cord damage or vertebral artery tear. Direct transpedicular osteosynthesis of hangman's fracture according to Judet is a "physiological operation" that does not cause fusion and creates anatomical conditions. This procedure enables appropriate reduction, compressi

Traumatic spondylolisthesis of C-2, frequently referred to as the hangman's fracture, is typically regarded as a hyperextension injury with rare neurologic sequelae by virtue of decompression of the neural canal. The authors retrospectively evaluated their radiographic experience with this injury in 27 patients over a 24-month period. Lateral cervical spine studies and computed tomography (CT) scans (21 patients) were analyzed. CT studies provided better delineation of the fracture in all 21 patients and significant additional information in five patients (24%). Seven patients (26%) suffered i

The upper cervical spine includes the articulations of the occiput with atlas and the atlas with the axis, as well as the bony structures of the base of the skull, axis, and atlas. The unique anatomy of the upper cervical spine and the typical mechanisms of injury yield a predictable variety of injury patterns. Injuries to this area include occipital condyle fractures, occipitoatlantal dislocations, subluxations and dislocations of the atlantoaxial articulation, atlas fractures, odontoid fractures, and fractures of the arch of the axis. Injuries to this region are relatively common and can be

The trans-condylar approach to the craniocervical junction area (CCJA) requires a more or less extensive drilling of the two first cervical joints (C0-C1 and C1-C2). The extent of drilling necessary to resect a lesion at the CCJA was analyzed from a series of 125 cases including 114 tumours and 11 non-tumoural processes treated using a lateral approach (postero-lateral or antero-lateral) over a 15-year period (1980-1995). The extent of drilling was estimated on CT scanner axial views from the reduction of the joints surface and three groups were determined: A/less than one third B/between one

This article provides a comprehensive look at trauma to the upper cervical spine. It begins with an in-depth discussion of vertebral anatomy, then focuses on types of cervical trauma and examines the diagnostic techniques used to image cervical trauma.

OBJECTIVE: To review and compare the techniques of fixation of the axis vertebral segment. Also, to review the anatomy of the axis vertebrae, ligamentous attachments, and unique biomechanics of this segment.
METHODS: The use of wire, cable, screw, and plate fixation techniques are reviewed and discussed in the treatment of fractures of C1 and C2 along with utilization of the halo vest and cervical collar during the postoperative period.
RESULTS: All fixation methods were useful. However, the appropriate fixation technique is best determined by the local anatomy (eg, anomalous vertebral arter

The degree of lordosis and mobility of the cervical spine, the "formation of steps" at the top end of the cervical spine, the Atlas-Dens distance, the diameter of the bony cervical spine canal and the breadth of the retropharyngeal and retrotracheal space were measured on 360 lateral radiograms of the cervical spine in the median position, at maximum flexion and maximum extension. The subjects were 120 children aged three to 14 years. The discussion of the results draws attention to their practical value and to the importance of functional radiographical examination of the cervical spine durin

PURPOSE: This study explored the effects of genioplasty (Gep) and anterior subapical osteotomy of the maxilla and mandible (ASOMx+ASOMd) on the pharyngeal airway dimensions of patients with bimaxillary protrusion (BiP).
METHOD: Thirty-two patients were divided into 2 groups. Group 1 received ASOMx+ASOMd, and group 2 received ASOMx+ASOMd+Gep. The cephalograms of the patients were collected before surgery and 2 months after surgery. Changes in the landmarks, related cephalometric angles (gonial, SN-GoGn, Y-axis, and SN-C2C4 angles), and 2 pharyngeal airway dimensions (uvulo-pharyngeal airway [U

OBJECTIVE: The optimal treatment modality for traumatic spondylolisthesis of the axis is still a controversial issue. Many conservative, posterior and anterior fixation techniques have been used to treat these injuries. The C2 pars (isthmus) was used as a novel screw anchor point in upper cervical spine injuries. The direct C2 pars screws are also used to fixate the posterior and anterior parts of the C2 vertebra.
METHODS: Four cases with traumatic spondylolisthesis of the axis were presented. The cases were classified as type II according to Levine and Edwards' classification. C2 pars was us

Symptomatic atlantoaxial dislocation occurs rarely in psoriatic arthropathy and has previously been reported only as a late complication in this disorder. We report severe upward axial dislocation and acquired basilar impression as a presenting manifestation of psoriatic arthropathy. Magnetic resonance imaging is useful in evaluating this condition.

Occipitoatlantoaxial malformation was diagnosed in a 19-month-old mare of Appaloosa breeding and in a 3-year-old Quarter Horse gelding. Both horses had abnormal head and neck carriage since birth, but neurologic deficits did not become evident clinically until the horses reached 2 and 3 years of age, respectively. Palpation and manipulation of the base of the skull and cervical area proved to be useful diagnostically. Movement could not be elicited at the atlantooccipital joint but, in comparison with clinically normal horses, the range of dorsoventral motion at the atlantoaxial joint was incr

An 18-month-old child was rendered tetraplegic below C6 in a road traffic accident. There was an associated fracture-dislocation of the atlanto-axial joint. Early management, and the probable role of autonomic dysreflexia in increasing the neurological deficit are considered.

Trauma of the cranio-vertebral junction does not only cause osseous and ligamentous injuries but also nervous and vascular damage. The peculiar structure of these joints and trauma by hyperextension as well as forced bending of the spinal cord provoke distorsion, dislocations and fractures of atlas and axis as well as occipital condyles. A radiographic examination of these lesions must be minute and tomography and functional tests employed. Post-traumatic, neurologic and vascular complications are often the cause for acute and chronic bulbo-medullary alterations.
