Cellular mechanisms of depressed atrial contractility in patients with chronic atrial fibrillation.

BACKGROUND: After cardioversion of atrial fibrillation (AF), the contractile function of the atria is temporarily impaired. Although this has significant clinical implications, the underlying cellular mechanisms are poorly understood. METHODS AND RESULTS: Forty-nine consecutive patients submitted for mitral valve surgery were investigated. Twenty-three were in persistent AF (>/=3 months); the others were in sinus rhythm. Before extracorporal circulation, the right atrial appendage was excised. ss-Adrenoceptors were quantified by radioligand binding, and G proteins were quantified by Western b

[CURRENT ISSUES OF BETA-BLOCKERS USE IN THE MANAGEMENT OF COVID-19 (REVIEW)].

Over the past two decades, the world has witnessed severe acute respiratory syndrome CoV (SARS-CoV), an underlying serious diseases, with a high rate of mortality in patients with comorbidities. Taking into account the clinical features of pulmonary edema, septic shock, deep vein thrombosis and pulmonary embolism, it seems that the severe course of COVID-19 can manifest itself in the body as a sympathetic storm and, possibly, can underlie many of the pathological processes that occur with COVID-19. It is the regulation of the RAAS by the adrenergic system may be crucially important in the cour

Effects of atenolol on exercise capacity in patients with mitral stenosis with sinus rhythm.

Exercise capacity is frequently impaired in patients with mitral stenosis (MS) and sinus rhythm (SR). The resulting increased heart rate, which shortens the diastolic filling period, and the increased cardiac output lead to further elevations of left atrial pressure and subsequent pulmonary congestion. The effect of the beta-receptor blocking agent atenolol, 100 mg/day, was assessed in 13 patients with MS and SR. Exercise performance was assessed using a modified multistage Bruce protocol after 2 weeks of placebo and after 2 weeks therapy with atenolol in a single-blind, crossover, placebo-con

Pickles, peptide hormones and pregnancy: a hypothesis.

In western societies a high frequency of heartburn, nausea and vomiting occur during pregnancy. The causes and mechanisms of these clinical symptoms remain poorly understood. Evidence suggests steroid hormones modulate gastrointestinal transit time and plasma peptide hormones, while peptide hormone modulated food intake and preferences. Since diurnal and episodic release of steroid and peptide hormones occur, it is postulated that heartburn and other digestive dysfunction during pregnancy are associated with elevated steroid and peptide (beta-endorphin, NPY) hormone interaction with innate bio

Beta-endorphin and dynorphin mimic the circadian immunoenhancing and anti-stress effects of melatonin.

We have recently demonstrated that the pineal neurohormone melatonin can enhance immune reactivity in normal mice and counteract the effects of acute stress or corticosterone treatment on antibody production, thymus weight and anti-viral resistance. These remarkable immunopharmacologic effects of melatonin were abolished by naltrexone, suggesting an involvement of the endogenous opioid system. Here we compared the immunopharmacologic action of beta-endorphin, dynorphin 1-13, leu-enkephalin and metenkephalin with that of melatonin in restraint-stressed or prednisolone-treated mice and in normal

Metoprolol vs ivabradine in patients with mitral stenosis in sinus rhythm.

BACKGROUND: Severe mitral stenosis is usually symptomatic and is treated by BMV or surgery, whereas mild to moderate mitral stenosis is usually asymptomatic or mildly symptomatic and managed medically. Patients in the later group may become symptomatic during episodes of exercise and increased heart rate. Beta-blockers are frequently used in patients with mitral stenosis to control the heart rate and alleviate exercise-related symptoms. The objective of our study was to investigate the comparative efficacy of ivabradine versus metoprolol in patients with mitral stenosis in sinus rhythm. METHO

Brain electrical source differences between depressed subjects and healthy controls.

Many brain regions show metabolic and perfusion abnormalities in major depressive disorder (MDD), including anterior cingulate and prefrontal cortices. Some of these same areas also show abnormal function with low resolution electromagnetic tomography (LORETA). However, LORETA results are not always consistent across studies, nor with findings from other imaging modalities. These discrepancies may be due, among other factors, to the sensitivity of EEG source localization to different electrode montages. Thirty-six channel EEG was collected from healthy controls and age- and gender-matched unme

Circadian and sleep phenotypes in a mouse model of Alzheimer's disease characterized by intracellular accumulation of amyloid β oligomers.

Disturbances in sleep-wake and circadian rhythms may reportedly precede the onset of cognitive symptoms in the early stages of Alzheimer's disease (AD); however, the underlying mechanisms of these AD-induced sleep disturbances remain unelucidated. To specifically evaluate the involvement of amyloid beta (Aβ) oligomers in AD-induced sleep disturbances, we examined circadian and sleep phenotypes using an Aβ-GFP transgenic (Aβ-GFP Tg) mouse characterized by intracellular accumulation of Aβ oligomers. The circadian rhythm and free-running period of wheel running activity were identical between Aβ-

The rhythm of decline: Circadian disruption in neurodegeneration.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder with a multifactorial etiology involving genetic, environmental, and metabolic factors. Among these, circadian rhythm disruption has emerged as a crucial but under-explored contributor to disease progression. The circadian system, regulated by the suprachiasmatic nucleus (SCN), controls essential physiological functions such as the sleep-wake cycle, metabolism, and neuroendocrine signaling. Disruption of this system has been increasingly linked to key pathological features of AD, including amyloid-beta accumulation, tau hyper

Longitudinal Exploration of Cortical Brain Activity in Cognitive Fog: An EEG Study in Patients with and without Anosmia.

BACKGROUND: Long-Covid, characterized by persistent symptoms following acute Covid-19 infection, represents a complex challenge for the scientific community. Among the most common and debilitating manifestations, cognitive fog is a neurological disorder characterized by mental confusion and cognitive difficulties. In this study, we investigated the long-term effects of previous Covid-19 infection on cortical brain activity in patients experiencing cognitive fog symptoms in the medium and long term. METHODS: A total of 40 subjects (20 females and 20 males) aged between 45 and 70 years (mean ag

[Asystole after anesthetic induction with remifentanil].

A 45-year-old man was scheduled for laparoscopic cholecystectomy. He had hypertension controlled with beta-adrenergic, calcium channel and angiotensin II receptor blocking agents. Because he had complications of symptomatic cervical spondylosis and sleep apnea syndrome, we performed awake fiberoptic intubation with remifentanil at a dose of 0.05 microg x kg(-1) min(-1). After intubation and following administration of propofol and vecuronium, ECG unexpectedly changed to asystole. We administered atropine 1.5 mg and performed chest compressions, which successfully restored sinus rhythm within 1

Dual AV Nodal Non-Reentrant Tachycardia as a Suspected Cause of Tachyarrhythmia-Induced Cardiomyopathy.

We describe the case of a 69-year-old female patient who presented with a sensation of her heart pounding and an electrocardiogram showing tachycardia with unusual grouped beating. She was recently diagnosed with heart failure with reduced ejection fraction presumed to be secondary to atrial fibrillation which did not improve with beta-blocker therapy. She underwent an electrophysiology study which demonstrated Dual AV nodal non-reentrant tachycardia status post slow pathway ablation. She had significant improvement in her ejection fraction and palpitations thereafter. This case highlights the

Mechanisms of Improved Mortality Following Ablation: Does Ablation Restore Beta-Blocker Benefit in Atrial Fibrillation/Heart Failure?

Observational trials have shown that atrial fibrillation ablation favorably impacts long-term outcomes in systolic heart failure. These outcomes have been confirmed by randomized prospective trials highlighting the favorable impact of ablation on left ventricular function and remodeling, risk of heart failure hospitalization, and mortality. Ablation along with established heart failure medications is new and supported conceptually by the value of restoring sinus rhythm, avoiding long-term antiarrhythmic drugs, and minimizing drug-drug interactions. Observational data suggest a potential long-t

Prophylaxis and management of atrial fibrillation after general thoracic surgery.

Atrial fibrillation (AF) commonly affects patients after general thoracic surgery. Postoperative AF increases hospital stay and charges. Effective prophylaxis and treatment is the goal. Calcium channel blockers prevent postoperative AF. Beta blockers are a less viable choice. Amiodarone prophylaxis should be avoided in patients with pulmonary dysfunction or who require pneumonectomy. In management of AF, a brief trial of rate-control agents is appropriate; however, chemical cardioversion with rhythm-control agents should be instituted after 24 hours. High-risk patients with history of stroke o

Ventricular arrhythmias: antiadrenergic therapy for the patient with coronary artery disease.

Patients who have had a recent myocardial infarction (MI) are at high risk of ventricular arrhythmias that often cause sudden cardiac death. It is believed that sympathetic overactivity in the peri-infarction period may alter the electrophysiology and structure of the myocardium, thus placing these patients at risk of developing rhythm disturbances. A number of pharmacologic and nonpharmacologic therapies have been shown to reduce the risk of post-MI mortality, including sudden cardiac death. beta-Adrenergic blockers are recommended for all post-MI patients without contraindications because of
