The Impact of Lipoprotein Apheresis on Changes in Biochemical Enzymes: A Systematic Review and Meta-Analysis.

Lipoprotein apheresis (LA) is an established therapy for severe, drug-refractory dyslipidemias. Nevertheless, clinicians often monitor hepatic and muscle enzymes during treatment because transient biochemical shifts can be misinterpreted as injury. In this study, we synthesize the evidence on how repeated apheresis sessions affect circulating enzymes. We systematically searched PubMed, Scopus and Web of Science from inception to August 2025 for observational human studies reporting pre- and post-apheresis values (mean and SD) for aspartate aminotransferase (AST), alanine aminotransferase (ALT)

Two-phase Intravesical Injection of Apheresis-derived Platelet-rich Plasma for Non-ulcerative Primary Bladder Pain Syndrome: A Prospective Single-center Study.

OBJECTIVE: To prospectively evaluate the efficacy and predictive factors of a 2-phase intravesical injection protocol using apheresis-derived platelet-rich plasma (PRP) in patients with refractory non-ulcerative primary bladder pain syndrome (PBPS). MATERIALS AND METHODS: Between June 2023 and August 2025, 102 patients with PBPS were stratified into Groups A, B, and C based on cystoscopic grading of hydrodistention (Grade I-III). All patients underwent video-urodynamic studies and PRP collection, followed by a 2-phase intravesical injection protocol comprising an intensive weekly phase for th

Lipoprotein apheresis for anti-nephrin antibody-positive steroid-resistant nephrotic syndrome: a pediatric case report.

Steroid-resistant nephrotic syndrome (SRNS) in children carries a poor prognosis, and recent discoveries implicate anti-nephrin autoantibodies (ANABs) as mediators of podocyte injury. Lipoprotein apheresis, originally developed for familial hypercholesterolemia, has been explored as rescue therapy in refractory SRNS, but its immunologic effects remain uncertain. This case demonstrates a decline in ANAB titers accompanied by clinical improvement following lipoprotein apheresis. We report a 7-year-old male of Afro-Caribbean descent with biopsy findings consistent with minimal change disease and

Lipoprotein Apheresis: Utility, Outcomes, and Implementation in Clinical Practice: A Scientific Statement From the American Heart Association.

Despite the availability of multiple classes of lipoprotein-lowering medications, some high-risk patients have persistent hypercholesterolemia and may require nonpharmacologic therapy. Lipoprotein apheresis (LA) is a valuable but underused adjunctive therapeutic option for low-density lipoprotein cholesterol and lipoprotein(a) lowering, particularly in children and adults with familial hypercholesterolemia. In addition to lipid lowering, LA reduces serum levels of proinflammatory and prothrombotic factors, reduces blood viscosity, increases microvascular myocardial perfusion, and may provide b

Clinical impact of a novel lipoprotein apheresis treatment on no-option chronic limb-threatening ischemia: Result from the REDUCTION VISCOSITY study.

INTRODUCTION: Lipoprotein apheresis (LA) is a possible adjunct treatment for no-option chronic limb-threatening ischemia (CLTI). This study aimed to assess the impact of a novel LA for no-option CLTI. METHODS: We retrospectively assessed 19 patients with no-option CLTI treated using the novel LA. The primary outcome was a change in the skin perfusion pressure (SPP) after treatment with LA, and the secondary outcomes were changes in the viscosity-related laboratory parameters. RESULTS: The wound-healing rate was 68.4%. The SPP at 2-3 weeks after series of LA were significantly higher both in

Case Report: C-Reactive Protein Apheresis in a Patient With COVID-19 and Fulminant CRP Increase.

BACKGROUND: Plasma levels of C-reactive protein (CRP), induced by Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) triggering COVID-19, can rise surprisingly high. The increase of the CRP concentration as well as a certain threshold concentration of CRP are indicative of clinical deterioration to artificial ventilation. In COVID-19, virus-induced lung injury and the subsequent massive onset of inflammation often drives pulmonary fibrosis. Fibrosis of the lung usually proceeds as sequela to a severe course of COVID-19 and its consequences only show months later. CRP-mediated complem

Apheresis principles in a patient with chronic myeloid leukemia during pregnancy: challenges in cell separation and assessing transcript levels.

BACKGROUND: Chronic myeloid leukemia (CML) is rarely diagnosed in pregnant women. CASE REPORT: We report a case of a pregnant woman who presented with a leukocyte count of 250 × 109 cells/L at gestational age (GA) 26 weeks and was diagnosed with CML in the chronic phase. Because the patient deliberately opted out of interferon α and tyrosine kinase inhibitor treatment, the main goal was to reduce the leukocyte count to postpone delivery beyond the number of weeks considered severely premature and avoid thromboembolic complications while continuously evaluating the clinical safety of the mothe

History of lipidology and lipoprotein apheresis.

This review tells the story of atherosclerosis research in the beginning of the 20th century. It presents the significance of cardiovascular diseases and addresses major questions currently being discussed among lipidologists and the current thinking with respect to low LDL-cholesterol levels and HDL. It provides an overview of the period during which lipid-modifying drugs were introduced and their relevance with respect to cardiovascular outcome data and lists possible reasons why some patients develop new cardiovascular events while being treated with statins. Especially impressive is the hi

[Above-standard proceeding in nephrotic syndrome - case report].

The nephrotic syndrome is characterized by the loss of many proteins, via the urinary system. It exceeds the bodies compensatory abilities and results in abnormalities in blood clotting system, particularly due to antithrombin deficiency. It significantly increases the risk of thromboembolic complications. A loss of erythropoietin and transferrin leads to anemia. Polycythemia is a rarely reported phenomenon. The case describes a 20-years old patient with massive nephrotic syndrome and polycythemia, complicated by a pulmonary embolism. The patient had a steroid-dependent submicroscopic glomerul

Side-effects and complications in large-bore catheters for apheresis.

Vascular catheters have become essential tools for management of hospitalized or chronically ill patients requiring intensive medical treatments such as extracorporeal detoxification procedures. The increased use of such devices has been accompanied by a corresponding increase in complications, such as bloodstream infection and thrombosis. In a retrospective study 332 large bore catheters which were inserted in 182 patients in the inferior or superior vena cava were investigated. The complication rate was in internal jugular vein puncture (N = 231) with 20% low and in the subclavian vein punct

Pediatric Therapeutic Apheresis: A Critical Appraisal of Evidence.

Apheresis technology has progressed significantly over the last 50-60 years from a predominately blood donation-based procedure to one that now includes a variety of therapeutic modalities. The last 25 years also has seen an increase in the number of diseases treated by therapeutic apheresis (TA) modalities. Because of ethical considerations, therapeutic modalities are often vetted first in adult populations before establishing utility in pediatric patients. TA is no different. The majority of published studies involve adult patients. Pediatric apheresis studies are traditionally retrospective

Red cell apheresis in autologous preoperative blood donation.

Preoperative autologous blood donation (PAD) is a commonly used technique for elective surgical procedures with a predictable blood loss. The conventional method is to collect one unit whole blood weekly several times within the shelf-life of red cell concentrates. Erythrapheresis with the Haemonetics cell separator MCS-3p or plus makes it possible to collect two units of red cells in one single session and with this option the time interval and operation can be prolonged to three weeks to improve erythropoiesis. 295 patients, who were scheduled for hip replacement or knee endoprothesis were e

Guidance for Documentation of Therapeutic Apheresis Interventions in the Medical Record: An American Society for Apheresis (ASFA) Practice Perspective.

In 2007, the American Society for Apheresis (ASFA) published guidance for physician documentation related to oversight of therapeutic apheresis (TA). Due to 21st century changes in healthcare delivery, the ASFA Board of Directors (BOD) charged its Public Affairs and Advocacy Committee (PAAC) to coordinate a review and update of the guidance. PAAC members reviewed minutes from various ASFA committees' earlier deliberations and sought additional information from the medical and general literature and regulatory/professional organizations involved with medical records documentation as well as fro

Lipoprotein apheresis as rescue therapy in lipoprotein glomerulopathy: The role of plasmatic extracellular vesicle removal.

BACKGROUND: Lipoprotein glomerulopathy (LPG) is a rare disease affecting the kidney, clinically characterized by type III hyperlipoproteinemia, mutated apolipoprotein E (ApoE), the presence of glomerular lipoprotein thrombi, proteinuria in the nephrotic range, and progression toward chronic kidney disease (CKD). OBJECTIVE: Evaluate the role of lipoprotein apheresis (LA) as rescue therapy in LPG. METHODS: Since 2011, we have identified 6 unrelated patients living in the same geographical area of Central Italy (Tuscany Region) who share the same rare ApoE mutation. RESULTS: Four patients, due

Beyond the Centrifuge: Is FcRn Inhibition Ready to Replace Apheresis in Treatment of Guillain-Barré Syndrome?

Despite the available treatment modalities such as intravenous immunoglobulin (IVIG) and therapeutic plasma exchange (TPE), many Guillain-Barré Syndrome (GBS) patients suffer from incomplete recovery and persistent paralysis. This paper evaluates the potential of targeting the neonatal fragment crystallizable receptor to reduce pathogenic immunoglobulin G (IgG) as a novel therapeutic approach. By reviewing current clinical evidence including data from 10 recent case reports and case series across various GBS presentations, we found that FcRn inhibitors such as efgartigimod consistently led to

Patients with Acute Kidney Disease in Need of Acute Hemodialysis and/or Apheresis: A Semi-Structured Interview Study.

Patients with acute kidney disease (AKD) may need treatment by hemodialysis and/or apheresis, which is often started in haste due to the life-threatening condition. To illuminate patients' lived experiences of being in acute need of such therapy due to AKD, we performed semi-structured interviews analyzed with qualitative content analysis. Eleven patients out of 104 were eligible and completed interviews. Six main categories were found - shock and emotional chaos, loneliness in an unfamiliar situation, a need for tailored information, emotional support from next of kin, the dialysis and/or aph

Granulocyte and Monocyte Adsorptive Apheresis for Ulcerative Colitis in a Patient with Low Bone Mineral Density Due to Fanconi-Bickel Syndrome.

Systemic steroid is required for the exacerbation of ulcerative colitis (UC), although its administration should be avoided in patients with a low bone mineral density (BMD) exacerbated by side effects of steroids. We herein report the successful induction of remission in an UC case with a low BMD due to Fanconi-Bickel syndrome-or glycogen storage disease type XI-using granulocyte and monocyte adsorptive apheresis (GMA). For a 43-year-old woman with a BMD of 50% the young adult mean, GMA was performed 2 times a week for a total of 10 times. GMA might be a steroid-free treatment option for UC p

Lipoprotein(a)-an interdisciplinary challenge.

Lipoprotein(a) (Lp(a)) is an internationally recognized atherogenic risk factor which is inherited and not changed by nutrition or physical activity. At present, only proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors may modestly decrease its concentration (but not in all patients)-leading to a certain decrease in cardiovascular events (CVE) in controlled studies. However, at present an elevation of Lp(a) is not a generally accepted indication for their use. More effective is lipoprotein apheresis (LA) therapy with respect to both lowering Lp(a) levels and reduction of CVE. In t
