Acute Normovolemic Hemodilution in Adult Cardiac Surgery.

IMPORTANCE: Acute normovolemic hemodilution (ANH) is a well-known blood conservation technique, yet its adoption in the US remains low (<20%), and cardiac surgery remains the largest consumer of blood components. Increasing vulnerabilities in the US blood supply underscore the need to reassess ANH effectiveness in blood conservation. OBJECTIVE: To evaluate ANH use in perioperative transfusion and blood component use in patients undergoing surgery with cardiopulmonary bypass (CPB). DESIGN, SETTING, AND PARTICIPANTS: A retrospective propensity-score matched cohort study using data from the Soc

Are we giving enough coagulation factors during major trauma resuscitation?

Hemorrhage is a major cause of trauma deaths. Coagulopathy exacerbates hemorrhage and is commonly seen during major trauma resuscitation, suggesting that current practice of coagulation factor transfusion is inadequate. Reversal of coagulopathy involves normalization of body temperature, elimination of the causes of disseminated intravascular coagulation (DIC), and transfusion with fresh-frozen plasma (FFP), platelets, and cryoprecipitate. Transfusion should be guided by clinical factors and laboratory results. However, in major trauma, clinical signs may be obscured and various factors conspi

European strategies against the parasite transfusion risk.

Protozoal infections are endemic in mainly tropical low income countries, affecting millions of people. Malaria, American trypanosomiasis (Trypanosoma cruzi/Chagas disease) and protozoal tickborne diseases (e.g. Babesia) can be efficiently transmitted by transfusion of cellular blood components. In non-endemic areas like Europe malaria, Chagas disease and Babesia are imported diseases resulting of travelling to endemic areas and migration of autochthons from these endemic areas. A recent International Forum showed that in Europe, as well as the USA, prevention of transfusion-associated protozo

General principles of small animal blood component administration.

Administration of blood components instead of whole blood has become the most important means of transfusion support in dogs and in some settings in cats as well. Component administration entails the transfer of biological material and carries an inherent risk of disease transmission and adverse reactions. This article reviews the indications, pretransfusion considerations, and follow up in small animal patients receiving blood component therapy.

Review of haemovigilance at the Rabat Regional Blood Transfusion Centre in Morocco (2017-2021).

INTRODUCTION: blood transfusion remains an essential therapeutic intervention, but the occurrence of transfusion reactions makes its administration even more complex. Vigilant reporting of such reactions by recipients of blood products is essential for effective haemovigilance. This study aimed to determine the frequency and nature of transfusion reactions. METHODS: conducted over five years (2017-2021) at the Haemovigilance Department of the Rabat Regional Blood Transfusion Centre, this retrospective study exploited incident forms notified by health establishments and data from the regional

TRALI-new challenges for histocompatibility and immunogenetics in transfusion medicine.

Antibodies against human leukocyte antigens (HLAs) have long been associated with transfusion-related acute lung injury (TRALI). In contrast to febrile transfusion reactions and refractoriness to platelet transfusions in immunized patients, the causative antibodies in TRALI are present in the transfused blood component, i.e. they are formed by the blood donor and not by the recipient. Consequently, blood components with high plasma volume are particularly associated with TRALI. In addition to antibodies against HLAs, antibodies directed against human neutrophil antigens (HNAs) present in the p

Transfusion-related acute lung injury.

Transfusion-related acute lung injury is a clinical syndrome that occurs within 6 hours of transfusion. It is the leading cause of transfusion-related mortality. It presents with shortness of breath, acute pulmonary edema, fever, hypotension, or hypertension followed by hypotension. Treatment consists of respiratory support and fluid administration to support blood pressure. A majority of cases are associated with antibodies to white blood cells in the blood donor. Blood centers in the United States are currently taking measures to reduce the risk of transfusion-related acute lung injury from

The Utility of Viscoelastic Testing in Guiding Intraoperative Transfusion Requirements in Pediatric Living Donor Liver Transplantation.

INTRODUCTION: Blood transfusion, though essential, is associated with increased morbidity following liver transplantation (LT). In pediatric liver transplantation (PLT), where physiological reserves are limited, optimal blood component transfusion (BCT) is critical. While viscoelastic testing (VET) has shown benefits in adult LT, pediatric evidence remains limited and transfusion triggers are poorly defined. This study evaluated the effectiveness of VET-guided BCT in pediatric living donor liver transplantation (LDLT). MATERIALS AND METHODS: A prospective analysis was conducted on children un

Transmission of Babesia microti in Minnesota through four blood donations from the same donor over a 6-month period.

BACKGROUND: Babesiosis is a tick-borne zoonosis caused by intraerythrocytic protozoa. More than 40 US cases of Babesia microti infection acquired by blood transfusion have been reported. This report describes the identification of a transfusion-associated case of babesiosis and the subsequent identification of the infected blood donor and three other infected recipients of cellular blood components from three other donations by this donor. STUDY DESIGN AND METHODS: Serum specimens from the donors of blood that had been made into cellular components received by the index recipient and from oth

[Transfusion and sickle cell disease].

Sickle cell disease is a hereditary pathology of the haemoglobin which affects only individuals from African ancestry. The frequency of the disease increases in France. Transfusion remains a major treatment of this disease. Depending of the indication, transfusion can be a simple transfusion or an exchange transfusion. In this last case, exchange can be performed manually or automatically. The transfusion protocols have to be adapted to the polytransfused status of these patients, but also to the high incidence of alloimmunisation against red blood cells. Alloimmunisation is a consequence of t

Study protocol for a pilot randomised controlled trial assessing prehospital whole blood versus component therapy in traumatic haemorrhage: SWiFT Canada (study of whole blood in frontline trauma).

INTRODUCTION: Major haemorrhage is the leading cause of preventable death in trauma, and prehospital blood transfusion may improve survival and outcomes for patients with prolonged out-of-hospital times. Globally, there is increasing interest in the use of whole blood in the prehospital environment, with randomised controlled trials ongoing. However, the results of these studies may not be generalisable to the longer out-of-hospital times seen in the Canadian trauma environment. The aim of this trial is to determine the feasibility of performing a randomised clinical trial evaluating the use o

Soluble antigens in plasma allow mismatched transfusion without hemolysis.

BACKGROUND: Universal plasma is a scarce resource when a massive transfusion protocol has been initiated. Previous studies have reported success using group A plasma in place of the universal plasma, group AB. It is unclear why there are not more reports of hemolytic reactions occurring from this practice. One possible explanation is the presence of water-soluble antigens in the patient plasma that bind to, and neutralize, the soluble antibodies present in the transfused plasma. STUDY DESIGN AND METHODS: Expired units of plasma were used to make dilutions that consisted of mixtures of group A

Future directions in utilization review: the role of transfusion algorithms.

Transfusion practice guidelines and retrospective utilization review have been ineffective in curtailing the inappropriate use of blood and blood products, particularly in cardiac surgical patients. Patients undergoing cardiac surgery with cardiopulmonary bypass (CPB) are at increased risk for excessive perioperative blood loss requiring transfusion of blood products. Recent evaluations have focused on the use of point-of-care coagulation assays for patient-specific therapy. Blood component administration in patients with excessive post-CPB bleeding is generally empiric, in part related to the

Massive transfusion in trauma.

The critically injured trauma patient in severe hemorrhagic shock presents a complex and arduous challenge to even the most experienced trauma staff. Lives are at stake as trauma teams feverishly pour massive amounts of blood into severely injured patients attempting to replace entire blood volumes several times over. Prompt identification of patients at risk for hemorrhage and establishment of massive transfusion protocols can provide guidance in managing the transfusion needs of the exsanguinating patient. These state-of-the-art protocols address component therapy, endpoints to transfusion a
