Lateral anterior column release (ACR) is a powerful extension of the minimally invasive lateral lumbar interbody fusion procedure that incorporates division of the anterior longitudinal ligament to allow manipulation of the anterior and middle spinal columns. The resulting surgical control permits restoration of significant segmental lordosis that, when combined with varying posterior column releases, can achieve global sagittal realignment on par with traditional 3-column osteotomies. METHODS: The recent literature concerning LCL at home and abroad was extensively reviewed, and the research and development were summarized.

Staged leg lengthening allows achondroplastic dwarfs to reach nearly normal height, but it takes long periods of external fixation and it can be burdened by delayed unions. Between 2009 and 2013, eight achondroplastic dwarfs showed delayed unions in the callus formation during femoral lengthening stages in our institute. BACKGROUND: Patients with congenital limb shortening can present with joint instability, soft tissue contractures, and significant leg length discrepancy. Classically, lengthening is done with external fixation, which can result in scarring, pin site infection, loss of motion, and pain. OBJECTIVE: To report three clinical cases suffering from limb shortening secondary to monostotic physeal closure, in which distraction osteogenesis had been performed without a latency period and with high distraction rates.

Although the simplest way to correct bone deformity is one-stage correction, the problem associated with that method is overstretching of the soft tissues, which limits the correction and leads to complications such as compartment syndrome or peripheral nerve palsy. In treating scaphoid nonunion, we have developed a technique of bone grafting and elongation of the scaphoid stabilizing the construct with K wires without compression. Bony union was achieved in the large majority of scaphoids as demonstrated on computed tomography (CT) scans. We advocate slight lengthening of the scaphoid with bone graft and K-wire fixation without compression of the grafted bone when treating scaphoid waist nonunion.

The combined experience of the Royal Hospital for Sick Children, Glasgow and the Robert Jones and Agnes Hunt Orthopaedic Hospital, Oswestry in lengthening 52 lower limb segments with the Orthofix device is reviewed. From 1991 to 1993, 23 distraction lengthenings were performed using the Ilisarow external fixator. It could be demonstrated that significant lengthening was possible without bone transplantation with the exception of two cases. In one further case, non-union resulted. Further complications were five pin infections, four loosenings of the apparatus, and in one case difficulties with the scar on the amputation site. CASE: A 51-year-old nonambulatory woman with type III osteogenesis imperfecta sustained a diaphyseal femur fracture during a transfer, distal to a childhood-placed intramedullary pin. At the time of presentation, the fracture was a painful atrophic nonunion, preventing effective transfer.
