In developmental dysplasia of the hip, a deficient acetabulum may be augmented by placing local autogenous iliac osseous graft, or the ilium itself, over the head of the femur with the expectation that the added bone will function as a bearing surface. Post-fracture osteolysis of the pubic bone is rare. We report a case of a 70-year-old woman with osteoporosis and a history of radiation therapy 2 years earlier. At presentation, she was found to have a bilateral sacral fracture and fractures of both pubic rami on one side. The pain persisted, and follow-up radiographs showed osteolysis of the pubic rami suggestive of metastatic disease. Between march 1990 and december 1992, 19 patients with compound fractures were treated with Ilisarow's external fixator at the Department of Traumatology of the University of Ulm. A consecutive, prospective series of ninety-seven patients who had 100 fractures of the femoral shaft that were treated with static interlocking nailing was analyzed to determine the incidence of union of the fracture without planned conversion from static to dynamic intramedullary fixation as a technique to stimulate healing of the fracture. We describe our experience using an intramedullary nail (Fitbone) which contains a motorised and programmable sliding mechanism for limb lengthening and bone transport. Sixty-two replantations of totally severed body parts in 25 patients were followed via serial radiography. Most of these replantations were of digits. Uncomplicated digital replantation showed a definite temporal pattern consisting initially of decreasing mineral content followed by increasing mineral content, decreasing soft-tissue swelling, and increasing callus formation. Deviations from the expected patterns were helpful in predicting complications such as infection and/or osteonecrosis.
