Six women received irradiation for lymphoma (Hodgkin's disease, n = 5; non-Hodgkin's disease, n = 1) to at least a mediastinal field (n = 2) or to a mantle field (n = 4), and subsequently developed breast cancer 10-27 years later. A case of histologically confirmed Paget's disease of the breast in a 72 year old man, without underlying breast carcinoma, is reported. This report raises questions about the pathogenesis of this condition and suggests that Paget's disease is an independent, intraepidermal carcinoma rather than a direct extension of intraductal carcinoma of the breast to the nipple and areola.

Metastasis of breast cancer is examined by the database of the Annuals of the Pathological Autopsy Cases in Japan from 1981 to 1983. Breast cancer metastases to the lungs (70.3%), liver (58.4%), bone marrow (53.1%), pleura (29.0%), adrenals (27.9%), skin-subcutaneous tissue (19.7%), brain (14.3%) are frequent in decreasing order. One hundred and nine patients with unilateral breast carcinoma and no palpable abnormality of the contralateral breast were evaluated by opposite breast biopsy. Our patient population yielded four noninfiltrating carcinomas, and one infiltrating carcinoma for an incidence of 4.5%. The one infiltrating cancer was suspected on a preoperative mammogram. We have discontinued the routine use of the opposite breast biopsy as a diagnostic tool in patients with unilateral breast cancer, in patients with no palpable abnormality and a normal mammogram.

A 32-year-old nonlactational women with a nipple piercing and previous oral-to-breast contact presented with findings consistent with mastitis and abscess, however, the patient failed multiple courses of empiric antimicrobials.
