Ultrastructural characterization and biochemical profile of human gross cystic breast disease.

Human gross cystic breast disease is a benign condition affecting about 7-10% of adult women occurring with the highest incidence in the premenopausal decade. Although breast cysts do not represent a preneoplastic condition per se, several studies indicate an increased breast cancer risk in women affected by this pathology. In this report we study 115 breast cystic fluid samples obtained by needle-aspiration from women with gross cystic breast disease. The samples were analysed biochemically and the cells contained therein were observed at the electron microscope. According to their biochemica…

Wolfe's mammographic patterns in women with gross cystic disease of the breast.

The epidemiologic determinants of the mammographic pattern were studied in 710 patients with aspirated gross (> 1 ml) cysts of the breast. The prevalence of the mammograms classified as P2-DY, that are considered to be associated with an increased breast cancer risk, was 636/710 or 89.6%. No relationship between mammographic patterns and characteristics of breast cyst fluid such as K+/Na+ ratio, apocrine changes and dehydroepiandrosterone sulfate concentration was observed. A significant decrease in the proportion of these patterns with increasing age (p = 0.006), Quetelet Index (p < 0.001), p…

Serum and intracystic levels of thyroxine (T4) and triiodothyronine (T3) in women with breast macrocysts.

In 15 patients with a total of 22 breast macrocysts serum and intracystic T4 and T3 and intracysts TSH were concomitantly radioimmunoassayed (using a technique with polyethylene glycol (PEG) for T3, T4, and a double antibody technique for TSH). While in most cysts (17/22) the T4 values were under those in the serum, in 21 cysts T3 was 2-18 times above the serum values. Intracystic TSH was at the lower limit of the normal serum TSH values whereas prolactin (PRL) and thyroglobulin (TGL) assayed in several cysts were not different from their serum value. It suggests that breast macrocysts are abl

Treatment of precocious pseudopuberty associated with follicular cysts of the ovary.

Two girls, aged 3 and 6 years, were treated for precocious pseudopuberty associated with a follicular ovarian cyst. Both patients had breast enlargement, areolar pigmentation, and elevated serum estradiol levels. In one girl, a normal displaced uterus was palpated; in the other, an ovarian cyst was identified by ultrasonography. Surgical treatment consisted of limited resection of the follicular cysts. Rapid regression of breast hypertrophy and return of estradiol levels to normal occurred in both children, who were followed for six and 1.5 years. We believe that a conservative operation that

Texture analysis of lesions in breast ultrasound images.

We investigate the use of Haralick's texture features and posterior acoustic attenuation descriptors (PAAD) for the characterization of ultrasound (US) breast lesions. 71 lesions (24 cyst, 21 benign solid mass and 26 malignant solid masses) were manually segmented on two-dimensional breast US images. 28 Haralick's descriptors and two PAAD were evaluated on these segmented lesions. Mean of Sum Average, Range of Sum Entropy and the second PAAD best discriminated cysts from noncysts. Range of Correlation and the second PAAD best discriminated solid malignant from benign lesions. PLASMINOGEN AND ANGIOSTATIN LEVELS IN FEMALE BENIGN BREAST LESIONS.

It is known that benign breast tissue exhibit relatively low angiogenic capacity. Activation of angiogenesis in mammary pre-malignant lesions could be associated with disease progression and high risk of transformation into the breast cancer. However, insight into the underlying molecular mechanisms involved in angiogenesis regulation in non-cancerous breast pathologies is still poorly defined. The purpose of the present study was to determine levels of plasminogen and its proteolytic fragments (angiostatins) in mammary dysplasia (mastopathy and breast cyst) and benign neoplasms (fibroadenomas…

Cystic breast masses and the ACRIN 6666 experience.

Masses due to cystic lesions of the breast are extremely common findings on mammography, ultrasonography, and magnetic resonance imaging. Although many of these lesions can be dismissed as benign simple cysts, requiring intervention only for symptomatic relief, complex cystic and solid masses require biopsy. Perhaps, the most challenging are complicated cysts, that is, cysts with internal debris. When the debris is mobile or a fluid-debris level is seen, complicated cysts can be dismissed as benign findings. As an isolated finding, homogeneous complicated cysts can be classified as probably be

Steroid patterns of benign breast disease.

We briefly review some biochemical aspects of benign breast disease (BBD), mainly focusing on free and conjugate estrogen content of breast cyst fluid (BCF), also in relation to cyst type. Evidence is reported that high K(+)-type I-cysts clearly associate with low Cl- levels and accumulate significantly higher quantities of dehydroepiandrosterone sulfate (DHAS) and estrone-3-sulfate (E1S). In spite of the limited number of cases, both increasing DHAS and E1S levels correlate with the increment of K+ to Na+ ratio. A positive correlation was also found between DHAS and E1S. Using electrochemical…

Interventional radiologic procedures in the investigation of lesions of the breast.

The interventional radiologist who performs mammography does far more than read films; he examines every patient, punctures cysts, and localizes other breast lesions for precise excisional biopsy. With specimen radiography, he can confirm that suspicious lesions are completely removed by the surgeon and that the tissue in question is selected by the pathologist for examination. Pneumocystography can save the patient the unnecessary, disfiguring surgical removal of benign cysts. Those patients requiring biopsy may be scheduled for the procedure at once, and close cooperation with the surgeon gr

Management of Cystic Conditions.

Cystic conditions are the most common disorder of the breast. Simple cysts are not malignant and do not require intervention. Patients with symptomatic simple cysts can undergo elective aspiration, and typical cyst fluid can be discarded. Bloody fluid should be sent for cytology. Cysts with thick walls, thick septations, or solid components have a risk of malignancy and should undergo biopsy.

Ultrasound-guided biopsy of nonpalpable breast masses by surgeons.

BACKGROUND: Recently, ultrasound (US)-guided needle biopsy has been proposed as an acceptable alternative to open biopsy in women with nonpalpable breast masses. This study evaluated the accuracy of US-guided needle biopsy of nonpalpable breast masses performed by surgeons at the time of the initial clinical examination. METHODS: Ultrasound-guided aspiration and/or core biopsy (US-GAB) was performed on 103 patients presenting with a nonpalpable, new, or increasing-size mass detected on mammography. Study patients included those whose US was classified as: fibroadenoma (FA) (n = 26), complex c

Fibrocystic breast disease.

Fibrocystic breast disease is characterized by breast pain, lumpiness or cysts. The few patients who have atypia on breast biopsy are at substantially increased risk for the development of breast cancer. Management of fibrocystic breast disease involves monitoring for breast cancer by repeated physical and mammographic examinations. Alleviation of symptoms may be attempted by reassurance, reduction of methylxanthine intake and treatment with danazol.

Surgical office-based ultrasound of the breast.

How surgical office-based ultrasound (US) influences the management of nonpalpable, new or increasing size, mammogram-detected breast masses was evaluated. Ninety-seven patients had diagnostic US only; of these, 67 had their mass diagnosed as a simple cyst, and 30 had a negative US. Fifty-three additional patients underwent US-guided aspiration and/or biopsy (US-GAB) of their breast mass after diagnostic US. Of 12 patients with diagnostic US of "simple cyst," US-GAB confirmed each to be cysts. Of five patients with diagnostic US of "fibroadenoma," four had fibroadenomas and one had insufficien

Fibroadenoma With Multiple Epidermal Inclusion Cysts Masquerading as Malignancy: An Underreported Entity With Review of Literature.

Fibroadenomas in postmenopausal women are quite uncommon, and so are epidermal inclusion cysts in the breast. The coexistence of both is very rare, and very limited literature is available about this occurrence. We report a postmenopausal female patient with complaints of breast lump masquerading as malignancy on triple assessment which upon subsequent histopathological evaluation was confirmed to be fibroadenoma with intervening multiple epidermal inclusion cysts. In addition, the epidermal inclusions cysts were seen within the fibroadenoma without any association with the overlying skin and

Plasma fatty acid profiles in benign breast disorders.

Breast pain (mastalgia) and macroscopic breast cysts present commonly. Mastalgia may be improved by dietary manipulation to reduce saturated fat or supplement essential fatty acid intake. Fatty acid profiles were measured in women with mastalgia and breast cysts, before and during treatment with evening primrose oil, a rich source of essential fatty acids. The fatty acid profiles of both groups of patients were abnormal, with increased proportions of saturated fatty acids and reduced proportions of essential fatty acids. Treatment with evening primrose oil improved the fatty acid profiles towa…

Breast macrocysts.

Clinical macrocysts occur in about 7% of adult women. Modern terminology and understanding of benign breast disorders considers microcysts and macrocysts as specific clinical and pathological entities. The terminology and concepts of fibrocystic disease and its many alternative terms have been discarded. Cysts arise as an aberration of normal lobular involution (ANDI) associated with active secretion of apocrine epithelium under hormonal stimulation. They are readily managed in most cases by aspiration alone. Multiple recurrent cysts may constitute a considerable nuisance and justify short-ter

Octreotide reduces hepatic, renal and breast cystic volume in autosomal-dominant polycystic kidney disease.

A 43-year-old woman with autosomal-dominant polycystic kidney disease (ADPKD) received octreotide for 12 months, and this was associated with a 6.3% reduction in liver volume, an 8% reduction in total kidney volume and stabilization of renal function. There was also a reduction of cyst size in fibrocystic disease of breast. These data suggest that the cyst fluid accumulation in different organs from patients with ADPKD is a dynamic process which can be reversed by octreotide. This is the first report of a case of simultaneous reduction in hepatic, renal and breast cystic volume with preservati…

The hybrid epidermoid and apocrine cyst. A combination of apocrine hidrocystoma and epidermal inclusion cyst.

We describe four cases of unusual superficial cutaneous cysts that have a lining epithelium composed of apocrine cells immediately adjacent to keratinizing squamous epithelium with an intact granular layer. Two of the cysts were on the nipple of the breast and two of the cysts were on the face. The lumina of all four cysts contained keratin. Immunoperoxidase staining for carcinoembryonic antigen in the facial cysts showed positive staining within the apocrine cells of the lining epithelium in one case. The pathogenesis of these lesions is uncertain. We suggest that these unusual cysts be calle
