Direct detection of respiratory syncytial (RS) virus antigen in nasopharyngeal secretions (NPS) provides the most rapid diagnostic test for RS infection, but more sensitive methods might be more beneficial in the study of virus shedding. RS virus RNA was extracted from cells stored at -70 degrees C either in suspension with added RNAse inhibitor or as a pellet without inhibitor. The RNA was reverse transcribed, the resultant cDNA amplified by the polymerase chain reaction and detected by ethidium bromide staining after electrophoresis through agarose gel (RT-PCR). Of 217 specimens tested, 106

About 1% of infants are admitted to hospital with acute bronchiolitis; 85% of cases are caused by infection with Respiratory Syncytial Virus (RSV). The pathophysiological changes during the acute illness are inflammatory obstruction in the small airways with submucosal cellular infiltration, epithelial necrosis and mucous plugging; FRC increases and dynamic compliance falls. Failure to respond to bronchodilator drugs suggests that muscle spasm contributes relatively little to the airway narrowing. Affected infants become increasingly dyspnoeic and hypoxic for 3-4 days then spontaneously improv

Acute bronchiolitis (AB) is a common lung disease in infants manifested clinically by dyspnea and wheezing. The purpose of this study was to measure simultaneous plasma levels of histamine and a stable prostaglandin F2 alpha metabolite [13,14-dihydro-15-keto-PGF2 alpha (PG metabolite)], by radioenzymatic and radioimmunoassays, respectively, during and after recovery from AB. Blood was obtained from 15 infants during AB and from 14 and 9 of these infants when re-evaluated 6 and 18 months later, respectively. Mean (+/- 1 SEM) pre- and posttherapy (inhaled isoetharine) histamine levels (pg/ml), 1

Routine administration of antibiotics in the treatment of pneumonia and bronchiolitis in infants and small children was evaluated in an open randomised prospective trial. From 1979-82 136 children between the age of 1 month and 6 years were allocated to one of two treatment groups shortly after their admission to a paediatric ward. Group A patients were to be given antibiotics but those in group B were not. None of the children had received antibiotics before hospital admission. A viral infection was diagnosed in 38 of the 72 patients from group A and in 34 of the 64 patients from group B. Res

An unusual, severe pneumonia probably caused by varicella-zoster virus is reported in a 19-year-old previously healthy man. The diagnosis was based on high titer of varicella-zoster antibodies in serum, and demonstration of varicella-zoster antigen from lung biopsy specimen. The uncommon feature in the pathophysiological course of the disease was the selective hypercarbia that responded well to bronchodilator therapy with theophylline. Furthermore, the patient had no skin manifestations during his illness.

To assess bronchial reactivity in children who have had bronchiolitis, we studied 48 children by challenging them with methacholine nine or ten years after admission to hospital with bronchiolitis. Pulmonary function was also evaluated. Fifty-seven percent of children studied had bronchial hyperreactivity. Thirty-three percent of first-degree relatives of those with a positive MCH challenge had a positive response. There was a significant correlation between the occurrence of a positive MCH challenge and a history of recurrent bronchiolitis. Pulmonary function tests demonstrated lower flow rat

OBJECTIVE: Combined diagnoses of bronchiolitis and asthma can create challenges in de-implementation in accordance with guidelines aiming to reduce low value care. We sought to quantify the cost and resource utilization associated with combined diagnoses of asthma and bronchiolitis in young children, hypothesizing that combined diagnoses are associated with increased cost.
METHODS: This was a retrospective cohort study of respiratory hospitalizations of children under 36 months of age in the Pediatric Health Information System during 2023-2024. Encounters were identified by ICD-10 codes and g

OBJECTIVES: To determine the demographic and clinical predictors of guideline-discordant care (GDC) that children with uncomplicated bronchiolitis receive in community emergency departments (EDs) and to explore the association between GDC and ED length of stay (LOS) for children who meet the criteria for supportive care alone.
STUDY DESIGN: We conducted a retrospective observational study from 2017 to 2024 of ED patients 28 days to 2 years old lacking comorbidities with bronchiolitis. All children were discharged home from the 21 community EDs in our Northern California health system. Our pri

IMPORTANCE: Viral bronchiolitis is the leading cause of nonelective admission to the PICU. Guidelines recommend management that focuses on supportive care. Evidence suggests that inhaled epinephrine may reduce lower airways resistance and work of breathing in critical bronchiolitis. It has been demonstrated at one institution that it is feasible to administer continuous inhaled epinephrine (CIE) via heated high-flow nasal cannula (HFNC).
OBJECTIVES: To describe the demographic and clinical characteristics of patients with bronchiolitis who received CIE and assess their physiologic response to

Post-Infectious Bronchiolitis Obliterans (PIBO) is a rare chronic obstructive lung disease characterized by irreversible airflow limitation due to inflammation, fibrosis, and obliteration of the small airways following severe lower respiratory tract infection. Adenovirus is the most frequently associated pathogen, followed by other viruses and atypical bacteria. PIBO pathogenesis appears to result from a complex interaction between severe epithelial injury, dysregulated immune response, persistent neutrophilic inflammation, abnormal tissue repair, and genetic susceptibility. Epithelial damage

Bronchiolitis is the leading cause of hospital presentation and admission for infants in Australasia. We aimed to synthesise current evidence on the effect of discharge criteria for infants (aged < 12 months) who are presenting to or are admitted to hospital with bronchiolitis, to inform a binational guideline recommendation update. Systematic searches were conducted on MEDLINE, EMBASE, PubMed, Cochrane Library and CINAHL (last search 19 February 2025) for non-randomised studies evaluating hospital discharge criteria in bronchiolitis. The primary outcomes were length of stay (LOS) and readmiss

Acute bronchiolitis is a leading cause of pediatric hospital admissions, particularly during winter months, placing significant pressure on healthcare systems. Currently, the BROSJOD scale allows for severity stratification based on clinical variables. However, the incorporation of biomarkers could improve predictive capacity. The main purpose of this study is to analyze whether adrenomedullin (MR-proADM) can serve as a predictor of severity and prognosis in patients with bronchiolitis. A single-center, prospective, observational cohort study was conducted over two consecutive epidemic periods

INTRODUCTION: Respiratory syncytial virus (RSV) is the leading cause of bronchiolitis and hospitalization in infants. Since 2023, France has implemented two preventive strategies aimed at reducing RSV burden: long-acting monoclonal antibody nirsevimab and RSV vaccine for pregnant women, the latter becoming available from September 2024. This study aimed to assess the impact of RSV preventive strategies on the clinical severity and viral epidemiology of bronchiolitis in hospitalized infants.
MATERIAL AND METHODS: We conducted a retrospective study at Armand Trousseau University Hospital, inclu

BACKGROUND AND OBJECTIVE: Bronchiolitis is the most common respiratory illness in children younger than 2 years. Although pharmacotherapy has not been shown to alter mild disease course, overuse continues in the primary care setting. We sought to improve the management of bronchiolitis in a large primary care network by reducing bronchodilator use from 24% to 15% while maintaining steroid use at 7% within 12 months.
METHODS: Our primary care network is composed of 39 clinics in rural and urban settings across 3 states. Interventions through 4 Plan-Do-Study-Act cycles included (1) stakeholder

UNLABELLED: Vitamin D's role in immune system regulation and its contribution to host defenses against respiratory infections may have implications for bronchiolitis pathophysiology. This cross-sectional study aimed to determine the association between the clinical severity of bronchiolitis and serum vitamin D levels in infants. Infants aged 1 month to 12 months, diagnosed with bronchiolitis, and healthy controls attending routine immunization were enrolled. Baseline characteristics were recorded, including clinical details, bronchiolitis severity, and course during hospital stay. Bronchioliti

Bronchiolitis is a viral lower respiratory tract infection primarily affecting children younger than 2 years; a common cause of health care encounters, including hospitalization; and a considerable economic burden for health care systems in the United States and worldwide. The American Academy of Pediatrics (AAP) most recently updated its bronchiolitis guideline in 2014 and reaffirmed supportive care as the mainstay of treatment. Despite these recommendations, there is still significant variability in care provided for these children, especially in bronchodilator usage, radiography, and high-f
