BACKGROUND: This case report presents a patient diagnosed with sick sinus syndrome who was successfully treated with permanent His-bundle pacing (PHBP).
CASE PRESENTATION: A 36-year-old man was transferred to our hospital due to recurrent syncope. He was diagnosed with sick sinus syndrome based on the 24-h Holter and a history of syncope. He was admitted to hospital and successfully treated with PHBP. The postoperative examination showed that the pacing rhythm, pacemaker pacing and perception function were normal. He was discharged without any complications after a successful pacemaker implan

A woman with ischemic cardiomyopathy presented with recurrent syncope. Electrocardiogram showed complete heart block and torsade de pointes (TdP) secondary to amiodarone, recently started for paroxysmal atrial fibrillation. We describe a novel application of His bundle pacing that suppressed TdP and corrected the underlying left bundle branch block. (Level of Difficulty: Intermediate.).

BACKGROUND: Cardiac resynchronization therapy (CRT) typically is attempted with biventricular pacing (BiVP). One-third of patients are nonresponders. His-bundle pacing (HBP) has been evaluated as an alternative means of effecting CRT because it generates truly physiologic ventricular activation, as evidenced in part by the morphologic identity between normally conducted and paced QRS complexes.
OBJECTIVE: The purpose of this study was to assess the feasibility of, and clinical response to, permanent HBP as an alternative to BiVP in CRT-indicated patients.
METHODS: Patients were implanted wit

Atrioventricular (AV) nodal reentrant tachycardia is the most common form of paroxysmal supraventricular tachycardia in adults, and slow AV nodal pathway ablation has evolved into first-line therapy. Variations in conduction system anatomy are occasionally found at electrophysiological study and may make this ablation procedure very challenging. Here, we present the case of a 69-year-old man with a structurally normal heart and posterior displaced His bundle who underwent successful slow pathway ablation. Demonstration of the characteristic slow pathway recording and His bundle electrogram is

Spike potentials of the sinus node and of the atrial conductivity pathways were recorded by way of heart microcatheterization via the subclavian vein in 40 canine experiments and in 307 patients, using the obtained data for a more precise interpretation of the bundle of His electrogramme taken in the course of these examinations. The importance of the density of the spikes for their successful recording was studied, and it was found that the onset of the atrial A--wave preceeded that of the P-wave by 10-30 ms on ECG tracings recorded from the body surface. The atrial A--complex recorded with t…

BACKGROUND: Although right ventricular pacing (RVP) is recommended by most of the guidelines for atrioventricular block, it can cause electrical and mechanical desynchrony, impair left ventricular function, and increase the risk of atrial fibrillation. Recently, the His-Purkinje system pacing, including His bundle pacing (HBP) and left bundle branch pacing (LBBP), has emerged as a physiological pacing modality. However, few studies have compared their efficacy and safety in atrioventricular block (AVB).
METHODS AND RESULTS: The PubMed, Web of Science, Cochrane Library, and ScienceDirect datab

Ventricular premature contractions are often located in the left ventricular outflow tract (LVOT). Components of the normal atrioventricular conduction apparatus are located just below the aortic valve in proximity to the membranous septum, between the noncoronary cusp and right coronary cusp. We present a case of injury to the bundle of His during an ablation of a ventricular premature contraction in the LVOT below the left coronary cusp, remote from the bundle of His, due to pressure from the proximal shaft of the catheter between the noncoronary cusp and the right coronary cusp.

BACKGROUND: His bundle pacing (HBP) is a recently developed pacing technique that can achieve an ideal physiological pattern of ventricular activation via stimulation of the native His-Purkinje system. Despite the widespread introduction of HBP in clinical practice, its appropriate indications are yet to be determined clearly. Moreover, the efficacy and safety of HBP and long-term prognosis of patients undergoing such are unknown.
METHODS: We conducted a multicenter observational prospective study in patients undergoing HBP in Japan. Patients with atrioventricular block or conduction delay an

OBJECTIVE: To assess and compare the clinical outcomes of His bundle pacing (HBP) versus right ventricular pacing (RVP) in patients who develop conduction disturbances following transcatheter aortic valve replacement (TAVR).
METHODS: In this retrospective study, 120 patients who developed CD following TAVR were enrolled, and were implanted with HBP or RVP between January 2015 and December 2024. To adjust for variations in initial risk factors and baseline characteristics between patients who underwent HBP or RVP, we employed the propensity score matching. Each patient was matched in a 1:1 rat

Four hundred patients with atrial septal defect treated surgically were reviewed. Thirty five (8.7%) developed arrhytmias post-surgery which persisted for over a year. Sinus bradycardia was found in 10 patients, nodal rhythm in 21, and atrial fibrilation and flutter in 4 patients. Thirty five per cent of the patients with late arrhythmias developed related symptomatology. In 14 patients the function of the sinus node was studied with electrical stimulation of the atrium and with His registry. The interatrial conduction time, AV node and His Purkinje were analized employing various stimulation

The effects of atropine on normal and anomalous A-V pathways were investigated using HBE in 40 cases consisting of 25 subjects without pre-excitation (group I) and 15 cases with WPW syndrome (group II). In group I, A-H interval shortened with a mean decrease of 30% following the administration of atropine (p less than 0.001), while P-A and H-V intervals as well as QRS duration remained unchanged. In group II the electrocardiogram showed WPW syndrome pattern (pattern W) in 12, normalization (pattern N) in 1 and both patterns (pattern WN) in 2 cases during the control study. In 10 cases pattern

OBJECTIVES: The purpose of this study was to investigate whether heart failure patients with narrow QRS duration (or right bundle branch block) but with long PR interval gain acute hemodynamic benefit from atrioventricular (AV) optimization. We tested this with biventricular pacing and (to deliver pure AV shortening) direct His bundle pacing.
BACKGROUND: Benefits of pacing for heart failure have previously been indicated by acute hemodynamic studies and verified in outcome studies. A new target for pacing in heart failure may be PR interval prolongation, which is associated with 58% higher mo

INTRODUCTION: In patients with sinus node disease (SND), the dual-chamber pacemaker (PM) is programmed in DDDR mode with an algorithm to avoid unnecessary right ventricular (RV) pacing. This pacing mode may prolong PR interval with consequently atrioventricular (AV) asynchrony which is associated with a higher risk of atrial fibrillation (AF). We evaluate whether preserving AV synchrony by setting a fixed AV delay during physiological RV pacing, that is, His bundle pacing (HBP), could reduce the risk of AF occurrence in comparison with a standard pacing mode with an algorithm to avoid unnecess

BACKGROUND: Atrioventricular block (AVB) is common after transcatheter aortic valve replacement (TAVR) and permanent pacemaker (PPM) implantation is needed in up to 30% of patients. Main predictors of long term AVB are electrocardiographic. The purpose of this study is to assess the prognostic value of serial HV intervals measured before and after TAVR to shorten the timing of PPM implantation.
METHODS: His bundle recordings were performed before (HV1), immediately after TAVR (HV2) and at day 2 for Edwards Sapien (ES) and 5 for Medtronic CoreValve (CV) (HV3). PPM indications were high degree
