Development and Validation of Predictive Models of Cardiac Mortality and Transplantation in Resynchronization Therapy.

BACKGROUND: 30-40% of cardiac resynchronization therapy cases do not achieve favorable outcomes. OBJECTIVE: This study aimed to develop predictive models for the combined endpoint of cardiac death and transplantation (Tx) at different stages of cardiac resynchronization therapy (CRT). METHODS: Prospective observational study of 116 patients aged 64.8 ± 11.1 years, 68.1% of whom had functional class (FC) III and 31.9% had ambulatory class IV. Clinical, electrocardiographic and echocardiographic variables were assessed by using Cox regression and Kaplan-Meier curves. RESULTS: The cardiac mort…

Vectorcardiographic QRS area as a novel predictor of response to cardiac resynchronization therapy.

BACKGROUND: QRS duration and left bundle branch block (LBBB) morphology are used to select patients for cardiac resynchronization therapy (CRT). We investigated whether the area of the QRS complex (QRSAREA) on the 3-dimensional vectorcardiogram (VCG) can improve patient selection. METHODS: VCG (Frank orthogonal lead system) was recorded prior to CRT device implantation in 81 consecutive patients. VCG parameters, including QRSAREA, were assessed, and compared to QRS duration and morphology. Three LBBB definitions were used, differing in requirement of mid-QRS notching. Responders to CRT (CRT-R…

Effect of cardiac resynchronization therapy on pulmonary function in patients with heart failure.

Pulmonary congestion due to heart failure causes abnormal lung function. Cardiac resynchronization therapy (CRT) is a proven effective treatment for heart failure. The aim of this study was to test the hypothesis that CRT promotes increased lung volumes, bronchial conductance, and gas diffusion. Forty-four consecutive patients with heart failure were prospectively investigated before and after CRT. Spirometry, gas diffusion (diffusing capacity for carbon monoxide), cardiopulmonary exercise testing, New York Heart Association class, brain natriuretic peptide, the left ventricular ejection fract…

The Contemporary Role of Echocardiography in Improving Patient Response to Cardiac Resynchronization Therapy.

Cardiac resynchronization therapy (CRT) is an important therapy for heart failure patients with widened electrocardiographic QRS complexes and depressed ejection fractions, however, approximately one-third do not respond. This article presents a practical contemporary approach to the utility of echocardiography to improve CRT patient response by assessing mechanical dyssynchrony, optimizing left ventricular lead positioning, and performing appropriate echo-Doppler optimization, along with future potential roles. Specifically, recent long-term outcome data are presented that demonstrates that b…

Cardiac resynchronization therapy and the relationship of percent biventricular pacing to symptoms and survival.

BACKGROUND: With the advent of cardiac resynchronization therapy, it was unclear what percentage of biventricular pacing would be required to obtain maximal symptomatic and mortality benefit from the therapy. The optimal percentage of biventricular pacing and the association between the amount of continuous pacing and survival is unknown. OBJECTIVE: The purpose of this study was to assess the optimal percentage of biventricular pacing and any association with survival in a large cohort of networked patients. METHODS: A large cohort of 36,935 patients followed up in a remote-monitoring networ…

Cardiac resynchronization therapy as an adjunct to conventional surgical treatment for heart failure.

From March 2004 through October 2007, we prospectively evaluated the benefits of cardiac resynchronization therapy as an adjunct to conventional procedures in patients who were undergoing surgery for heart failure.Twenty severely symptomatic patients (14 men and 6 women, with a mean age of 70 +/- 8 years) who displayed advanced cardiomyopathy, QRS duration > or =130 ms, or mechanical dyssynchrony, underwent isolated or combined coronary artery revascularization and mitral valve overreduction. In all patients, an epicardial lead was secured to the left ventricular wall at the end of the procedu…

Sleep-disordered breathing in heart failure and the effect of cardiac resynchronization therapy.

Respiratory disturbances during sleep are common in patients with heart failure (HF) and can trigger the occurrence of sleep apnoea or deteriorate pre-existing breathing disorder. This in turn may lead to worsening of the HF itself. Optimal treatment for HF has been found to reduce respiratory disturbances during sleep significantly, whereas cardiac resynchronization therapy (CRT), achieved by biventricular pacing, appears to cause a further reduction in episodes of central type apnoea, although it may also have an effect on episodes of obstructive type. The beneficial effect of CRT is due to …

Computer model for the optimization of AV and VV delay in cardiac resynchronization therapy.

An optimal electrode position, atrio-ventricular (AV) and interventricular (VV) delay in cardiac resynchronization therapy (CRT) improves its success. An optimization strategy does not yet exist. A computer model of the Visible Man and a patient heart was used to simulate an atrio-ventricular and a left bundle branch block with 0%, 20% and 40% reduction in interventricular conduction velocity, respectively. The minimum error between physiological excitation and pathology/therapy was automatically computed for 12 different electrode positions. AV and VV delay timing was adjusted accordingly. Th…

Prospective validation of stress echocardiography as an identifier of cardiac resynchronization therapy responders.

BACKGROUND: Cardiac resynchronization therapy (CRT) provides benefit for congestive heart failure (CHF), but predictors of the clinical response are debated. OBJECTIVE: The aim of this prospective study was to assess the predictive role of dobutamine stress echocardiography (DSE) in identifying a suitable candidate for CRT. METHODS: From March 2001 to December 2003, 71 CHF patients were prospectively enrolled on the basis of four criteria: New York Heart Association (NYHA) class III and IV; QRS > or =150 ms with a left bundle branch block pattern, and left ventricular ejection fraction (LVEF…

Outcome of Patients With Cardiac Sarcoidosis Who Received Cardiac Resynchronization Therapy: Comparison With Dilated Cardiomyopathy Patients.

INTRODUCTION: Cardiac resynchronization therapy (CRT) has been shown to be effective for patients with chronic heart failure; however, the efficacy of CRT in patients with cardiac sarcoidosis (CS) has not been established. METHODS: We compared the outcomes of patients with CS who received CRT to patients with dilated cardiomyopathy (DCM). The incidence of major adverse cerebral and cardiovascular events (MACCE) in 11 consecutive CS patients (8 females; mean age, 66 ± 8.0 years) who received CRT were compared with 29 DCM patients (9 females; mean age, 70 ± 8.9 years). RESULTS: Females and pat…

Effect of cardiac resynchronization therapy on cardiac sympathetic nervous dysfunction and serum C-reactive protein level.

BACKGROUND: Cardiac autonomic dysfunction is associated with a poor prognosis in patients with heart failure (HF). Systemic inflammation is elevated in patients with HF. We hypothesized that cardiac resynchronization therapy (CRT) improves cardiac sympathetic nervous dysfunction and systemic inflammation. To test our hypothesis, we evaluated cardiac sympathetic activity and serum levels of high sensitive C-reactive protein (hs-CRP) before and after CRT. METHODS: Twenty-seven patients with chronic HF (19 men, eight women; mean age 67 ± 10 years) with nonischemic cardiomyopathy who underwent CR…

Cardiac Resynchronization Therapy Associated With Improved Survival in Advanced Transthyretin Cardiac Amyloidosis Despite Higher Disease Severity.

Transthyretin cardiac amyloidosis (ATTR-CM) frequently affects native conduction, necessitating permanent pacemaker (PPM), implantable cardioverter-defibrillator, or cardiac resynchronization therapy (CRT) implantation. This study characterizes right ventricular pacing (RVP) burden in ATTR-CM patients and evaluates its impact on left ventricular ejection fraction (LVEF), heart failure (HF) rehospitalization, and survival. Retrospective chart review of ATTR-CM patients requiring device implantation (4/2012 to 9/2022). Patients were categorized by pacing burden: low RVP (pacing burden <40%, N = …

Cardiac efficiency and oxygen consumption measured with 11C-acetate PET after long-term cardiac resynchronization therapy.

UNLABELLED: Cardiac resynchronization therapy (CRT) is a treatment option in patients with severe heart failure and left bundle-branch block (LBBB). This study evaluated the effects of 4 and 13 mo of CRT on myocardial oxygen consumption (MVO2) and cardiac efficiency as compared with mild heart failure patients without LBBB. METHODS: Sixteen patients with severe heart failure and LBBB due to idiopathic cardiomyopathy were studied at baseline and after 4 and after 13 mo of therapy. Thirteen patients with mild heart failure without LBBB served as a comparison group. The clearance rate (k2) of 11…

Role of Atrial Pacing Support in Cardiac Resynchronization Therapy: A Noninferiority Randomized Trial.

BACKGROUND: The role of atrial pacing support is unclear in patients receiving cardiac resynchronization therapy-defibrillator (CRT-D) without sinus node dysfunction. METHODS: We conducted a randomized, parallel-group, noninferiority trial to evaluate whether a 2-lead CRT-D capable of atrial sensing (but no pacing) by a floating dipole on the right ventricular lead (CRT-DX) is not inferior to a 3-lead CRT-D with a conventional atrial lead. Between October 17, 2018, and March 5, 2024, a total of 636 patients (68±10 years old, 28.6% women) with standard CRT-D indication, optimized medical thera…

Cardiac Resynchronization Therapy and Circulating Metabolomic Profile in Patients With Advanced Heart Failure.

BACKGROUND: It is unknown whether cardiac resynchronization therapy (CRT) improves systemic metabolomic profile by enhancing left ventricular function. The observational cohort study aimed to investigate the effect of CRT on left ventricular ejection fraction (LVEF) and circulating plasma metabolites in patients who had heart failure with reduced LVEF. METHODS: We prospectively screened patients with ischemic cardiomyopathy and nonischemic cardiomyopathy who received CRT with a defibrillator for LVEF ≤35% according to current guidelines. Clinical assessment included echocardiography and devic…
