Treatment of knee cartilage defect in 2010.

Treatment of knee cartilage defect, a true challenge, should not only reconstruct hyaline cartilage on a long-term basis, but also be able to prevent osteoarthritis. Osteochondral knee lesions occur in either traumatic lesions or in osteochondritis dissecans (OCD). These lesions can involve all the articular surfaces of the knee in its three compartments. In principle, this review article covers symptomatic ICRS grade C or D lesions, depth III and IV, excluding management of superficial lesions, asymptomatic lesions that are often discovered unexpectedly, and kissing lesions, which arise prior…

Management of articular cartilage defects of the knee.

Articular cartilage has a poor intrinsic capacity for healing. The goal of surgical techniques to repair articular cartilage injuries is to achieve the regeneration of organized hyaline cartilage. Microfracture and other bone marrow stimulation techniques involve penetration of the subchondral plate in order to recruit mesenchymal stem cells into the chondral defect. The formation of a stable clot that fills the lesion is of paramount importance to achieve a successful outcome. Mosaicplasty is a viable option with which to address osteochondral lesions of the knee and offers the advantage of t…

[Space-occupying lesion of the thyroid cartilage].

A 46-year-old patient had had a slowly growing progressive but painless prelaryngeal space-occupying lesion for approximately 1 year. In addition there was also a longstanding hyperuricemia with gout tophi on the metatarsal basal joints 1-5 of both hands. The extirpated tumor proved to be a gout tophus by histological examination. Although this is a rare occurrence it should be considered as the possible cause of a tumor if there is a corresponding case history.

Molecular network of cartilage homeostasis and osteoarthritis.

This review article presents the current understanding of the molecular basis of articular cartilaginous homeostasis, and outlines potential areas to focus on within the developing field of therapeutics for cartilage disorders. Articular cartilage, an integral component of joints in extremities and the vertebral column, is essential for locomotion. Disturbance of joint development or cartilage homeostasis causes congenital osteocartilaginous dysplasia or osteoarthritic diseases, respectively. Symptomatic treatments and surgical replacement of joints are effective but can also be problematic in…

Cell death and apoptosis in osteoarthritic cartilage.

Osteoarthritis (OA) is the most common chronic joint disease in the elderly population, causing significant pain and disability. Because the cardinal feature of OA is a progressive loss of articular cartilage, a great portion of the research endeavour into the pathogenesis of OA has been focused on the regulation of matrix synthesis and degradation. The phenotypic stability and survival of the chondrocytes are essential for the maintenance of a proper cartilage matrix. This has lead to the long-standing assumption that cell death is a central feature in OA cartilage degeneration. The important…

[Cartilage cell transplantation as an alternative to endoprosthesis].

The incidence of cartilage lesions is high. Due to pain and loss of function, long-term therapy is often necessary. Isolated, full-thickness articular cartilage lesions with a diameter of 4 cm2 are indications for chondrocyte transplantation. Many outcome studies report good long-term results. In recent years, the cultivation of chondrocytes has changed. Histologic investigations show hyalin-like cartilage after transplantation. Large cartilage lesions facing each other are a therapeutic dilemma in young patients. Implanting unicondylar or bicondylar prostheses is sometimes the last resort for…

Surgical repair of cartilage defects of the patella.

The structure and biomechanical forces on the patellar joint challenges researchers to define an ideal method for resurfacing the patellar cartilage. The articular surface of the patella presents variability between individuals, and has various minor articulations that bear partial or total compressive, shear, and combined forces during movement. Surgical techniques for the repair of patellar cartilage defects have evolved from cumulative advances in basic science and technology. Such surgeries include the techniques that promote either fibrocartilage formation or hyalinelike cartilage formati…

Ultrastructure of the vocal process of the arytenoid cartilage.

An investigation of the ultrastructure of the vocal process of the arytenoid cartilage was performed on excised human adult larynges. The results are summarized as follows. 1) The size of the chondrocytes is small and their density is high at the tip of the vocal process. 2) The size of the chondrocytes increases and their density decreases toward the posterior portion of the vocal process. 3) At the tip of the vocal process, the collagenous fibers are thin and run in various directions. 4) The thickness of the collagenous fibers increases and they form thick bundles toward the posterior porti…

Cartilage and bone morphology in osteochondritis dissecans.

Osteochondritis dissecans (OD) is a syndrome that can be characterized as a non-infectious disturbance of enchondral ossification or as a post-traumatic event. OD occurs in the joint cartilage and physis of long bones, as well as in the talus or metacarpal head especially of young athletes. The medial femoral condyle is the most commonly affected site. The causes of osteochondritis dissecans are poorly understood. Thirty human osteochondral cylinders from patients (aged 16-44 years) with osteochondritis dissecans of the medial femoral condyle (grades IV and V according to Rodegerdts and Gleiss…

[Magnetic resonance of the cartilages of the large joints].

MRI of the articular cartilage requires a careful technical approach since this structure is very thin, with a peculiar internal architecture between the supporting matrix and the cell component. On MR images the normal articular cartilage has a zonal appearance. To optimize the variables for best visualization of the internal architecture of the hyaline articular cartilage, an ex vivo and in vivo study was carried out. Accurate T1 and T2 relaxation times of the articular cartilage were obtained with a particular mixed sequence and then used to create isocontrast intensity graphs. The latter a…

Compensation and decompensation of articular cartilage in osteoarthritis.

Although contemporary descriptions of the pathology of osteoarthritis emphasize the loss of articular cartilage, in the earlier stages a marked increase in synthetic activity of the chondrocytes can lead to an increase in proteoglycan concentration and commonly, to cartilage hypertrophy. Thus, osteoarthritis exhibits a biphasic course, with an initial "compensatory" phase, during which homeostatic mechanisms may maintain a reasonable articular surface until the second phase, "decompensation" (joint failure), develops. Salicylates interfere with homeostatic repair mechanisms and can markedly tr…

Autoantibodies to cartilage collagens in relapsing polychondritis.

Relapsing polychondritis is a systemic disease associated with a destruction of cartilage in various parts of the body. Sera from six patients with relapsing polychondritis and one patient with microscopic polyarteritis nodosa as well as from six controls were analyzed by immunoblotting and ELISA. All patients had autoantibodies against native collagens II and IX. The serum from one patient showed a strong reaction with all three collagen chains of the high molecular weight fraction of collagen IX after denaturation; sera from four patients showed autoantibodies against alpha 2 (XI) and sera f…

Mineral-matrix interactions in bone and cartilage.

Mineral-matrix interactions regulate the process of hydroxyapatite formation in bones and teeth. In mineralizing tissues, many anionic macromolecules bind to mineral. By means of this binding, such molecules are able to regulate the size and shape of the mineral crystals, determine the site of initial crystal deposition, and determine the type of mineral crystals deposited. Collagen, which provides a template for hydroxyapatite deposition; extracellular matrix vesicles, which provide a protected environment for crystal deposition; and noncollagenous matrix proteins that have high affinities fo

Biochemistry of articular cartilage in health and disease.

Articular cartilage covers the ends of long bones in synovial joints, providing smooth articulation and cushioning of the underlying bone during joint movement. The tissue can be viewed as a viscoelastic, composite material composed of collagen type II (and smaller amounts of other collagens) entrapping compressed (underhydrated) proteoglycan aggregates which generate a high osmotic/swelling pressure. This abundant extracellular matrix (ECM) is synthesized and turned over by relatively few cells, the chondrocytes. These cells produce a compartmentalized ECM, the components of which are heterog…

Malignant meningioma with cartilage and giant cells.

An autopsy case of recurrent and malignant meningioma is reported. This case was originally typical benign transitional meningioma of the falx, however, the histology of the tumor changed to show malignant features during successive recurrences. At autopsy, the tumor revealed findings consistent with malignant meningioma. One of the most interesting features was the presence of cartilage and giant cells in some parts. Immunohistochemistry showed positive immunoreactivity for S-100 protein in some cartilage and giant cells and for cytokeratin in some giant cells. Multidifferential potential of …

Perichondral grafting for cartilage lesions of the knee.

Twenty-five patients with 30 chondral lesions of the knee were treated with an autogenous strip of costal perichondrium. The graft was fixed … here was no decrease. In 28 cases the defect was completely filled with tissue resembling articular cartilage. We conclude that in most cases perichondral arthroplasty of cartilage defects of the knee gives excellent results.

Mechanism of the reduction of Meckel's cartilage in man.

The mechanism of reduction of the anterior end of Meckel's cartilage was studied in human embryos, with the following findings: 1. Meckel's cartilage is surrounded, from the outside and from below, by newly formed mandibular bone over the extent of the insertion of the musculus mylohyoideus. 2. Blood vessels from the newly formed bone penetrate Meckel's cartilage and break it down in the same way as in enchondral ossification of cartilaginous models of other bones. 3. The anlagen of the musculus mylohyoideus and musculus genioglossus are at first inserted on Meckel's cartilage; further muscle …

A study of cartilage development in pulmonary hypoplasia.

To discriminate between different forms of pulmonary hypoplasia (PH), 24 hypoplastic lungs were studied for their development of bronchial cartilage plates and peripheral air spaces. In 6 lungs from premature infants with oligohydramnios, normal amounts of immature and irregularly shaped cartilages were distributed with mitoses concentrated toward the periphery of the bronchi. Pulmonary acini appeared markedly immature. In 5 lungs from infants with diaphragmatic hernia, large numbers of cartilage bars were clustered around the proximal bronchi, whose branching was much reduced. Peripheral air …

[Traumatic cartilage damage of the knee joint].

Thirty-five traumatic lesions to cartilage were recorded, together with other injuries, from 225 arthroscopic examinations in two years. Most of the lesions (20) were located at the femoral condyle. The patella was involved in eight cases. However, if the patellar surface was related to the surface of the femoral condyles, the former was just as strongly affected by traumatic effects as the latter. No unambiguous localisation was possible in six cases, most of them smaller cartilage abrasions. Optional therapy proved to be applicable via the arthroscope in seven cases, while small incisions in…
