Mesenteric steal syndrome as an unusual manifestation of catastrophic antiphospholipid syndrome.

Catastrophic antiphospholipid syndrome is characterized by rapidly progressive multiorgan thrombosis. We report a 50-year-old man with triple-positive antiphospholipid syndrome who presented with superior vena cava syndrome, complete infrarenal aortic occlusion, and mesenteric steal syndrome. Preoperative management included plasmapheresis and systemic anticoagulation. The patient underwent open infrarenal aortic thrombectomy with aorto-aortic bypass and inferior mesenteric artery reimplantation, achieving complete clinical resolution. Histopathology revealed acute and chronic thrombosis witho

A retrospective study of catastrophic pelvic fractures in California Thoroughbred racehorses.

Here, we describe and characterize catastrophic pelvic fractures in Thoroughbred racehorses in California. We reviewed retrospectively racehorse autopsy reports from horses that died spontaneously or were euthanized because of complete pelvic fractures in California between 1992 and 2018. The exercise history of affected horses was compared with eventmatched control horses. Catastrophic pelvic fractures were sustained by 133 horses. Pelvic fractures were 2.7% of all musculoskeletal fatalities. Pelvic fractures occurred during training (48.0%), racing (35.0%), and non-exercise (17.0%). Horses ≤

Catastrophic antiphospholipid syndrome complicated with multiple infections: a case report.

BACKGROUND: Catastrophic antiphospholipid syndrome (CAPS) is a rare and life-threatening manifestation of antiphospholipid syndrome (APS), which is frequently later diagnosed with high mortality. CASE REPORT: A 54-year-old woman with a history of livedo reticularis presented with fever for four days, hematuria for two days, and impaired consciousness and petechiae for one day. Laboratory tests indicated thrombocytopenia, anemia, elevated indirect bilirubin and lactate dehydrogenase levels, as well as elevated free hemoglobin, isolated C3d positivity on direct antiglobulin test, and positive l

Catastrophic iatrogenic abdominal aorta haemorrhage during anterior resection.

A woman in her 70s with hypertension, breast cancer and diverticulosis underwent laparoscopic anterior resection for a tubule-villous adenoma, converted to open Hartmann's with aorto-bi-iliac bypass due to a vascular injury. Intraoperative complications included haem-o-lok penetration of the calcified aorta, necessitating vascular team intervention. Postoperative issues included bilateral popliteal artery emboli requiring embolectomy and fasciotomy, and a parastomal abscess. Despite significant blood loss and complications, the patient recovered well and remains cancer-free. This case undersco

Button Battery Ingestion: A Tiny Object with the Potential for a Catastrophic Outcome.

Foreign body ingestion is a common complaint encountered in the emergency department among both adults and children. Children under the age of 6 years are at especially high risk of foreign body ingestion, including coins, toys, jewelry and batteries. The button battery is the most common battery ingested and has the highest potential for serious and fatal injuries in infants and children. Over the past decade there has been a dramatic increase in the incidence of button battery ingestions directly related to the widespread expansion and availability of common consumer electronics powered by b

Coronavirus disease 2019 and catastrophic antiphospholipid syndrome: Case report.

RATIONALE: The emergence of catastrophic antiphospholipid syndrome (CAPS) alongside coronavirus disease 2019 (COVID-19) is of great concern, because of its high mortality and unclear mechanism. This severe disease, characterized by multiple thrombi and multisystem disorder, has notably diverse clinical presentations, which complicates its diagnosis in clinical practice. Now, we report a rare case of CAPS in a patient with COVID-19. PATIENT CONCERNS: A 64-year-old patient who mainly presented with pain and swelling 2 months ago progressed gradually into multiple thrombi, including pulmonary em

The Interaction of Pain Catastrophizing and Chronic Pain Acceptance in Sickle Cell.

PURPOSE: Chronic pain in sickle cell disease is more complex and challenging to manage than in other conditions. To better inform chronic pain assessment and management, this study evaluated the role of chronic pain acceptance in the context of pain catastrophizing and their combined association with chronic pain intensity and chronic pain disability in adults with sickle cell disease. DESIGN: This was a secondary analysis of cross-sectional survey data from 170 adults with sickle cell disease (mean age of 28.05 years and 53.5% female, with moderate to severe chronic pain and high levels of c

Subcapsular Hepatic Hemorrhage: An Uncommon and Catastrophic Manifestation of Metastatic Melanoma.

Subcapsular hepatic hematoma is a rare but potentially life-threatening manifestation of metastatic melanoma, often presenting with vague abdominal pain and risk of misdiagnosis. We report a 76-year-old man with a history of malignant melanoma who presented with acute epigastric pain radiating to the right shoulder, vomiting, and a syncopal episode. While initial blood tests showed mild leukocytosis and raised lactate, a CT aorta excluded dissection but revealed multiple hepatic metastases with a large subcapsular hematoma and pulmonary nodules. A follow-up CT mesenteric angiogram demonstrated

Catastrophic storms, forest disturbance, and the natural history of Swainson's warbler.

The core breeding range of Swainson's warbler (Limnothlypis swainsonii) overlaps a zone of exceptionally high tornado frequency in southeastern North America. The importance of tornadoes in creating breeding habitat for this globally rare warbler and other disturbance-dependent species has been largely overlooked. This paper estimates tornado frequency (1950-2021) and forest disturbance in the 240 counties and parishes in which breeding was documented from 1988 to 2014. The frequency of destructive tornadoes (EF1-EF5) varied 6-fold across the breeding range with a peak in the Gulf Coast states

The impact of tobacco consumption on household catastrophic health expenditure in Türkiye.

BACKGROUND: Understanding the financial burden of smoking on households is crucial for developing effective strategies and policies to reduce smoking and mitigate its impact on household health. AIM: To investigate the relationship between smoking and catastrophic health expenditure in Türkiye. METHODS: This cross-sectional study used microdata from household budget surveys conducted by the Turkish Statistical Institute in 2015 and 2019. The data included the socioeconomic characteristics, income levels and consumption patterns of various goods and services by individuals and households. We

Socioeconomic Inequalities in Out-of-Pocket and Catastrophic Health Expenditures in Pakistan.

OBJECTIVES: In Pakistan, healthcare utilization is linked to out-of-pocket payments (OOP) which disproportionately affect low-income households. We investigated socioeconomic inequality in OOP and catastrophic health expenditures (CHEs), and the contribution of sociodemographic factors to these inequalities. METHODS: Socioeconomic inequalities were quantified using the concentration index (CI), and the slope (SII) and relative (RII) indices of inequality using data from three rounds of Household Integrated Economic Survey (2007-08, 2011-12, and 2018-19). Decomposition analyses were conducted

Brain signatures of catastrophic events: Emotion, salience, and cognitive control.

Anticipatory brain activity makes it possible to predict the occurrence of expected situations. However, events such as traffic accidents are statistically unpredictable and can generate catastrophic consequences. This study investigates the brain activity and effective connectivity associated with anticipating and processing such unexpected, unavoidable accidents. We asked 161 participants to ride a motorcycle simulator while recording their electroencephalographic activity. Of these, 90 participants experienced at least one accident while driving. We conducted both within-subjects and betwee

Case report: Timing of eculizumab treatment in catastrophic antiphospholipid syndrome.

Catastrophic antiphospholipid syndrome (CAPS) is a life-threatening condition of small-vessel thrombosis with acute multiple-organ involvement and visceral damage. In this report, we present a case of a patient with CAPS who is refractory to conventional therapy. For the first time in a patient with CAPS, marked C5b-9 formation was demonstrated on microvascular endothelial cells, suggesting the usefulness of therapeutic complement inhibition in this setting. Eculizumab, a C5-blocking monoclonal antibody, is remarkably effective in the treatment of different forms of thrombotic microangiopathy

Turning islands into bridges: Communitybased response after a catastrophic earthquake.

The workforce rallying point model is intended to activate within the first 24 hours after a catastrophic earthquake. Its purpose is to provide employees with access to critical information and early response assignments following a Cascadia subduction zone-type event. The consequences from a catastrophic event could include loss of communication with departmental leadership, as well as loss of safe worksite facilities. A systems failure of this magnitude requires rethinking traditional centralised disaster response models. Building from lessons learned in past catastrophes, the City of Seattl

Chicken Bone Ingestion Leads to Aortoesophageal Fistula With Catastrophic Bleeding.

Aortoesophageal fistula (AEF) is an uncommon, but potentially fatal cause of upper gastrointestinal bleeding. Aortoesophageal fistulas caused by foreign body ingestion are rare but devastating. The classic clinical triad of AEF consists of mid-thoracic pain or dysphagia, a herald episode of hematemesis, followed by fatal exsanguination after a symptom-free period (Chiari's triad). Computed tomography angiography (CTA) is the preferred diagnostic tool for identifying AEF and is substantially more sensitive than upper endoscopy for detecting AEF. Endoscopy can detect AEF as it might show pulsati

Early HELLP Syndrome or Catastrophic Antiphospholipid Syndrome? A Diagnostic Dilemma.

Hypertensive disorders of pregnancy typically occur in the third trimester, with earlier presentations associated with underlying disorders such as antiphospholipid syndrome (APLS). We describe a case of a young primigravida presenting at 15 weeks 6 days gestation with epigastric pain, vomiting, new-onset severe-range hypertension, and subsequent development of anemia, thrombocytopenia, and transaminitis. Antiphospholipid antibodies (aPL) were triple-positive and imaging was negative for thrombosis. She was treated with aspirin, therapeutic anticoagulation, and ultimately dilatation and evacua

Emergency laparotomy for abdominal catastrophes secondary to lymphoma: A systematic review.

Intestinal lymphomas can rarely present as abdominal catastrophes with perforation or small bowel obstruction. There is little data regarding their optimal surgical management and associated outcomes. We aimed to systematically review relevant published literature to assess the presentation, diagnosis, optimal surgical approach and associated post-operative outcomes. A systematic on-line literature search of Embase and Medline identified 1485 articles of which 34 relevant studies were selected, including 7 retrospective studies, 1 case series and 26 case reports. Selected articles were assesse

Blood magic in the age of psychopathy, mass shootings and ecological catastrophe.

This paper is an effort to locate blood magic in the age of psychopathy. Taking as a starting point Michael Eigen's observation that psychopathic tendencies can become cut off from balancing capacities and wreak havoc, we consider the ways in which practices of blood magic, including rituals surrounding menstruation have functioned variously across cultures to balance destructive tendencies and sustain relationship with the living surround. We argue that the lack of these practices and the attitudes fed by and feeding them has contributed to an upsurge in ultra-violent phenomena like mass shoo
