Cauda Equina Syndrome Due to Lumbar Disc Herniation: a Review of Literature.

Cauda equina syndrome (CES) is a rare neurologic condition that is caused by compression of the cauda equina. Cauda equina consists of spinal nerves L2-L5, S1-S5 and the coccygeal nerve. The compression of these nerve roots can be caused mainly by lumbar disc herniation (45% of all causes). The diagnosis consists of two critical points: a) detailed history and physical examination and b) MRI or CT. The gold standard of the treatment of this syndrome is the surgical approach in combination with the timing of onset of symptoms. The surgery as an emergency situation is recommended in the fi rst 4…

Brucellar spondylodiscitis with rapidly progressive spinal epidural abscess showing cauda equina syndrome.

Early diagnosis of Brucellosis is often difficult in the patient with only single non-specific symptom because of its rarity. We report a patient with Brucellar spondylodiscitis, in which the low back pain was the only symptom and the magnetic resonance imaging (MRI) showed not radiographic features about infection at initial stage. He was misdiagnosed as a lumbar disc herniation for inappropriate treatment in a long time. The delay in diagnosis and correct treatment led to rapid progression of the disease and severe complications. The patient was treated successfully with triple-antibiotic an

Lower motor neuron syndrome due to cauda equina hypertrophy with onion bulbs.

INTRODUCTION: Hypertrophy of the nerve roots of the cauda equina may occur with both acquired and inherited neuropathies. Although selective nerve root involvement of the sensory roots has been described and termed chronic inflammatory sensory polyradiculoneuropathy (CISP), selective involvement of the proximal motor roots has not been described. METHODS: Clinical, electrophysiological, MRI, and pathological findings are reported. RESULTS: Here, we report a patient with cauda equina hypertrophy presenting with a pure lower motor neuron syndrome without clinical or electrophysiological eviden

Timing of surgical intervention in cauda equina syndrome: a systematic critical review.

OBJECTIVE: Cauda equina syndrome (CES) is a rare but important neurosurgical emergency. Despite being a recognized clinical entity since 1934, there remains significant uncertainty in the literature regarding the urgency for surgical intervention. The past decade has seen the emergence of the much-referred-to 48-hour limit as a possible window of safety. The ramifications of this time point are significant for early patients who may subsequently have urgent treatment delayed, and for litigation cases, after which adverse decisions are more likely to occur. METHODS: A systematic principally qu…

Postoperative cauda equina syndrome in trivial lumbar congenital kyphosis: a case report.

A 25-year old man presented with chronic low back pain for about 5 years due to mild congenital lumbar kyphosis (L1-L3 25° with congenital posterior wedge vertebra L2). Preoperative neurologic examination was normal. After posterior spinal fusion and instrumentation with moderate curve correction, the patient gradually developed the symptoms and signs of cauda equina syndrome due to intraoperative L2-3 disc herniation. After 5 days the patient underwent posterior decompression surgery and on the latest follow up visit at 2 years later, nearly all the motor power was recovered but the patient c…

Lumbar disk herniation presented with cauda equina syndrome in a pregnant woman.

Despite low back pain being common in pregnancy, cauda equina syndrome is rare. Misdiagnosis and delay in treatment may cause neurological sequelae including urinary and fecal incontinence, sexual dysfunction in patients. A case of cauda equina syndrome in a pregnant woman at 25-week gestation is presented here. The patient underwent an emergency, standard lumbar microdiscectomy under general anesthesia on prone position. Neither the patient nor the baby had any complication related to surgery.

Acute thrombosis of an abdominal aortic aneurysm presenting as cauda equina syndrome.

Acute aortic occlusion is an uncommon vascular emergency that can present with predominantly neurologic symptoms owing to spinal cord ischemia. We describe a 62-year-old woman who experienced acute thrombosis of an abdominal aortic aneurysm that initially presented as cauda equina syndrome. She was treated operatively with an axillary bifemoral bypass. Our case report is followed by a discussion of acute aortic occlusion.

Cauda equina syndrome and dural ectasia: rare manifestations in chronic ankylosing spondylitis.

Cauda equina syndrome (CES) is a rare manifestation in patients with long-standing ankylosing spondylitis (AS). We report a 57-year-old male patient with a 30-year history of AS who developed CES in the past 4 years. The CT and MRI examinations showed unique appearances of dural ectasia, multiple dorsal dural diverticula, erosion of the vertebral posterior elements, tethering of the conus medullaris to the dorsal aspect of the spinal canal and adhesion of the nerve roots of the cauda equina to the wall of the dural sac. A large dural defect was found at surgery. De-adhesion of the tethered con…

Does patient history and physical examination predict MRI proven cauda equina syndrome?

STUDY DESIGN: Systematic review. STUDY RATIONALE: While magnetic resonance imaging (MRI) is used as the diagnostic gold standard for cauda equina syndrome (CES), many MRI scans obtained from patients presenting with signs and/or symptoms of CES do not reveal concordant pathology. As a result, the role of the history and physical examination remains unclear when determining which patients require emergent MRI.Objective or clinical question: Are there elements from the history or physical examination that are associated with CES as established by MRI? METHODS: A systematic review of the litera…

Concomitant cauda equina syndrome and lumbar diskitis secondary to urinary tract infection.

Cauda equina syndrome and lumbar diskitis are relatively uncommon conditions. This case report describes a case of concomitant presentation of cauda equina syndrome and lumbar diskitis in a patient with an Escherichia coli culture-positive extruded disk fragment. To our knowledge this is the first case of a patient who presented with cauda equina syndrome secondary to lumbar diskitis from a urinary tract infection.

Magnetic resonance imaging of cauda equina syndrome in long-standing ankylosing spondylitis.

The cauda equina syndrome is an uncommon complication of long-standing ankylosing spondylitis and its aetiology is controversial. We report a case of the cauda equina syndrome, erosion of the posterior elements of the lumbar spine and traction of the lumbar nerve roots because of multiple dural diverticula in a patient with long-standing ankylosing spondylitis. Magnetic resonance imaging is valuable in excluding other spinal lesions. Extensive dural diverticula formation is characteristic of ankylosing spondylitis and has the potential to provide an early diagnosis.

[Diagnosis and treatment of cauda equina syndrome secondary to lumbar disc herniation].

OBJECTIVE: To discuss the diagnosis and treatment of cauda equina syndrome. METHODS: Thirty-four cases of cauda equina syndrome from 1993 to 2005 were analysed retrospectively. Its clinical representation, pathology character and clinical outcome were observed. RESULTS: At follow-ups of 3 to 102 months after the treatment (averagely 43 months), the muscle strength and function of the constrictor began to resume to different degrees. Muscle strength resumed gradually one week after the operation, resumed faster 3 months after the operation, and resumed completely more one year later. Function…

Posterior epidural migration of sequestered lumbar disc fragment causing cauda equina syndrome.

Posterior epidural migration of free disc fragments is rare, and posterior migration of the free fragments causing cauda equina syndrome is exceptionally rare. This report describes a 53-year-old man with disc fragment extrusion at the levels of L3-4 and a 54-year-old man with disc fragment extrusion at L5-S1 intervertebral space. The patients responded well to the operative therapy with complete relief of the symptoms. The pathological examination confirmed that the specimen was a degenerated intervertebral disc. Early surgery should be the first choice of therapy in patients with large poste…

Disseminated enteropathy-type T-cell lymphoma: cauda equina syndrome complicating coeliac disease.

A patient with coeliac disease developed a progressive cauda equina syndrome in the 2 months prior to his death, for which no cause could be identified. At post-mortem examination this proved to be due to disseminated enteropathy-type T-cell lymphoma. Although rare, disseminated enteropathy-type T-cell lymphoma should enter the differential diagnosis in any patient with coeliac disease developing new neurological signs.

Piriformis syndrome diagnosed by cauda equina action potentials: report of two cases.

STUDY DESIGN: Evaluation of recorded potentials from the lumbar spine after stimulation of the peroneal nerve in patients with piriformis syndrome is reported. OBJECTIVES: To report a new method of diagnosis of piriformis syndrome. SUMMARY OF BACKGROUND DATA: Clinically, the Freiberg and Pace signs were used to diagnose this entity. Among imaging examinations, computed tomography and magnetic resonance imaging have been used. An electrophysiological method using H reflex has also been reported. However, the best method for diagnosis of piriformis syndrome has yet to be clearly established.
