OBJECTIVE: As cell-free DNA levels in the pleural fluid and serum of parapneumonic pleural effusion (PPE) patients have not been thoroughly explored, we evaluated their diagnostic potential.
METHODS: Twenty-two PPE and 16 non-PPE patients were evaluated. Serum and pleural fluids were collected, and cell-free DNA was quantified. All biomarkers were assessed for correlation with days after admission. Receiver operating characteristic (ROC) curve analysis was used to determine diagnostic accuracy and optimal cut-off point.
RESULTS: Nuclear and mitochondrial DNA levels in the pleural fluid and n

There is increasing interest in the use of cell-free circulating tumor DNA (ctDNA) as a serum marker for therapy assessment in prostate cancer patients. Prostate cancer is characterized by relatively low numbers of mutations, and, in contrast to many other common epithelial cancers, commercially available single nucleotide mutation assays for quantification of ctDNA are insufficient for therapy assessment in this disease. However, prostate cancer shares some similarity with translocation-affected mesenchymal tumors (e.g., leukemia and Ewing sarcoma), which are common in pediatric oncology, whe

Sterile inflammation driven by circulating cell-free nucleic acids (cfNAs) plays a critical role in immune activation across various pathological conditions, including autoimmune diseases and tissue injuries. In this study, we explore the potential of polycation-coated microparticles as localized cfNA scavengers designed to mitigate inflammation without the risk of cellular uptake that could reactivate toll-like receptor (TLR) pathways. Using chemical-induced cell death models, we show that the amount of cfNAs released varies depending on cell type and drug treatment. The cationic microparticl…

The proper functioning of any protein depends on its three dimensional conformation which is achieved by the accurate folding mechanism. Keeping away from the exposed stress conditions leads to cooperative unfolding and sometimes partial folding, forming the structures like protofibrils, fibrils, aggregates, oligomers, etc. leading to several neurodegenerative diseases like Parkinson's disease, Alzheimer's, Cystic fibrosis, Huntington, Marfan syndrome, and also cancers in some cases, too. Hydration of proteins is necessary, which may be achieved by the presence of organic solutes called osmoly

BACKGROUND: Plasma cell-free DNA (cfDNA) whole-genome methylation sequencing enables simultaneous characterization of DNA methylation and fragmentomic features for cancer detection. However, it remains unclear whether methylation library preparation faithfully preserves fragmentomic signals, how methylation and fragmentation are biologically coupled, and how these two modalities can be systematically integrated.
METHODS: We systematically evaluated the preservation of cfDNA fragmentomic features in whole-genome bisulfite sequencing (WGBS) and enzymatic methyl sequencing (EM-seq) relative to c

BACKGROUND: Trisomy 21 (Down syndrome) remains the most prevalent autosomal aneuploidy, necessitating accurate prenatal diagnosis. While cell-free fetal DNA (cffDNA)-based non-invasive prenatal testing (NIPT) has transformed screening, challenges persist in low fetal fraction cases and confined placental mosaicism. Integrating artificial intelligence (AI), multi-omics, and fetal cell-based approaches represents a paradigm shift toward comprehensive prenatal diagnostics.
OBJECTIVE: This AI-assisted meta-analysis evaluates NIPT platforms for Trisomy 21 detection, comparing cffDNA-based, SNP-bas

Metastatic breast cancer is a heterogeneous disease entity with varying genomic alterations. Mutations may be acquired through tumor evolution under the pressure of treatment. While both tumor tissue genotyping and cell-free DNA (cfDNA)/circulating tumor DNA (ctDNA) may be used to identify actionable mutations for therapeutic intervention, cfDNA/ctDNA offer the advantages of being less invasive and more feasible for serial sampling, which make them conducive for real-time tumor genotyping. In addition, due to heterogeneity within a tumor, a single tumor tissue biopsy may not capture all mutati

Large language models can synthesize biomedical knowledge, parse vast amounts of data, and generate code, positioning them as promising tools for biomarker discovery from high-throughput omics data. Here, we benchmark six models from OpenAI, Anthropic, and Google on plasma cell-free RNA datasets spanning three clinical cohorts: Kawasaki disease versus multisystem inflammatory syndrome in children, active tuberculosis versus symptomatic respiratory controls, and myalgic encephalomyelitis/chronic fatigue syndrome versus sedentary controls. We evaluate literature-guided nomination of diagnostic g

BACKGROUND: Tuberculous pleurisy is the most common form of extrapulmonary tuberculosis. Diagnosis remains challenging due to the paucibacillary nature of pleural effusions, leading to low sensitivity of conventional microbiological methods and frequent reliance on invasive biopsy. Cell-free DNA (cfDNA), comprising fragmented genetic material released from host cells and pathogens into biofluids, presents a promising minimally-invasive biomarker. This protocol outlines a systematic review and meta-analysis designed to evaluate the overall diagnostic accuracy of cfDNA for tuberculous pleurisy a

The analysis of cell-free DNA (cfDNA) in blood is expected to serve as a new diagnostic method for early cancer detection and treatment monitoring by examining trace amounts of ctDNA derived from cancer cells. In cancer patients, ctDNA circulates in the bloodstream, and thanks to technological innovations in next-generation sequencing (NGS) and the analysis of fragmented DNA at low levels, it is now possible to non-invasively detect the presence and type of cancer. Multi-cancer early detection (MCED) is a method for detecting cancer early and identifying its specific type by analyzing cfDNA fr

DNA methylation alterations are early and stable hallmarks of cancer and represent promising biomarkers for non-invasive detection using circulating cell-free DNA (cfDNA). However, current computational approaches often model DNA sequence and methylation features separately and struggle to capture complex read-level methylation architecture in heterogeneous, low-signal liquid biopsy data. Here, we present DNAmBERT, a Transformer-based deep learning framework designed to jointly model DNA sequence context and read-level methylation haplotype structure from cfDNA methylation sequencing data. 5-hydroxymethylcytosine (5hmC) is a stable epigenomic mark associated with gene regulation and tumorigenesis. To evaluate its potential for multi-cancer detection, epigenomic distribution of 5hmC was analyzed in tumor tissues (n = 217), normal tissues (n = 50), and cell-free DNA (cfDNA) from cancer (n = 1009) and non-cancer subjects (n = 1678) across breast, colon, lung, ovarian, and pancreatic cancers. Differential analysis revealed extensive redistribution of 5hmC in early-stage tumors that persisted into the late-stage, while global 5hmC abundance decreased in all tumors. Two distinct patte

Comprehensive molecular profiling by next-generation sequencing has revolutionized tumor classification and biomarker evaluation. However, routine implementation is challenged by the scant nature of diagnostic material obtained through minimally invasive procedures. Here, we describe our long-term experience in profiling cytology samples with an in-depth assessment of the performance, quality metrics, biomarker identification capabilities, and potential pitfalls. We highlight the impact of several optimization strategies to maximize performance with 4,871 prospectively sequenced clinical cytol

BACKGROUND: An NGS-based assay was developed to determine the presence or absence of paternally inherited genetic variants in cfDNA derived from the fetus in the plasma of pregnant women. This assay can be used in connection with NGS-based prenatal prediction of fetal blood groups in immunised pregnant women. The purpose of the assay is to minimise the risk of a false-negative outcome in the situation with a prediction of the absence of a blood group allele from the fetus.
METHODS: The underlying principle was to examine genetic markers, with each single marker giving a small contribution to

BACKGROUND: Cell-free fetal DNA (cffDNA) screening is routinely performed in pregnancy. Abnormal fetal fraction has been associated with adverse pregnancy outcomes, including hypertensive disorders of pregnancy, which are associated with severe maternal and neonatal morbidity and mortality.
OBJECTIVE: This study examined whether abnormal fetal fraction, defined in this study as fetal fraction either <6 or >15 on the basis of restricted-cubic-spline-plot within our study population, was associated with HDP in a retrospective sample, as well as whether fetal fraction improves the prediction of

Cardiovascular diseases are the foremost contributor to global mortality, presenting a complex etiology and an expanding array of risk factors. Coronary artery disease characterized by atherosclerotic plaque build-up in the coronary arteries, imposes significant mortality and financial burdens, especially in low- and middle-income nations. The pathogenesis of coronary artery disease involves a multifaceted interplay of genetic, environmental, and epigenetic factors. Epigenetic regulation contributes to the dynamic control of gene expression without altering the underlying DNA sequence. The mou

At least 5% of cancer diagnoses are attributed to a causal pathogenic or likely pathogenic germline genetic variant (hereditary cancer syndrome-HCS). These individuals are burdened with lifelong surveillance monitoring organs for a wide spectrum of cancers. This is associated with substantial uncertainty and anxiety in the time between screening tests and while the individuals are awaiting results. Cell-free DNA (cfDNA) sequencing has recently shown potential as a non-invasive strategy for monitoring cancer. There is an opportunity for high-yield cancer early detection in HCS. To assess clinic
