Multi-scale mechanistic modelling of the host defence in invasive aspergillosis reveals leucocyte activation and iron acquisition as drivers of infection outcome.

Aspergillus species are ubiquitous environmental moulds, with spores inhaled daily by most humans. Immunocompromised hosts can develop an invasive infection resulting in high mortality. There is, therefore, a pressing need for host-centric therapeutics for this infection. To address it, we created a multi-scale computational model of the infection, focused on its interaction with the innate immune system and iron, a critical nutrient for the pathogen. The model, parameterized using published data, was found to recapitulate a wide range of biological features and was experimentally validated in

Mitochondrial dysfunction in diabetic tubulopathy.

Diabetic kidney disease (DKD) is a devastating microvascular complication associated with diabetes mellitus. Recently, the major focus of glomerular lesions of DKD has partly shifted to diabetic tubulopathy because of renal insufficiency and prognosis of patients is closely related to tubular atrophy and interstitial fibrosis. Indeed, the proximal tubule enriching in mitochondria for its high energy demand and dependence on aerobic metabolism has given us pause to focus primarily on the mitochondria-centric view of early diabetic tubulopathy. Multiple studies suggest that diabetes condition di

Influence of Woodsmoke Exposure on Molecular Mechanisms Underlying Alzheimer's Disease: Existing Literature and Gaps in Our Understanding.

Woodsmoke poses a significant health risk as a growing component of ambient air pollution in the United States. While there is a long history of association between woodsmoke exposure and diseases of the respiratory, circulatory, and cardiovascular systems, recent evidence has linked woodsmoke exposure to cognitive dysfunction, including Alzheimer's disease dementia. Alzheimer's disease is a progressive neurodegenerative disorder with largely idiopathic origins and no known cure. Here, we explore the growing body of literature which relates woodsmoke-generated and ambient air pollution particu

Recent advances in the diagnosis and management of autoimmune pancreatitis: similarities and differences in Japan and Korea.

Two subtypes (types 1 and 2) of autoimmune pancreatitis (AIP) are currently recognized. Type 1 AIP is related to immunoglobulin G4 (lymphoplasmacytic sclerosing pancreatitis), and type 2 AIP is characterized by neutrophilic infiltration into the epithelium of the pancreatic duct (idiopathic duct-centric pancreatitis). Although type 2 AIP is sometimes observed in the United States and Europe, most cases of AIP in Japan and Korea are type 1. The international consensus diagnostic criteria for AIP were created to be applicable worldwide and to distinguish between the two types of AIP. AIP is diag

It's Probably Mine: Self-Prioritization Can Be a Decisional Strategy.

Personal possession exerts a significant influence on decision-making, such that stimulus classification is speeded when objects belong to the self (vs. other persons). Exactly when and how this self-prioritization effect arises, however, remains a matter of speculation and debate. Accordingly, adopting a psychophysical approach in combination with computational modeling, here we hypothesized that self-prioritization could derive from the application of an egocentric strategy (i.e., default-to-self response) during decisional processing. Using a modified object-classification task in which par

Systemic delivery of synapsin-promoted caveolin-1 overexpression ameliorates pathological TDP-43-induced cognitive decline and neurodegenerative changes.

INTRODUCTION: Transactive response DNA-binding protein 43 (TDP-43) proteinopathy is associated with frontotemporal dementia and Alzheimer's disease (AD). We previously demonstrated that synapsin-promoted caveolin-1 (SynCav1) preserves cognitive function in the mouse model of AD. This study investigated the therapeutic potential of SynCav1 in a mouse model of TDP-43 proteinopathy. METHODS: AAV-PhP.eB-SynCav1 was delivered systemically to the TDP-43A315T mouse, followed by cognitive evaluation and biochemical and ultrastructural analysis of brain tissue. RESULTS: SynCav1 exerted robust neuropr

Mapping and Cataloguing Microbial and Biochemical Determinants of Health: Towards a 'Database of Salutogenic Potential'.

Microbial and biochemical research has historically focused on pathogenic agents due to their clear association with disease. This is a perspective that has saved countless lives but encourages a skewed, threat-centered view of microbes and biogenic compounds. Emerging evidence shows that exposure to diverse environmental microbiomes and natural biochemical products is also salutogenic-promoting health and resilience. Here we introduce the 'Database of Salutogenic Potential', a prototype relational repository cataloguing environmental microbes and biochemical compounds linked to health benefit

Edge-Community Entropy Is a Novel Neural Correlate of Aging and Moderator of Fluid Cognition.

Decreased neuronal specificity of the brain in response to cognitive demands (i.e., neural dedifferentiation) has been implicated in age-related cognitive decline. Investigations into functional connectivity analogs of these processes have focused primarily on measuring segregation of nonoverlapping networks at rest. Here, we used an edge-centric network approach to derive entropy, a measure of specialization, from spatially overlapping communities during cognitive task fMRI. Using Human Connectome Project Lifespan data (713 participants, 36-100 years old, 55.7% female), we characterized a pat

Organizational Culture as a Determinant of Outcome in Teams: Implications for the Pediatric Cardiac Specialist.

Although enormous effort has focussed on how to build an effective culture in the business community, relatively little effort has addressed how to achieve this in the hospital environment, specifically related to the field of congenital heart disease teams. The examination of culture in pediatric cardiac care is particularly important for several key reasons: first, it represents high-stakes medicine, second, there are multiple stakeholders requiring collaboration between cardiologists, surgeons, anaesthesiologists, perfusionists, nursing staff, and allied health care professionals, and final

Exparel for Postoperative Pain Management: a Comprehensive Review.

PURPOSE OF REVIEW: Multimodal pain management is the most effective way to treat postsurgical pain. However, the use of opioids for acute pain management has unfortunately been a significant contributor to the current opioid epidemic. The use of opioids should be limited and only considered a "rescue" pain medication after other modalities of pain management have been utilized. RECENT FINDINGS: It may be difficult to curtail the use of opioids in the treatment of chronic pain; however, in the postsurgical setting, there is compelling evidence that an opioid-centric analgesic approach is not n

Animal Modelling of Interstitial Cystitis/Bladder Pain Syndrome.

The etiology of interstitial cystitis/bladder pain syndrome (IC/BPS) remains elusive and may involve multiple causes. To better understand its pathophysiology, many efforts have been made to create IC/BPS models. Most existing models of IC/BPS strive to recreate bladder-related features by applying noxious intravesical or systemic stimuli to healthy animals. These models are useful to help understand various mechanisms; however, they are limited to demonstrating how the bladder and nervous system respond to noxious stimuli, and are not representative of the complex interactions and pathophysio

A Systematic Mapping Review of 525 Randomised Controlled Trials on Pharmacological and Non-pharmacological Interventions for People Living With Dementia and Their Formal and Informal Carers in Chinese Communities.

OBJECTIVES: Aligned with the World Health Organization's call for enhanced dementia research to inform national strategies, this systematic mapping review identifies critical gaps in the evidence base for Chinese populations. Representing over 20% of people living with dementia globally, this group remains underrepresented in the Western-centric research that currently guides best practices. This is the first study that comprehensively reviews evidence on dementia-related interventions trialled among participants identified as Chinese worldwide across both Chinese and English literature. Temporomandibular joint disorders and maxillomandibular malformations: role of condylar "repositionin" plate.

Even if the relationship between condylar position and/or temporomandibular disorders (TMDs) and dentofacial deformity is controversial in literature, several patients presenting malocclusion refer to pain and TMDs as the main trouble. There are also various opinions concerning the alterations or improvements of temporomandibular joint symptoms after orthognathic surgery. In agreement with the experience of Universität Würzburg, the purpose of this study was to evaluate the validity of splint technique to reproduce centric condyle positioning in bimaxillary osteotomy surgeries for the patients

Challenges, care gaps, and coping mechanisms in patients with advanced cancer and their caregivers following palliative gastrointestinal surgery.

BACKGROUND: Palliative gastrointestinal surgery improves symptoms and health-related quality of life in patients who present with complications from advanced cancer. However, palliative surgery near the end-of-life is associated with significant physiological and psychological stressors for both patients and their caregivers. Yet literature on patient and caregiver centric narratives following surgery remains limited. OBJECTIVES: We aim to explore challenges, perceived care gaps, and coping mechanisms of patients with advanced cancer and their caregivers following palliative gastrointestinal

Mechanisms and treatment of obesity-related hypertension-Part 2: Treatments.

Hypertension is a frequent comorbidity of obesity that significantly and independently increases the risk of cardiovascular and renal events. Obesity-related hypertension is a major challenge to the healthcare system because of the rapid increase in obesity prevalence worldwide. However, its treatment is still not specifically addressed by current guidelines. Weight loss (WL) per se reduces blood pressure (BP) and increases patient responsiveness to BP-lowering medications. Thus, a weight-centric approach is essential for the treatment of obesity-related hypertension. Diet and physical activit

The contribution of bloodstain pattern analysis (BPA) in the investigation of crimes against domestic and wild animals.

Crimes against domestic and wild animals represent a growing global concern that introduces unique investigative challenges. Although Bloodstain Pattern Analysis (BPA) is a well-established discipline in human-centric forensics, its potential within veterinary medicine remains significantly underutilized. To bridge this critical gap, this review moves from foundational theory to the practical application of BPA in animal-related contexts. First, the article establishes the scientific validity of this application by demonstrating that fluid dynamic differences between human and animal blood are

Cellular lipids: fundamental host factors for monkeypox virus replication and pathogenesis.

The recent outbreaks of mpox and other poxvirus diseases worldwide have highlighted the critical need for effective antiviral treatments. Throughout its life cycle, MPXV, a member of the Orthopoxvirus genus, takes advantage of the host’s lipid metabolism. In addition to being essential for maintaining cellular homeostasis, lipids are also used by viruses for entrance, reproduction, assembly, and egress. Lipid rafts (LRs), membrane microdomains rich in cholesterol that are essential for viral entry and viral envelope structure, are a crucial component of this exploitation. This study summarizes

Current State of Fibrotic Interstitial Lung Disease Imaging.

Interstitial lung disease (ILD) diagnosis is complex, continuously evolving, and increasingly reliant on thin-section chest CT. Multidisciplinary discussion aided by a thorough radiologic review can achieve a high-confidence diagnosis of ILD in the majority of patients and is currently the reference standard for ILD diagnosis. CT also allows the early recognition of interstitial lung abnormalities, possibly reflective of unsuspected ILD and progressive in a substantial proportion of patients. Beyond diagnosis, CT has also become essential for ILD prognostication and follow-up, aiding the ident
