Immediate regression of leukoaraiosis after carotid artery revascularization.

BACKGROUND: Leukoaraiosis (LA) affects cognition after stroke and reversal of LA may improve cognitive performance. We aimed to determine the impact of cerebral perfusion and circle of Willis (CoW) flow patterns on the extent of LA after carotid artery revascularization. METHODS: LA was scored on fluid-attenuated inversion recovery magnetic resonance (MR) images at the levels of the centrum semiovale and frontal horns in both cerebral hemispheres of 62 contiguous patients (men/women = 38/24, mean age = 63.2 ± 8.4 years, range 44-82) before and after unilateral carotid artery revascularization

Childhood moyamoya disease: quantitative evaluation of perfusion MR imaging--correlation with clinical outcome after revascularization surgery.

PURPOSE: To evaluate whether perfusion magnetic resonance (MR) imaging can depict hemodynamic status after revascularization surgery and whether changes at perfusion MR imaging after revascularization surgery correspond with clinical outcome in moyamoya disease. MATERIALS AND METHODS: An institutional review board approved this retrospective study; informed consent was waived. Pre- and postoperative perfusion MR imaging data in 67 children with moyamoya disease (mean age, 7.2 years; range, 2-13 years) were included. Regional time to peak (rTTP) and regional cerebral blood volume (rCBV) were c

Percutaneous transluminal angioplasty for acute thrombotic occlusion of the cervical internal carotid artery using a perfusion balloon catheter.

We performed emergency percutaneous transluminal angioplasty (PTA) with a perfusion balloon catheter (PBC) for PTCA and, at the same time, thrombolytic therapy was administered by urokinase (UK) perfusion in five cases of severe cervical internal carotid artery stenosis in which sudden cerebral hemisphere ischemia occurred due to acute thrombotic obliteration. In four out of the five cases, effective revascularization was obtained; three resulted in Good recovery, but one showed extensive cerebral infarction after PTA. In the remaining one case, where stenosis of the internal carotid artery of

Motor excitability in bilateral moyamoya vasculopathy and the impact of revascularization.

OBJECTIVE: Motor cortical dysfunction has been shown to be reversible in patients with unilateral atherosclerotic disease after cerebral revascularization. Moyamoya vasculopathy (MMV) is a rare bilateral stenoocclusive cerebrovascular disease. The aim of this study was to analyze the corticospinal excitability and the role of bypass surgery in restoring cortical motor function in patients by using navigated transcranial magnetic stimulation (nTMS). METHODS: Patients with bilateral MMV who met the criteria for cerebral revascularization were prospectively included. Corticospinal excitability, …

A Unique and Effective Bypass Technique to Treat Partially Thrombosed Giant Distal Anterior Cerebral Artery Aneurysms in Extremely Narrow Surgical Corridors.

BACKGROUND: Surgical treatment of large or giant thrombosed anterior cerebral artery (ACA) aneurysms often involves revascularization. Herein, we describe a unique and effective bypass technique to treat partially thrombosed giant distal ACA aneurysms in extremely narrow surgical corridors. METHODS: A 68-year-old man underwent aneurysm trapping and ACA revascularization for a partially thrombosed giant ACA aneurysm in a surgical corridor that was narrow due to anatomic factors. By combining a side-to-side anastomosis and an end-to-side anastomosis at a single anastomotic site, we successfully

Superficial temporal artery-to-middle cerebral artery anastomosis with encephalo-duro-myo-synangiosis as a modified operative procedure for moyamoya disease.

BACKGROUND: Various types of revascularization surgery have been performed for moyamoya disease. Although the efficacies of these operations are well recognized, the optimal operative procedure remains undecided. In this report, we describe our modified surgical revascularization procedure for moyamoya disease and retrospectively analyze the results of such surgeries on six sides in six adult patients. METHODS: Our operative procedure, combining direct and indirect bypasses, is a superficial temporal artery to middle cerebral artery anastomosis with encephalo-duro-myo-synangiosis. The encepha

Fusiform aneurysm of the anterior communicating artery treated by vascular reconstruction: case report.

OBJECTIVE: Fusiform aneurysm of the anterior cerebral artery is rare and difficult to treat because of perforators. We encountered a patient with subarachnoid hemorrhage due to rupture of a fusiform aneurysm of the anterior communicating artery complex, and treated this patient with cerebral revascularization. CASE PRESENTATION: A 39-year-old man presented with sudden severe headache resulting from subarachnoid hemorrhage. Digital subtraction angiography showed dilatation from the distal A1 segment to the proximal A2 segment of the left anterior cerebral artery. Despite intensive conservative…

A narrative review of techniques for surgical revascularization of the extracranial vertebral artery in vertebrobasilar insufficiency.

Vertebrobasilar insufficiency is a condition characterized by poor blood flow through the posterior circulation of the brain, which supplies the brainstem, thalamus, hippocampus, cerebellum, occipital lobes, and medial temporal lobes. Narrowing or occlusion of the vertebral arteries may be result in a range of neurological symptoms, including dizziness, imbalance, dysarthria, and even stroke. If symptomatic patients fail medical management, revascularization of the vertebral artery should be considered. Restoration of blood flow may involve transluminal balloon angioplasty or stent placement;

The Influence of Dual Revascularization Techniques on Middle Meningeal Artery Collateral Networks in Moyamoya Angiopathy.

BACKGROUND AND OBJECTIVES: Moyamoya disease is characterized by spontaneous and progressive occlusion of the intracranial internal carotid arteries and their proximal branches. Combined surgical revascularization uses a superficial temporal artery to middle cerebral artery direct bypass with concomitant indirect revascularization, most commonly encephaloduroarteriosynangiosis. The role and evolution of collaterals, particularly from the middle meningeal artery (MMA), is incompletely understood in combined revascularization. METHODS: Patients undergoing combined revascularization for Moyamoya

Rapid and near-complete resolution of moyamoya vessels in a patient with moyamoya disease treated with superficial temporal artery-middle cerebral artery bypass.

Several groups have reported partial resolution of moyamoya vessels following surgical revascularization in pediatric patients, but angiographic documentation of near-complete resolution has been poor. We report here a case of a 5-year-old boy who underwent bilateral superficial temporal artery-middle cerebral artery bypass combined with unilateral encephaloduroarteriosynangiosis who demonstrated near-complete resolution of moyamoya vessels and a dramatic improvement in cerebral perfusion as measured by xenon-enhanced CT. The resolution of moyamoya networks may serve as an indicator of success

Regional cerebral hemodynamics in childhood moyamoya disease.

Regional cerebral blood flow (rCBF) and its reactivity to acetazolamide were analyzed in a series of 15 cases of childhood moyamoya disease, using the xenon-133 inhalation method and single photon emission CT (SPECT). Most of the patients had normal mean hemispheric cerebral blood flow (mCBF), comparable to that of age-matched healthy children. However, they had abnormal rCBF distribution and disturbed reactivity to acetazolamide in the territory of the internal carotid artery, especially in the frontal lobe. Surgical revascularization for the anterior circulation, which consisted of STA-MCA a

Intracranial angioplasty and stenting for cerebral atherosclerosis: new treatments for stroke are needed!

INTRODUCTION: Intracranial atherosclerosis is a common cause of stroke. INTRACRANIAL ANGIOPLASTY AND STENT REVASCULARIZATION: Recent technological developments offer improved methods for endovascular revascularization of symptomatic and asymptomatic cerebral artery stenosis. Identification of appropriate patients remains a diagnostic challenge, and our knowledge about the natural history of the disease remains limited. RECOMMENDATIONS: At this time, patients with significant intracranial stenosis should receive counseling on the benefits and risks of revascularization therapy. Ultimately, de

Angiographic demonstration of postoperative cortical artery stenosis induced by Biemer temporary clips.

Five patients underwent cerebral revascularization operations during which Biemer clips were used to occlude temporarily a branch of the middle cerebral artery. Postoperative angiograms revealed stenoses at the sites where the clips were applied. Excessive clip closing forces may have caused significant endothelial injury, which then produced the stenoses. Clips that exert a minimal occlusive force would avoid this response and are recommended for temporary occlusion of cortical arteries.

Dissection of superficial temporal artery for STA-M4 bypass, an integrated method. How I do it.

BACKGROUND: Extracranial-intracranial (EC-IC) bypass is an effective revascularization technique for flow augmentation in conditions including Moyamoya disease and ischemic cerebrovascular disorders. Preparation of donor vessel is a key step for EC-IC Bypass surgery. This procedure involves careful dissection and liberation of the Superficial Temporal Artery with subsequent anastomosis to branches of the middle cerebral artery re-establishing flow. METHODS: This article outlines a comprehensive surgical strategy, emphasizing anatomical considerations, procedural nuances, and integration of di

[Various Bypass Surgery:Preoperative Simulation for STA-MCA Bypass].

In STA-MCA bypass surgery, it is important to select the optimal recipient using preoperative simulation to avoid complications. We report a preoperative simulation for STA-MCA bypass using the Brain LAB iPLAN platform®BRAIN LAB)and the 3DCG simulation software GRID®Kompath). Here, we introduce the basics and applications of preoperative simulation for occlusive atherosclerotic lesions and present a target bypass for periventricular anastomosis and peripheral vessels of aneurysms in Moyamoya disease. By creating and visualizing 3D fusion images, the optimal donor and recipient can be selected.
