PURPOSE: The aim of this review is to provide an update of the technical nuances of microsurgical and endoscopic-assisted approaches to the craniovertebral junction (transnasal, transoral, and transcervical), and to report on the available clinical results in order to identify the best strategy.
METHODS: A nonsystematic update of the reviews and reporting on the anatomical and clinical results of endoscopic-assisted and microsurgical approaches to the craniovertebral junction (CVJ) was performed.
RESULTS: Pure endonasal and cervical endoscopic approaches still have some disadvantages, includ

A single fracture of the ring of a vertebra is a rare injury of the spine. In this report, we present five single fractures of the posterior ring of the cervical spine below the atlas from four patients after motor vehicle accidents. Initial radiographs failed to show any of these fractures; all were detected by computed tomography. Single ring fractures are stable and none of our patients presented with a neurological deficit. Patients were treated conservatively with a cervical collar or a cervicothoracic brace. They all recovered without any neurological abnormalities.

We report the unusual association of a split atlas and an odontoid fracture in a case of a cervical trauma, thus mimicking a complex C1-C2 fracture. The normal embryology of C1 and the literature on this rare malformation are reviewed.

The phenotypic features of Down's syndrome are easily recognized and include characteristic facial features, hypotonia, ligament laxity, transverse palmar creases and mental subnormality. Associated manifestations and complications are also familiar and involve almost every organ system. Congenital heart defects, bowel malformations and a tendency to leukemia are common attendant problems. Less common, however, are defects of the skeletal system; in fact, the most recent edition of a standard pediatric textbook makes no mention of anomalies of the vertebral column. The purpose of this paper is

A child with 46,XY, 17q+ chromosome aberration and unusual skeletal abnormalities had the presenting symptom of respiratory arrest after a fall and minor head trauma. He exhibited proportionate short stature, peculiar facies with antimongoloid slant, simian creases, postaxial hexadactyly, metatarsus adductus deformity, and anomalous penoscrotal configuration. Delayed speech and mild mental retardation were also present. Atlantoaxial dislocation secondary to odontoid process hypoplasia was demonstrated. Additional skeletal abnormalities included dysplastic changes in the first and fourth metaca

Four cases of combination fractures of the atlas and axis are presented. Three types of management were performed: plaster immobilization, odontoid screw fixation combined with atlantoaxial pedicle screw fixation, occipito-cervical fusion with anterior operation by staged. Based on a literature review and our experience, treatment strategies is discussed according to the stability of the upper cervical spine and neurological involvement, with a reminder that combined injuries in the upper cervical spine should be sought in any patient with a cervical injury and early surgical solution may brin

OBJECTIVE: To evaluate NK6 homeobox 1 (NKX6.1) promoter methylation in cervical lesions and its association with human papillomavirus (HPV)16/18 infection, histological sub-type, and patient outcomes using clinical and bioinformatic data.
METHODS: A total of 207 cervical tissue samples, including cervicitis (n = 22), cervical intraepithelial neoplasia grade 3 (n = 20), adenocarcinoma in situ (n = 6), adenocarcinoma (n = 59), and squamous cell carcinoma (n = 100), were analyzed by methylation-specific polymerase chain reaction and bisulfite sequencing. HPV genotyping was performed with the INN

STUDY DESIGN: Narrative Review.
OBJECTIVES: The premise of this review is to provide a review of the literature pertaining to studies describing outcomes of surgical cohorts when implementing C1 osteosynthesis for arch fractures with or without transverse atlantal ligamentous (TAL) injury.
METHODS: A comprehensive search strategy was implemented across several search engines to identify studies which evaluate the outcomes of C1 osteosynthesis for patients with C1 arch fractures with and without TAL injury.
RESULTS: Ten studies were identified. Parameters reported included osteosynthesis fus

A case of Jefferson fracture in a 53-year-old patient is reported. After conservative treatment, the result at 5 years of follow-up was satisfactory. Review of the literature is included.

Isolated anterior arch atlantal fracture is a rare entity. Two cases of fracture of the atlas following direct oropharyngeal trauma are presented. The importance of careful upper cervical spine evaluation under these circumstances is emphasised, especially if a potential disaster following anaesthetic intubation or surgical head manipulation is to be avoided. Computed tomographic scanning is useful not only to demonstrate the site of fracture but also for visualisation of the spinal canal, especially to exclude intracanalicular bony fragments. The risk of further trauma is minimised due to the
