Cervical nerve root contribution to diaphragm motor innervation revisited via intraoperative clinical and neurophysiological observations.

OBJECTIVE: Classical descriptions state that diaphragmatic motor innervation arises predominantly from the fourth cervical nerve root (C4), with additional contributions from C3 and C5. However, clinical evidence supporting the functional involvement of C3 and C5 remains limited. Study aimed to reassess cervical root contribution to diaphragmatic innervation through intraoperative observations. METHODS: Eighteen consecutive patients undergoing dorsal root entry zone (DREZ) lesioning for refractory pain in upper-limb mostly due to brachial plexus injury were included. The procedure allowed int

Cervical radiofrequency neurotomy reduces central hyperexcitability and improves neck movement in individuals with chronic whiplash.

OBJECTIVE: This study aims to determine if cervical medial branch radiofrequency neurotomy reduces psychophysical indicators of augmented central pain processing and improves motor function in individuals with chronic whiplash symptoms. DESIGN: Prospective observational study of consecutive patients with healthy control comparison. SETTING: Tertiary spinal intervention centre in Calgary, Alberta, Canada. SUBJECTS: Fifty-three individuals with chronic whiplash associated disorder symptoms (Grade 2); 30 healthy controls. METHODS: Measures were made at four time points: two prior to radiofreq

Careful Dissection of the Distal Ureter Is Highly Important in Nerve-sparing Radical Pelvic Surgery: A 3D Reconstruction and Immunohistochemical Characterization of the Vesical Plexus.

OBJECTIVE: Radical hysterectomy with pelvic lymphadenectomy (RHL) is the preferred treatment for early-stage cervical cancer. Although oncological outcome is good with regard to recurrence and survival rates, it is well known that RHL might result in postoperative bladder impairments due to autonomic nerve disruption. The pelvic autonomic network has been extensively studied, but the anatomy of nerve fibers branching off the inferior hypogastric plexus to innervate the bladder is less known. Besides, the pathogenesis of bladder dysfunction after RHL is multifactorial but remains unclear. We st

The C5 root dermatome enlarges and modulates hand pain in total brachial plexus palsy.

Hand pain is a major complaint in 80% of the patients with complete brachial plexus palsy; and, in 80% of these patients, the C5 root is ruptured and the C6-T1 roots avulsed from the spinal cord. It has been suggested that pain in brachial plexus injuries may not arise from avulsed roots, but rather from ruptured roots. Traditionally the C5 root dermatome does not extend to the hand. We have hypothesized that in total lesions of the brachial plexus the C5 root dermatome expands, reaching the hand. In 20 patients with confirmed C5 root rupture and C6-T1 root avulsion, we investigated the distri

Phantom Remodeling Effect of Dorsal Root Entry Zone Lesioning in Phantom Limb Pain Caused by Brachial Plexus Avulsion.

BACKGROUND: Dorsal root entry zone (DREZ) lesioning has been reported to be effective for phantom limb pain caused by brachial plexus avulsion pain. Most reports on DREZ lesioning for brachial plexus avulsion pain have focused on the results of pain relief without a detailed description of phantom sensations following DREZ lesioning. MATERIALS AND METHODS: Two patients (1 with amputation and the other nonamputated) with chronic intractable phantom limb pain caused by brachial plexus avulsion underwent DREZ lesioning on the avulsed segments of the cervical spinal cords. Changes of the phantom

Oberlin transfer and partial radial to axillary nerve neurotization to repair an explosive traumatic injury to the brachial plexus in a child: case report.

PURPOSE: Explosive injuries to the pediatric brachial plexus are exceedingly rare and as such are poorly characterized in the medical literature. METHODS: Herein, we describe an 8-year-old who was struck in the neck by a piece of shrapnel and suffered multiple vascular injuries in addition to a suspected avulsion of the cervical 5 and 6 ventral rami. The patient had a complete upper brachial plexus palsy and failed to demonstrate any clinical improvement at 6-months follow-up. He was taken to the operating from for a partial ulnar to musculocutaneous nerve neurotization as well as a partial r

Part 1. Injuries to the brachial plexus: mechanisms of injury and identification of risk factors.

Upper-arm weakness (paresis) or paralysis indicates peripheral-nerve damage to the brachial plexus, a network of lower cervical and upper thoracic spinal nerves supplying the arm, forearm, and hand. Physical findings reflect muscle paralysis from spinal nerve roots. The mechanism of injury includes maternal, obstetric, and infant factors that apply traction on or compression to the anatomically vulnerable brachial plexus. Nerve regeneration can occur if nerve tissue components are preserved. Recovery is affected by multiple factors, including the type and site of injury, intervention timing, a

Recurrent stingers in an adolescent American football player: dilemmas of return to play. A case report and review of the literature.

We present the case of a 16-year-old football linebacker with a history of recurrent stingers. Initial physical examination was normal as were cervical spine radiographs. MRI of the cervical spine revealed relative stenosis. Electrodiagnostic testing revealed chronic bilateral neurogenic changes of the superior trunk of the brachial plexus. A Kerr Collar was obtained to minimize head acceleration and force transmission through the neck. While there are return-to-play guidelines for recurrent stingers, there are inconsistencies with those recommendations. Our case highlights the challenges in c

High-resolution sonography detects extraforaminal nerve pathology in patients initially diagnosed with cervical disc disease: a case series.

In patients presenting with neck and upper limb complaints, MRI changes suggestive of cervical disc disease do not exclude concomitant extraforaminal pathology, as shown in the series of three cases presented here. High-resolution ultrasound of the brachial plexus and peripheral nerves may be useful in identifying an extraforaminal pathology when (1) symptoms and signs are disproportionate to MRI findings of cervical disc disease; (2) there is obvious discordance between MRI and nerve conduction findings; (3) an entrapment neuropathy is suspected but the site of nerve lesion is unidentifiable

Contribution of embryology in the understanding of cervical venous system anatomy within and around the transverse foramen: a review of the classical literature.

Anatomic arrangement of venous system within the transverse foramen is a controversial topic among authors. Precise knowledge of this arrangement is necessary in imaging where vertebral artery dissection is suspected, as well as in surgical approaches of cervical spine. This knowledge objective cannot be achieved without a prerequisite knowledge of primitive venous system. We present here an update on the development of the transverse foramen venous system through a literature review. Our review of the classical literature aimed at synthesis of available related embryological knowledge and rel

[Diagnostic value of somatosensory evoked potentials (SEP) in lesions of the brachial plexus (author's transl)].

Simultaneous recording of SEP from the contralateral scalp, the lower neck (C 7) and Erb's point following median or ulnar nerve stimulation may be helpful in the localisation of lesions of the brachial plexus and associated spinal roots. In lesions proximal to the dorsal root ganglia, the potential recorded from Erb's point (EP), as well as the first component of the neck-SEP ("NSEP 1", "N 9") are normal, whereas the following components of the cervical response ("NSEP 2 + 3", "N 11 + N 13") are reduced or absent, depending on the severity of the functional impairment (Fig. 1). Neurotization of elements of the brachial plexus.

Satisfactory therapy for an avulsion injury of the brachial plexus has yet to be described. Dorsal root entry zone lesions will usually mitigate the searing pain which is so disabling in some of these patients. Neurotization procedures are effective in restoring limited function to these patients. The most useful isolated movement of the upper extremity is elbow flexion, which is thus the primary target of neurotization procedures. Intercostal nerves and elements of the cervical plexus are the most commonly used donor nerves for neurotization procedures. From our experience and from a review o…

Transfer of the peroneal component of the sciatic nerve in total brachial plexus lesion: An anatomical feasibility study.

Closed brachial plexus lesions (BPLs) are generally associated with a traumatic mechanism of forced traction between the neck and the shoulder-arm complex. For brachial plexus reconstruction different techniques have been proposed with donor motor nerves like intercostal nerves, or the ipsilateral cervical plexus, the phrenic nerve, the contralateral C7 root, and many others. Despite all these surgical possibilities, the overall recovery is generally poor and not satisfactory. The principal drawback is linked to the loss of upper limb proprioception, in a way that dramatically influences even …

Evidence for vagal enkephalinergic neural control of the feline pylorus and stomach.

The atropine- and guanethidine-resistant contraction of the cat pyloric sphincter induced by efferent stimulation of the cervical vagi, was studied with a new method for measuring transpyloric flow. The pyloric contraction was blocked in a dose-dependent manner by the opiate antagonist naloxone infused intraarterially to the pyloric region. Local intraarterial injection of morphine or enkephalinamide elicited a pyloric and gastric contraction. Leu- and met-enkephalin intraarterially elicited a pyloric contraction and a gastric relaxation. These effects could be prevented or reversed by naloxon

Thoraco-cervico-brachial confined spaces an anatomic study.

A description is made on narrow spaces through which the brachial plexus and subclavian-axillary vessels run from the cervical spine up to the inferior border of the pectoralis major muscle. The spaces are: "Pleural suspensory Apparatus", interscalene spaces (vascular and neural), costo-clavicular, clavi-pectoral, retro-pectoralis minor, pre-humeral head, median nerve roots compass, pre-scalene space. Compressions with or without cervical rib, static or dynamic, are also described. Downward migration of shoulder girdle and muscle variations are analyzed. Forced positions which may produce comp

An indication for continuous cervical paravertebral block (posterior approach to the interscalene space).

We present a patient who required perioperative analgesia with continuous nerve block for shoulder disarticulation, for whom the only approach possible to the brachial plexus was from posterior. A 51-yr-old woman was suffering from intractable upper extremity pain and dysfunction as a result of severe lymphedema after metastatic spread of breast cancer to the axilla. Her pain was poorly controlled despite aggressive treatment with oral, systemic, and intrathecal opiates. She presented for amputation of her arm as a last resort for management of pain. In order to provide optimal postoperative a

[Cervical Lesions or Peripheral Nerve Disorders? How Far Can Neurological Examination Localize the Lesion?].

Accurate lesion localization is a fundamental component of neurological diagnosis. This review describes how neurological examination can be used to differentiate cervical spinal cord lesions, cervical radiculopathy, brachial plexopathy, and peripheral nerve disorders in patients presenting with upper-limb sensory and motor symptoms. Particular attention is given to the distribution of sensory impairment, tendon reflex abnormalities, and patterns of muscle weakness. The characteristic clinical features of cervical spondylotic myelopathy, cervical radiculopathy, brachial plexus disorders, and e

Postradiation neural sheath sarcoma of the brachial plexus: a case report.

Sarcoma arising in the brachial plexus is a rare complication following radiation therapy, with only 7 patients reported in the literature. The absolute number of patients is likely to increase with the growth of radiation therapy to treat breast cancer. Because of its rarity, early diagnosis requires a high degree of suspicion. We present a patient with neural sheath sarcoma of the brachial plexus arising 18 years after external beam radiation for breast cancer. The interval between presentation and definitive therapy was prolonged by workup and treatment of carpal tunnel syndrome and cervica

Post-operative emergence of acute brachial neuritis following posterior cervical laminectomy with fusion: A case report and review of the literature.

INTRODUCTION: Idiopathic brachial plexus neuritis or neuralgic amyotrophy is a rare neurological condition whose true etiology currently remains unknown. Epidemiologically, the incidence of this condition is exceptionally rare with only 1.6 cases for every 100,000 people (Turner and Parsonage, 1987). Symptoms present an initial acute and sudden pain to the shoulder girdle and upper arm which is followed by a sense of profound weakness and numbness to the upper arm (Parsonage and Turner, 1948). Localized neuropathy within the arm-pit region may also be presented. The pain often exacerbates upon

[True neurological thoracic outlet syndrome].

The thoracic outlet syndrome (TOS) encompasses various clinical entities affecting the neurovascular bundle crossing the thoracic outlet. Unfortunately, this term often proves to be confusing because many of these entities have little in common beyond their known or presumed lesion site. Neurogenic TOS (true TOS) is caused by compression of the lower trunk in the brachial plexus, the cervical ribs or fibrous band. This syndrome is extremely rare. We consider that this neurological form of TOS is a clearly defined neurological syndrome. We report 10 patients with true TOS. All were females. Sta
