True neurogenic thoracic outlet syndrome (nTOS) is a rare condition resulting from brachial plexus compression, frequently associated with congenital anomalies such as cervical ribs or fibrous bands. We report a unique case of nTOS involving both a cervical rib and fibrous band, complicated by an incidental intraoperative finding of a fascicular rupture in the lower part of lower trunk. Notably, the patient had no history of trauma but had undergone multiple sessions of manual therapy and a single extracorporeal shockwave therapy to the cervical region, raising concerns about a potential iatro

During a 20-year period from 1974 to 1994, 37 thoracic outlet decompressions were performed. There were 28 females and six males (ratio 5:1). The median age was 37 years (range 15-64). Symptoms were predominantly neurological in 29 limbs (78%), arterial in five limbs (14%) and venous in three limbs (8%). Limb pain and paraesthesia were the most common symptoms. Surgical decompression was performed via a supraclavicular approach in 24 limbs (65%) and a transaxillary approach in 13 limbs (35%). A cervical rib was excised in 21 limbs (57%), a first rib in 10 limbs (27%), a cervical and first rib

Identifying the exact cause for persistent and recurrent neurogenic thoracic outlet syndrome (NTOS) is challenging even with high-resolution imaging of the thoracic outlet. Improvement can be achieved with redo first rib resection, although the posterior first rib remnant is one of several potential points of brachial plexus compression. In approaching reoperative surgery for NTOS, the aim is to provide complete thoracic outlet decompression as guided by the patient's history, physical examination, and adjunctive imaging. This may involve resection of the posterior first rib remnant, scar tiss

In this article, the authors report 8 cases of abnormal first rib, producing mainly arterial vascular disorders. The costal abnormalities, the clinical vascular disorders and the arteriographic signs are then reviewed. Treatment was then discussed and the methods of resection of the cervical rib are noted.

BACKGROUND: Endoscopic-assisted transaxillary first rib resection is a novel approach in the management of thoracic outlet syndrome (TOS) and allows us to safely identify the neurovascular package and different structures. Our main objective is to assess the results of morbidity and mortality of the surgical treatment in TOS with this technique.
METHODS: We carried out a prospective, longitudinal study with 22 surgical interventions of transaxillary first rib resection with endoscopic support in patients with TOS from January 2000 to January 2009 in a private hospital located in Puebla, Mexic

Paresthesia, pain and coldness of the upper limb strongly suggest ischemia. The questions raised concern the etiology and treatment. 1) Treatment is based on confirmation of the unilateral nature of the disorder requiring search for the a locoregional cause (examination of the subclavian outlet) and on elimination of a cardiac or general origin. Two noninvasive examinations are indicated. An X-ray examination of the upper chest bones is performed to search for an abnormal formation, usually a cervical rib. Presence of a cervical rib indicates a possible damage to the sub-clavian artery in the

The ribs are essential structures of the osseous thorax and provide information that aids in the interpretation of radiologic images. Techniques for making precise identification of the ribs are useful in detection of rib lesions and localization of lung lesions. The big rib sign and the vertical displacement sign can be used to differentiate the right and left ribs on lateral chest radiographs. The clavicle, the xiphoid process, and the sternal angle may be used as anatomic landmarks for rib counting on computed tomographic scans. For rib counting on lateral chest radiographs, the sternal ang

Thoracic outlet syndrome (TOS) refers to a group of conditions caused by extrinsic compression of neurovascular structures at the thoracic outlet. Arterial TOS (aTOS) is the rarest form and may present with upper limb ischemic symptoms such as pain, coldness, pallor, and numbness, most often due to bony abnormalities such as a cervical rib. We report the case of a 19-year-old woman on combined oral contraceptive therapy who presented with acute ischemia of the right upper limb. Duplex ultrasound and computed tomography angiography revealed thrombosis of the right subclavian artery in the sett

The thoracic outlet syndromes encompass the diverse clinical entities affecting the branchial plexus or subclavian artery including cervical ribs or bands. Thoracic outlet syndrome are often difficult to diagnose on existing clinical and electrophysiological criteria and new diagnostic methods are necessary. This study reports our experience with magnetic resonance imaging (MRI) of the brachial plexus in 20 patients with suspected thoracic outlet syndrome. The distribution of pain and sensory disturbance varied widely, weakness and wasting usually affected C8/T1 innervated muscles, and electro

It is generally believed that trauma from child abuse or bone fragility from diseases such as osteogenesis imperfecta accounts for most cases of multiple rib fractures. We report an infant with unexplained fractures from the right 3rd rib through to the 8th rib who had undergone resection of a large cervical tumor and had been admitted to a neonatal intensive care unit. Neither trauma nor bone fragility disease was found in the patient during the detailed investigation conducted by the Malpractice Investigative Committee.

BACKGROUND: Thoracic outlet syndrome (TOS) is one of the most complicated diseases in thoracic surgery with regard to both diagnosis and treatment. Surgical removal of the first rib and cervical rib, if present, has been suggested as the treatment of choice in patients who do not benefit from physiotherapy. In this retrospective study, our surgical experience with TOS and the management of surgical complications are presented.
METHODS: A total of 109 cases with the diagnosis of TOS were operated on between January 1995 and April 2010. Ninety-five of the cases were females (87.2 %) and 14 (12.

Careful anatomic studies during 1,120 operations for thoracic outlet syndrome indicate that most TOS patients have anomalous fibromuscular bands near the brachial plexus and subclavian vessels that predispose them to neurovascular symptoms in the shoulder and upper extremity. Nine different types of these "soft tissue" anomalies not visible on roentgenograms have been identified during these operations. The anomalies often are multiple, and may or may not be associated with bone abnormalities as well. These anomalies are clearly associated with the symptoms of thoracic outlet syndrome, particu

Thoracic Outlet Syndrome (TOS) is a rare condition caused by compression of the neurovascular structures of the thoracic outlet. Patients with TOS are found to have a greater prevalence of supernumerary or abnormal first ribs compared to the general population. Symptoms can sometimes be incited by a neck injury, especially in patients with abnormal cervical anatomy. This case report highlights a unique patient with bilateral cervical ribs who presented with complicated symptoms following a neck injury who required a series of surgical interventions to improve.

BACKGROUND: Much published data exists on the position of cervicothoracic ganglion, but a little published research has been done on the cervicothoracic system of dog. Herein, we illustrated topographical position and shape of each ganglion of cervicothoracic system to determine the distribution of nerves dispersing from them on two sides, left and right.
MATERIALS AND METHODS: Our work designed on the usage of 10 healthy adult dogs. Left cervicothoracic sympathetic system is represented by two ganglia: caudal and middle ganglion, while the right system is represented by three ganglia: caudal

Congenital anomalies of the ribs are relatively common, and they can be divided in numerical (for example cervical ribs) or structural abnormalities (for example bifid ribs). These anomalies are usually asymptomatic. Literature on symptomatic bifid ribs is limited. We present the case of a 36-year old male without any relevant medical history or medication who was referred to our center. After lifting a heavy object, he experienced sudden onset pain on the right anterior chest, with associated shortness of breath. Computed tomography of the chest showed a significant hemothorax on the right si

Narrowing of the thoracic outlet may give rises to symptoms in the arm from compression of the nerves, the subclavian artery, and the subclavian vein. Anatomic studies show that nerve compression may affect fibers from T-1, C-8, and not infrequently C-7, providing an explanation for the diffuse nature of the nerve symptoms in the arm. Relief from this compression at the thoracic outlet is readily accomplished by transaxillary resection of the first rib together with any associated cervical rib, fibrous cervical rib analogues, or other fibrous bands encroaching on the neurovascular structures.
