The case of a 27 year old female, being pregnant the second time and suffering from myasthenia gravis since 1974 is reported. The patient was admitted to the hospital for repeat caesarian section. Anaesthesia of the preoxygenated patient during caesarian section was induced with 0.5% halothane and ketamine (2 mg/kg b.w.) intravenously. Muscle relaxing drugs (competitive and depolarizing inhibitors) as well as morphine derivates (e.g. fentanyl) and barbiturates are contraindicated. Therefore anaesthesia was maintained with 0.2:N2O = 1:1 combined with 0.5% halothane up to the point of ligation o

A German court (Oberlandesgericht Hamm, 4. 11. 1981) has ruled that a gynaecologist cannot be accused of having committed an error in treatment if he does not recommend Caesarean section in a multipara and breech presentation if the birth proceeds in all other respects in a normal manner and if there is no disproportion between the child and the maternal pelvis. Vaginal birth in case of breech presentation in a women who has been pregnant six times and has given birth to three children, is in accordance with good medical practice. It was also ruled that the obstetrician cannot be considered at

To assess the risks and benefits of vaginal delivery in patients with prior cesarean section, the English literature on this subject from 1950 to 1980 was reviewed. The following conclusions were reached: 1) Properly conducted vaginal delivery after cesarean section is relatively safe, with a 0.7% incidence of uterine rupture, 0.93 perinatal mortality, and no maternal deaths due to uterine rupture. 2) Of those patients allowed a trial of labor, 66.7% were successfully delivered vaginally. Successful vaginal delivery occurred in 74.2% of those patients with a nonrecurrent indication for their p…

The incidence of post-cesarean section febrile morbidity was prospectively studied in 774 patients undergoing cesarean section during 1 year. The overall morbidity was 20.5%. Postoperative endometritis was found in 9.8%, bacteriuria in 7.3%, and wound infection in 6.8% of the patients. A highly significant (P < .001) correlation between the rate of postoperative endometritis and duration of labor, number of vaginal examinations, and number of rectal examinations was found. Parity (P < .01), rupture of the membranes (P < .05), and skill of the operator (P < .05) were significantly correlated to…

BACKGROUND: Psoas muscle abscess is a rare but potentially life-threatening condition and is exceptionally uncommon after cesarean section. Its nonspecific clinical presentation may delay diagnosis.
CASE PRESENTATION: A 37-year-old multiparous woman (gravida 4, para 2, with two previous spontaneous abortions) at 34 weeks of gestation underwent emergency repeat cesarean section for pathological cardiotocography, uterine hypertonia, threatened fetal asphyxia, and previous cesarean section. The pregnancy was complicated by pregnancy-induced hypertension. The cesarean section was uncomplicated, a…

This case report presents the diagnosis and management of a 41-year-old woman who developed a vesicouterine cutaneous fistula after a cesarean section. She initially presented with pus discharge from the cesarean wound site, and later exhibited cyclical hematuria and fluid leakage from the skin incision, prompting further investigation. Diagnostic modalities, including computed tomography with contrast, confirmed the presence of a complex fistula involving the bladder, uterus, and skin. Conservative management with gonadotropin-releasing hormone therapy was attempted but discontinued due to wo…

In a nonendemic setting, the confluence of malaria and pregnancy presents unique anesthetic challenges, particularly when the infection is undiagnosed at the time of an urgent cesarean section. This report involves a woman in her early 30s at 39 weeks of gestation with no prior health issues, who developed malarial symptoms upon returning from Haiti five months before. During labor, severe fetal heart rate decelerations necessitated immediate surgical intervention. Given the patient's febrile state, rapid induction with etomidate and succinylcholine was selected to minimize hemodynamic instabi…

Spinal needle fracture during neuraxial anesthesia is a rare but potentially serious complication. We report the case of a term primigravida with scoliosis and obesity who underwent labor epidural analgesia followed by spinal anesthesia for cesarean section. Multiple attempts were required before cerebrospinal fluid return was achieved, and a local anesthetic was administered. Upon withdrawal, the spinal needle fractured, leaving a fragment in the paravertebral musculature. The fragment was successfully removed endoscopically on postpartum day three without sequelae. This case highlights risk …

Uterine rupture is an intrinsically biomechanical process associated with high maternal and fetal mortality. A previous Cesarean section (C-section) is the main risk factor for uterine rupture in a subsequent pregnancy due to tissue failure at the scar region. Finite element modeling of the uterus and scar tissue presents a promising method to further understand and predict uterine ruptures. Using patient dimensions of an at-term uterus, a C-section scar was modeled with an applied intrauterine pressure to study how scars affect uterine stress. The scar positioning and uterine thickness were v…

OBJECTIVES: This case report highlights the clinical presentation, diagnostic challenges, and effective management of bladder endometriosis, while emphasizing the importance of considering this diagnosis in patients with chronic pelvic pain and urinary symptoms.
METHODS: A 32-year-old woman presented with severe pelvic pain, dysuria, and dyspareunia. Diagnosis of bladder endometriosis was achieved through clinical suspicion supported by vaginal ultrasound, 3D imaging, and magnetic resonance imaging.
RESULTS: Conservative medical treatment provided temporary relief, which necessitated resecti

Background and Objectives: This study aimed to assess the impact of monopolar electrocautery on the fetus during cesarean section. Materials and methods: A retrospective analysis was conducted with 552 patients delivered by cesarean section. Patients were grouped based on usage of monopolar electrocautery. In 272 patients, monopolar electrocautery was used to separate the tissues before the delivery. In 280 patients, no electrocautery was used. Newborn vital signs, Apgar scores, umbilical cord blood parameters, newborn serum parameters collected within 6th postpartum hour, and rate of newborn …

OBJECTIVES: This article suggests a unified way to perform Cesarean sections. Even in the same departments, different modifications are in use. Therefore, one cannot rely on the early or late outcome of the procedure as long as all the surgical steps are not standardized.
METHODS: The Misgav Ladach (Stark) Cesarean Section presented here is an evidence-based operation. Its basic principles are a modified Joel-Cohen abdominal incision, one-layer continuous suturing of the uterus using a big needle, leaving peritoneum open, closing fascia continuously and a few Donati skin sutures.
RESULTS: Th…
