[Cost reduction in the assessment of thoracic pain with the chest pain unit in first aid].

BACKGROUND: The evaluation of chest pain is a constant challenge for cardiologists, due to its clinical, organizational, economical and legal implications. The costs of the diagnostic classification of non-specific thoracic pain during ordinary, albeit short, hospitalization are here compared with a quick screening of the same symptoms in a "Chest Pain Unit", which may form an appendix of the emergency room (ER). METHODS: The study involves patients admitted to the hospital used as reference during the year 2000 and discharged with a diagnosis of thoracic pain or precordial pain, as identifie…

Diaphragmatic cramp as a possible cause of noncardiac chest pain and referred mandibular pain.

The initial assumption that sudden acute chest pain may be of cardiac origin is justifiable, but when this proves not to be the case the patient is left with little explanation of the cause. It is suggested here that diaphragmatic cramp may be a cause of some undiagnosed noncardiac chest pains associated with mandibular referred pain. The phrenic nerve provides both motor and sensory innervation to the diaphragm, while the trigeminal nerve carries sensation from the mandibular teeth. Both nerves originate in separate nuclei close together in the lower medulla. Interconnections between these nu…

Successful Management of Chest Pain Through Cardiac Autonomic Rehabilitation.

BACKGROUND: Some patients experience persistent chest pain after complete coronary revascularization without evidence of ischemia. Dysautonomia may be the underlying cause. CASE SUMMARY: A 43-year-old man with recurrent chest pain after multivessel revascularization showed poor response to standard cardiac rehabilitation. Evaluation revealed significant autonomic dysfunction (sympathetic overactivation), not ischemia. After implementing an integrated autonomic-regulation protocol (low-intensity aerobic exercise combined with abdominal breathing, standing meditation, and mindful walking), his …

Atypical Chest Pain: A Diagnostic Challenge for Takotsubo Cardiomyopathy.

Takotsubo cardiomyopathy (TTC) typically presents in post-menopausal females with chest pain, dyspnea, or palpitations and often mimics acute coronary syndrome. This case report describes a unique presentation of TTC in an elderly patient who exhibited gastrointestinal and musculoskeletal symptoms without cardiopulmonary complaints. This constellation of symptoms has not been previously described in the literature. The patient was ultimately diagnosed with TTC with evidence of ST-T wave abnormalities on ECG, elevated serum troponin levels, a reduced left ventricular ejection fraction, the abse…

Aortic Dissection Masquerading as Musculoskeletal Chest and Back Pain.

Aortic dissection is a life-threatening condition that deteriorates rapidly following onset. Factors such as uncontrolled hypertension, hyperlipidemia, and genetic factors contribute to the development of the condition. This case report covers the history, presentation, and treatment of a 66-year-old male with a Stanford Type A aortic dissection. This case highlights the urgency of the condition and the cruciality of teaching recognition and prompt treatment to maximize patient survival.

Intramyocardial Bridging: An Overlooked Cause of Atypical Chest Pain.

Intramyocardial bridging (IMB) is a congenital anomaly characterized by the tunneling of a coronary artery segment through the myocardium, potentially leading to serious cardiac complications, such as myocardial ischemia, infarction, and sudden death, challenging the traditional view of it being benign. A case involving a 42-year-old man with a seven-day history of atypical chest pain highlights the significance of considering IMB in the differential diagnosis. Despite normal troponin levels, creatine kinase (CK), CK-MB, D-dimer, a negative drug screen, a normal ECG, and chest X-ray and no app…

Prehospital Chest Pain Management: Disparity Based on Homeless Status.

BACKGROUND: People experiencing homelessness may use emergency medical services to access health care. We sought to examine the relationship between homelessness and prehospital evaluation and treatment of chest pain. METHODS: We obtained 2019 data of all emergency medical services activations from a single 9-1-1 provider in San Francisco, California with a clinician's primary impression of chest pain. Using chart review, we categorized patients as experiencing homelessness or not and determined treatment rates between the two groups based on local chest pain/acute coronary syndrome protocol.…

Pulmonary sequestration: An uncommon presentation with acute chest pain.

This is a case of 18-year-old teenager presented with acute left sided chest pain for five days. This was associated with intermittent cough, and loss of weight in two weeks. Diagnosis was made by computed tomography of thorax plus angiogram that showed infected left intralobar pulmonary sequestration with lung abscess. Sputum culture grew Pseudomonas aeruginosa. He completed 14 days of antibiotic and subjected to feeding artery embolization. The aim of this case report is to highlight on the uncommon presentation and the need of high index of suspicion of pulmonary sequestration with support …

CT or Invasive Coronary Angiography in Stable Chest Pain.

BACKGROUND: In the diagnosis of obstructive coronary artery disease (CAD), computed tomography (CT) is an accurate, noninvasive alternative to invasive coronary angiography (ICA). However, the comparative effectiveness of CT and ICA in the management of CAD to reduce the frequency of major adverse cardiovascular events is uncertain. METHODS: We conducted a pragmatic, randomized trial comparing CT with ICA as initial diagnostic imaging strategies for guiding the treatment of patients with stable chest pain who had an intermediate pretest probability of obstructive CAD and were referred for ICA…

Gastric Volvulus: A Rare Etiology of Acute Chest Pain.

Источник пометил публикацию как отозванную

Chest pain is one of the most common complaints in the emergency department. The etiologies of chest pain include a wide spectrum of conditions, ranging from mild and self-limited to life-threatening conditions. Gastrointestinal origins of acute chest pain, including esophageal, gastric, and biliary conditions, are not uncommon. We present the case of a 67-year-old man who presented to the emergency department with chest pain of three hours duration with pressure-like quality. The pain was associated with sweating, palpitation, nausea, and vomiting. The past medical history of the patient was …

Dysphagia Lusoria: A Little Known Cause of Chest Pain.

Dysphagia lusoria is a congenital abnormality characterized by an aberrant right subclavian artery. It often presents as either an incidental finding on imaging or chronic dysphagia. We describe the case of a 66-year-old female who presented with severe chest pain, worse with swallowing, along with an ongoing globus sensation. She was found to have a negative cardiac workup for ischemia with a subsequent computed tomography angiogram (CTA) of the chest showing an abnormal right subclavian artery. We emphasize the unique diagnostic approach of this rare anatomical anomaly and its potential pres…

Functional Chest Pain and Esophageal Hypersensitivity: A Clinical Approach.

Functional chest pain, functional heartburn, and reflux hypersensitivity are 3 functional esophageal disorders defined by the Rome IV criteria. Specific criteria, combining symptoms and the results of objective testing, allow for an accurate diagnosis of these conditions. Management may include medications targeted at optimizing acid suppression or neuromodulation, as well as a host of complementary or alternative treatment options. Psychological and behavioral interventions, such as cognitive behavioral therapy and hypnotherapy, have displayed substantial benefits in the treatment of function…

Cholecystitis Masquerading as Cardiac Chest Pain: A Case Report.

BACKGROUND Cope's sign is the association of bradycardia with symptoms of acute cholecystitis, which can occur due to a vagal cardiobiliary reflex. The clinical and electrocardiographic changes of bradycardia or complete heart block can mimic the presentation of acute coronary syndrome. This report highlights the unique possibility that bradycardia in patients with abdominal pain and gallstones can be due to this reflex. CASE REPORT A 46-year-old obese man with hyperlipidemia and gallstones presented with chest pain suggestive of cardiac ischemia. The initial electrocardiography (EKG) was norm…

Chronic chest pain after percutaneous atrial septal defect closure.

Percutaneous closure of atrial septal defect is recognised as a safe and effective procedure, however, in some patients complications may occur. Although chest pain has been sporadically reported, its exact aetiology has been poorly studied. Herein, a 14-year-old female with an atypical and long-lasting chest pain after percutaneous atrial septal defect closure is described.

A 37-Year-Old Man With Pleuritic Chest Pain.

A 37-year-old man with poorly controlled type 2 diabetes presented with severe right-sided pleuritic chest pain, respiratory splinting, and cough. Two weeks earlier, he had been evaluated at an urgent care for cough and was prescribed a 5-day course of azithromycin for bronchitis. He then presented to our ED reporting mild, right-sided pleuritic chest pain. Vital signs were normal, and his chest radiograph showed a trace right pleural effusion (Fig 1A). He was discharged with naproxen for pleurisy. Three days later, he returned, reporting a dramatic increase in the severity of his pleuritic ch…
