Chloramphenicol Contamination in Milk: Measurement, Methods, and Regulatory: A Systematic Review.

Chloramphenicol (CAP) is a broad-spectrum antibiotic with applications in human and veterinary medicine. However, due to its severe adverse effects, including fatal aplastic anemia, its use in food-producing animals has been banned in many countries. Despite these regulations, CAP residues have been detected in milk, raising concerns about food safety and regulatory compliance. This systematic review aimed to evaluate the presence of CAP in milk, detection methods, and regulatory limits. A comprehensive literature search identified 918 studies, of which 27 met the inclusion criteria. The inclu…

Bidirectional and regiospecific C21-modification of steroids by a chloramphenicol O-acetyltransferase.

Bidirectional acetylation and deacetylation of small molecules by acetyltransferases (ATs) remain poorly understood. In this study, we present the structural, functional, and computational characterization of a chloramphenicol O-AT from Bacillus sp. PAMC22265 (AT65) that exhibits bidirectional catalytic activity toward the regiospecific C21-acetylation and deacetylation of steroid substrates. The crystal structure of AT65 was determined at 2.40 Å resolution (PDB ID: 24ZY), revealing a trimeric architecture in which the putative active-sites located at the interfaces between adjacent subunits. …

Structural insights into context-dependent inhibitory mechanisms of chloramphenicol in cells.

Ribosome-targeting antibiotics represent an important class of antimicrobial drugs. Chloramphenicol (Cm) is a well-studied ribosomal peptidyl transferase center (PTC) binder and growing evidence suggests that its inhibitory action depends on the sequence of the nascent peptide. How such selective inhibition on the molecular scale manifests on the cellular level remains unclear. Here, we use cryo-electron tomography to analyze the impact of Cm inside the bacterium Mycoplasma pneumoniae. By resolving the Cm-bound ribosomes to 3.0 Å, we elucidate Cm's coordination with natural nascent peptides an…

Hydrolysis of amphenicol and macrolide antibiotics: Chloramphenicol, florfenicol, spiramycin, and tylosin.

Antibiotics that enter the environment can present human and ecological health risks. An understanding of antibiotic hydrolysis rates is important for predicting their environmental persistence as biologically active contaminants. In this study, hydrolysis rates and Arrhenius constants were determined as a function of pH and temperature for two amphenicol (chloramphenicol and florfenicol) and two macrolide (spiramycin and tylosin) antibiotics. Antibiotic hydrolysis rates in pH 4-9 buffer solutions at 25°C, 50°C, and 60°C were quantified, and degradation products were characterized. All of the …

Chloramphenicol eye drops in the treatment of conditions indicative of maxillary sinusitis.

BACKGROUND: Our own clinical experience of general practice over the last 15 years has indicated that chloramphenicol eye drops may have a favourable effect on many patients troubled by symptoms indicative of acute maxillary sinusitis. We wanted to conduct a pilot study to test whether this observation could be verified. MATERIAL AND METHOD: Treatment with chloramphenicol eye drops or systemic peroral antibiotics was tested on patients with symptoms indicative of acute maxillary sinusitis. The patients were randomised to two groups, one of which received systemic peroral antibiotics, the othe…

Evaluation of efficacy and safety of ciprofloxacin ophthalmic solution versus chloramphenicol.

The results of this clinical study demonstrate that ciprofloxacin ophthalmic solution 0.3% is as safe and as effective as 0.5% chloramphenicol ophthalmic solution in the treatment of conjunctivitis and blepharitis of bacterial aetiology. Both agents achieved microbiological improvement rates in excess of 90% after 1 week's treatment. On ciprofloxacin 93.5% of patients were judged clinically cured or improved versus 84.6% on chloramphenicol after 1 week. There were no serious adverse affects. One patient in each group suffered drug-related side-effects (chemosis, erythema) which resolved on dis…

Effect of penicillin and chloramphenicol on the growth and endotoxin release by N. meningitidis.

The action of two antibiotics, penicillin and chloramphenicol, on bacterial growth and endotoxin liberation was studied in 18 strains of Neissevia meningitidis isolated from blood and CSF of patients with meningococcal infections. The antibiotics were administered both separately and in combined form in doses equivalent to 1 MIC and 100 MIC. Penicillin was found to produce a faster and more intense bactericidal effect than chloramphenicol during the first hour, whereas at 12 h these differences were not significant. This could explain the initial worsening observed in some infected patients wh…

Interactions between chloramphenicol, acepromazine, phenylbutazone, rifampin and thiamylal in the horse.

The potential for interactions between chloramphenicol, phenylbutazone, acepromazine and thiamylal and chloramphenicol, rifampin, and phenylbutazone were evaluated in two groups of experiments. In the first, five horses were given thiamylal intravenously (iv) (6.6 mg/kg) after pretreatment with acepromazine, and the time of recumbency was determined. Administration of chloramphenicol iv (25 mg/kg) 1 h prior to anaesthesia significantly lengthened the recumbency time from 21.8 +/- 4.8 mins to 36.0 +/- 8.3 mins. There was an apparent but not statistically significant decrease in recumbency time …

Hypotension produced by rapid intravenous administration of chloramphenicol in anaesthetised dogs.

Rapid intravenous administration (60 mg s-1) of chloramphenicol (50 mg kg-1) in a 40 per cent w/v polyethylene glycol (400 to 600), 30 per cent ethyl alcohol, 2 per cent benzyl alcohol and 28 per cent distilled water vehicle produced a transient but significant decrease in systemic arterial blood pressure and heart rate with no effect on central venous pressure in sodium pentobarbital anaesthetised dogs. The vehicle alone had no significant effect on any of the parameters studied. Various approaches including the use of anticholinergic, antihistaminergic, antiadrenergic and ganglion blocking d…

Randomized treatment of patients with typhoid fever by using ceftriaxone or chloramphenicol.

Sixty-three patients with Salmonella typhi infections were randomly assigned to receive either ceftriaxone iv in single daily doses of 75 mg/kg for children and 3-4 g for adults for seven days or to receive 60 mg of chloramphenicol/kg a day orally or iv in four divided doses until defervescence and then 40 mg/kg a day to complete 14 d. In the ceftriaxone group, one death occurred, and two of seven patients still febrile 11 d after starting treatment were given chloramphenicol. In the chloramphenicol group, one death and one gastrointestinal perforation occurred. The probability of remaining fe…

Hypersensitivity reaction following chloramphenicol administration in a patient with typhoid fever.

Hypersensitivity reactions due to chloramphenicol are rarely reported in the literature. We present a case report of a patient with typhoid fever who experienced a hypersensitivity reaction subsequent to the infusion of chloramphenicol sodium succinate. The patient had previously reacted similarly to ampicillin infusion. A brief review of reported cases of chloramphenicol hypersensitivity in the English-language literature, as well as possible alternative explanations in this case, are provided.

Chloramphenicol, lincomycin and oxytetracycline disposition in calves with experimental pneumonic pasteurellosis.

The effects of pneumonia on the pharmacokinetics of chloramphenicol, lincomycin, and oxytetracycline were evaluated in two-month-old calves. Pneumonia was induced by injection of Pasteurella haemolytica cultures directly through the thoracic wall into each lung. Six days prior to induction of pneumonia, the antibiotics were administered in a single i.v. dose. The antibiotics were administered again 48 (i.v.), 60 and 72 h (i.m.) following injection of P. haemolytica. The pharmacokinetics of chloramphenicol (25 mg/kg) and lincomycin (10 mg/kg) were not significantly different in calves with pneu…

Ceftriaxone versus ampicillin and chloramphenicol for treatment of bacterial meningitis in children.

78 patients with bacterial meningitis were evaluated in a prospective, randomised study comparing twice-daily ceftriaxone as single-drug therapy with ampicillin and chloramphenicol given every 6 h. The groups were comparable in age, sex, days of illness before admission, and bacterial colony counts in cerebrospinal fluid (CSF). The pathogens were Haemophilus influenzae type b (54 cases), streptococci (9 cases), meningococci (9 cases), and unknown (6 cases). In 40 CSF specimens obtained 4-12 h after initiation of therapy, cultures were negative in 57% of the ceftriaxone patients and in 42% of t…

Antagonism of ampicillin and chloramphenicol for meningeal isolates of group B streptococci.

The increasing prevalence of ampicillin-resistant Haemophilus influenzae type b has led to the recommendation that ampicillin and chloramphenicol be given as the initial therapy for suspected bacterial meningitis in infants and children. However, during the first 2 months of life, H. influenzae type b is a rare cause of meningitis, whereas group B streptococcus is the most frequently isolated agent. Since ampicillin and chloramphenicol have been shown to be antagonistic for other streptococci, an in vitro study of their effect on group B streptococci was performed. The effect of ampicillin and…
