Surgical treatment for distal clavicle fracture with coracoclavicular ligament disruption.

BACKGROUND: Surgical reconstruction is usually indicated for distal clavicle fractures with coracoclavicular ligament disruption due to a high rate of nonunion and delayed union. We report the outcome of a surgical technique for this type of fracture. METHODS: The procedures consist of coracoclavicular reconstruction with a Mersilene tape, repair of torn coracoclavicular ligament, and wire fixation of the fracture fragments. From 1993 through 1998, this technique has been used on 13 patients with distal clavicle fracture and associated coracoclavicular ligament disruption. Eleven patients wit…

Outcomes of conservatively treated midshaft clavicle fractures with butterfly fragment.

OBJECTIVES: The aim of this study was to evaluate whether fracture shortening, displacement, and the length of butterfly fragments were reliable radiographic indicators of secondary healing failure in displaced midshaft clavicle fractures with butterfly fragments and to determine whether these radiographic parameters were effective in predicting healing disorders and could be utilized as prognostic factors. PATIENTS AND METHODS: Between January 2015 and January 2020, a total of 31 adult patients (29 males, 2 females; mean age: 43.6±13.2 years; range, 21 to 74 years) who presented with a close…

OSTEOSYNTHESIS OF CLAVICLE FRACTURES IN CHILDREN USING TITANIUM ELASTIC NAILS.

OBJECTIVES: Titanium Elastic Nail (TEN) fixation provides superior clinical and cosmetic outcomes with fewer complications compared to plating or Kirschner wire fixation in pediatric clavicle fractures. BACKGROUND: Clavicle fractures account for about 10% of all paediatric fractures. In displaced or unstable cases, surgical fixation is increasingly advocated. Titanium elastic nails (TEN) offer a minimally invasive intramedullary fixation option, but outcomes in children remain less widely documented. DESIGN: Retrospective cohort design study. SETTING: Single tertiary care center. PATIENT S…

Medial Femoral Condyle Flap as Salvage for Osteomyelitic Clavicle Nonunion.

Clavicle nonunion with osteomyelitis is a challenging condition, combining bone infection and structural defects. Traditional 2-stage procedures address these issues but may be unsuitable for patients unable to undergo multiple operations. The medial femoral condyle (MFC) flap has emerged as an effective option, offering both vascularized bone grafting and mechanical support in a single-stage approach, integrating infection control and bone reconstruction. We report a 21-year-old man with right clavicle nonunion and osteomyelitis following internal fixation for a fracture sustained in a motorc…

Functional Outcomes of Clavicle Open Reduction and Internal Fixation (ORIF).

BACKGROUND: The clavicle is among the most commonly fractured bones. It can be managed nonoperatively or surgically. Open reduction and internal fixation (ORIF) surgery is the technique to help restore the clavicle's anatomy and allow patients to rehabilitate sooner. The current study aims to assess the functional results and problems related to ORIF using plates for displaced midshaft clavicle fracture. METHODOLOGY: This retrospective study was conducted in Heartlands Hospital, Birmingham, United Kingdom. A total of 60 patients who underwent ORIF for displaced midshaft clavicle fracture in B…

Simultaneous bilateral midshaft clavicle fractures (Allman Type I): Case series.

RATIONALE: A simultaneous bilateral fracture of the middle clavicle is a very rare injury in clinical practice, and the necessity of surgical intervention and treatment modality remains a topic of debate. PATIENT CONCERNS: We report a case of multiple injuries in a 40-year-old woman following a road traffic accident and another case of polytrauma in a 46-year-old man following a collision injury. DIAGNOSES: The radiographs of the bilateral shoulder joints and the 3-dimensional reconstruction CT of the chest showed the fractures in the middle third of the clavicle on both sides. In addition, …

Simultaneous bilateral traumatic clavicle fractures: incidence, characteristics, and surgical outcomes.

BACKGROUND: Although clavicle fractures are common injuries in adults, simultaneous bilateral clavicle fractures are rarely reported. The present report describes 13 patients with simultaneous bilateral traumatic clavicle fractures who were treated with surgical management and followed for more than 12 months postoperatively. METHODS: This retrospective chart review involved skeletally mature patients with traumatic clavicle injuries. Patients with bilateral clavicle fractures who were followed up for at least 12 months after surgery were included. Data regarding the patients' demographics, i…

Surgical treatment of bipolar segmental clavicle fracture: A case report.

BACKGROUND: Shoulder injuries caused by trauma are common, including clavicle fractures. Even so, bipolar segmental fracture of the clavicle is extremely rare and seldom reported. Therefore, there is still a controversial issue about how to treat these bipolar segmental clavicle fractures. CASE SUMMARY: A 56-year-old security guard arrived at our emergency room after falling on his electric bicycle. There was no loss of consciousness or other pain. He had no numbness in his fingers. X-rays and 3D computed tomography revealed that the patient's right shoulder had a bipolar segmental clavicle f…

Dual Plate Minifragment Fixation of Clavicle Fractures- Technique and Approach.

The treatment of midshaft clavicle fractures continues to evolve, with a variety of fixation techniques and constructs used. Symptomatic hardware is a common complication related to plate fixation of the clavicle, and often results in secondary surgery for implant removal. The minifragment dual plate fixation technique for clavicle fractures provides a construct that may result in fewer implant related complications with comparable fixation stability to precontoured locking plates. Our case presentation demonstrates our approach to midshaft clavicle fractures, the benefits of this surgical tec…

A new approach to predicting shoulder dystocia: fetal clavicle measurement.

BACKGROUND/AIM: This study aimed to evaluate the relationship between second- and third-trimester clavicle lengths and birth weight and shoulder dystocia. MATERIALS AND METHODS: This prospective observational study included 181 patients who presented to the Private Etlik Lokman Hekim Hospital for routine pregnancy visits between March 2019 and March 2020. In addition to routine pregnancy examinations, the patients also underwent ultrasonography twice at weeks 20–23 and 33–36 to determine the length of the fetal clavicle. The patients were evaluated for shoulder dystocia in the second stage of…

Clavicle fracture following neck dissection: imaging features and natural course.

OBJECTIVE: This study aimed to assess the imaging features and natural course of clavicle fracture following neck dissection (ND). METHODS AND MATERIALS: Eight patients with clavicle fracture following ND were included. Because bilateral clavicle fractures occurred in one patient, a total of nine clavicle fractures following ND were assessed using CT images. Pathological fracture due to bone metastasis or local recurrence was carefully ruled out. RESULTS: The time interval from ND to clavicle fracture ranged from 2 to 8 months (median, 4 months). On CT images, all the nine fractures occurred…

Does altering projection of the fractured clavicle change treatment strategy?

BACKGROUND: Shortening of the fractured clavicle is proposed and debated as an indicator for surgical intervention. There is no standardized or uniform method for imaging and measuring shortening. Different methods and techniques can lead to different measured outcomes. However, the question remains whether a difference in measured shortening using a different technique has any short-term clinical relevance in terms of treatment strategy. The aim of this study was to investigate whether a different projection of the same midshaft clavicular fracture would lead to a different choice in treatmen…

Clavicle Kirschner Wire Migration into Left Lung: A Case Report.

BACKGROUND Kirschner wires are often used to perform osteosynthesis. Migration through tissue of these wires is a rare but well-known occurrence. CASE REPORT A 65-year-old female presented with light intensity pain complaints in the upper left chest area; personal history included left clavicle fracture 20 years ago that was treated surgically with fixation using a K-wire. Chest radiography showed the presence of metallic foreign body in the left pulmonary apex. An exploratory axillary thoracotomy was performed, and the foreign body was extracted by a pneumotomy. CONCLUSIONS To obtain satisfac…

Scapulothoracic Dissociation and Clavicle Fracture with Associated Brachial Plexus Palsy.

Scapulothoracic dissociation is the traumatic separation of the scapula from the thoracic wall. It is the outcome of rare, high-energy trauma to the shoulder girdle and may include vascular and neurologic damage of the upper extremity. Of the few cases presented in the literature, the majority refer to polytrauma patients. We present a rare variant of scapulothoracic dissociation combined with comminuted fractures of both the scapula and the clavicle associated with brachial plexus palsy. There were also vertebral fractures of the spinous processes of C6, T4, T5, and T6, of the transverse proc…

Intramedullary nail fixation of non-traditional fractures: Clavicle, forearm, fibula.

Locked intramedullary fixation is a well-established technique for managing long-bone fractures. While intramedullary nail fixation of diaphyseal fractures in the femur, tibia, and humerus is well established, the same is not true for other fractures. Surgical fixations of clavicle, forearm and ankle are traditionally treated with plate and screw fixation. In some cases, fixation with an intramedullary device is possible, and may be advantageous. However, there is however a concern regarding a lack of rotational stability and fracture shortening. While new generation of locked intramedullary d…

Outcome of distal end clavicle fractures treated with locking plates.

PURPOSE: Fractures of the lateral end of the clavicle are relatively uncommon. These fractures are unstable due to the various deforming forces which act on the fragments as well as the small distal fracture fragment. At most times the deforming forces are not taken into consideration, and the fracture is not fixed securely. In this study, we assessed a fixation technique using the precontoured locking plates to find out whether it provided a stable fixation with good functional outcome. METHODS: Totally, 32 patients with lateral end clavicle fracture (Neer's Type II) were included in the stu…

Does Subacromial Osteolysis Affect Shoulder Function after Clavicle Hook Plating?

PURPOSE: To evaluate whether subacromial osteolysis, one of the major complications of the clavicle hook plate procedure, affects shoulder function. METHODS: We had performed a retrospective study of 72 patients diagnosed with a Neer II lateral clavicle fracture or Degree-III acromioclavicular joint dislocation in our hospital from July 2012 to December 2013. All these patients had undergone surgery with clavicle hook plate and were divided into two groups based on the occurrence of subacromial osteolysis. By using the Constant-Murley at the first follow-up visit after plates removal, we eval…

SUBCLAVIAN VEIN THROMBOSIS FOLLOWING FRACTURE OF THE CLAVICLE: CASE REPORT.

Deep vein thrombosis in the upper limbs is uncommon in the orthopedic literature. We report on a case of subclavian vein thrombosis that occurred during conservative treatment of a fracture in the middle third of the clavicle. This is difficult to diagnose and requires a high degree of suspicion. Treating it may prevent fatal thromboembolism. In some rare cases, it has been described in association with fractures of the clavicle.

Displaced mid-shaft clavicle fractures: a subset for surgical treatment.

ABSTRACT: Recent studies have shown a high prevalence of symptomatic malunion and nonunion after nonoperative treatment of displaced midshaft clavicle fractures. We sought to study the result of surgical treatment of displaced mid-shaft fracture of clavicle. Forty mid shaft clavicle fractures in adults, displaced with initial shortening of more than 15 mm, were treated with primary internal fixation with plates. The inclusion criteria were displacement, shortening of clavicle of more than 15 mm and/or impending skin penetration. The follow-up period varied from 6 months to 24 months. Fracture …
