The reasons for triple therapy in stable COPD patients in Japanese clinical practice.

BACKGROUND: Triple combination therapy involving long-acting muscarinic antagonists long-acting β2 agonists, and inhaled corticosteroids has recently become an option for maintenance treatment of COPD. Some add-on clinical trials have reported the benefits of these combinations. However, the process to step up to triple therapy varies for individual cases. METHODS: Keio University and affiliated hospitals conducted an observational COPD cohort study, recruiting patients diagnosed as having COPD by pulmonary physicians and those referred for investigation of possible COPD. Their prescription h

Assessing clinical trial failure risk factors and reasons in gastric cancer.

BACKGROUND: Gastric cancer is one of the leading cancer-related death causes. Enormous efforts have been focused on this field in these years. However, clinical trial failure is becoming a massive obstacle for researchers to apply their research results for clinical use. This study aimed to analyze the reasons behind clinical failures and identify potential risk factors of clinical trial failures. METHODS: On December, 1, 2021, we queried ClinicalTrials.gov for gastric cancer listed in phase II/III. We included trials specifying their interests in "stomach cancer", "Stomach Neoplasms", "Gastr

'Sick and tired': Patients reported reasons for not participating in clinical psychiatric research.

BACKGROUND: Meaningful and generalizable research depends on patients' willingness to participate. Studies often fail to reach satisfactory representativeness. OBJECTIVE: This paper aims to investigate reasons for not participating in research among young adult patients with psychiatric illness. METHOD: A quantitative cross-sectional study was performed based on questionnaires reported on by 51 psychiatric patients (14 males, 35 females and two unspecified) who had previously declined participation in an ongoing research project. Thereafter, a qualitative interview with subsequent content an

Clinical audit on tonsils and adenoid surgery. Is day surgery a reasonable option?

Four hundred and five patients who underwent tonsillectomy and/or adenoidectomy over a 27-month period were reviewed retrospectively to determine local patient characteristics and complication rates. The predominance of adult patients (mean age of 23.8 years), male subjects (male to female ratio of 2:1), and tonsillectomy cases (85.4%) were in contrast to studies reported in Western countries. Chronic tonsillitis or adenoiditis was the most common indication for surgery and the most frequent complications encountered were fever (20.0%) and haemorrhage (5.2%). Three patients (0.7%) had signific

Early evidence for a multi-agent AI simulator for clinical reasoning practice: performance, consistency, and challenges.

OBJECTIVES: Clinical reasoning develops through repeated, deliberate practice, yet clinical and simulation environments are often limited by continuity, feedback, and scalability constraints. Large language models (LLMs) may address this by generating virtual clinical encounters accessible without live instructors or standardized patients but validity evidence remains limited. This study explored the performance of MAESSCR (Multi-Agent Educational Scenario Simulator for Clinical Reasoning), a platform where multiple LLM-based agents, each assigned a distinct role (i.e. patient, physical exam, …

Evaluation of causal reasoning for large language models in contextualized clinical scenarios of laboratory test interpretation.

This study evaluates causal reasoning in large language models (LLMs) using 99 clinically grounded laboratory test scenarios mapped to Pearl's Ladder of Causation: association, intervention, and counterfactual reasoning. We focused on common lab tests such as Hemoglobin A1c (HbA1c), creatinine, and vitamin D, and paired them with clinically relevant causal factors, including age, gender, obesity, and smoking. Two LLMs, GPT-o1 and Llama-3.2-8b-instruct, were tested, with responses rated by four medically trained human experts. GPT-o1 demonstrated superior discriminative performance (AUROC overa

Reconceptualizing Clinical Priority as Relational Influence: Derivation of Drawing Reasoning From a Sequential Programme of Research.

RATIONALE: Determining which patient problem requires attention first is a continuous cognitive task with direct consequences for patient safety. Despite its centrality, clinical prioritization remains conceptually underdeveloped. Organizational models frame it as protocol-driven; experiential models assume it improves with clinical exposure. Neither addresses the specific cognitive challenge that prioritization poses when patients present with multiple interacting conditions. AIMS AND OBJECTIVES: To reconceptualize clinical prioritization as a distinct cognitive task requiring relational int

From biomechanical mechanisms to clinical reasoning: Deciphering mechanobiological drivers of aortic valve calcification for precision therapy.

Calcific aortic valve disease (CAVD) is a degenerative cardiovascular disorder characterized by progressive valvular thickening and calcification, ultimately leading to aortic stenosis, heart failure, and high mortality. Despite associations with hyperlipidemia, inflammation, and bicuspid aortic valve (BAV) malformation, no targeted pharmacotherapies exist, indicating unaddressed core pathogenic mechanisms. The aortic valve is composed of valvular interstitial cells (VICs) and endothelial cells (VECs) which are constantly exposed to hemodynamic forces, with progressive extracellular matrix (EC

Clinical Reasoning: A 57-Year-Old Man With Chronic Gait Unsteadiness and Diminished Lower Extremity Sensation.

A 57-year-old man presented with a 5-year history of progressive gait unsteadiness, diminished lower extremity sensation, and a chronic cough. His symptoms began as episodic balance issues and visual disturbances but progressively worsened, leading to frequent tripping. Physical examination revealed sensory deficits in the lower extremities and cerebellar signs, including nystagmus, dysmetria, and a broad-based gait. Readers will walk through a stepwise approach to the case, developing a comprehensive differential diagnosis and questioning likely etiologies to arrive at a final diagnosis. This
