The reliability of skin testing in the diagnosis of penicillin allergy was studied in 86 adults and 167 children with a history of possible hypersensitivity reactions to penicillin. Nail scabies is an interesting clinical presentation of scabies. Although it is usually found concomitant with characteristic dermatologic manifestations, it may present as an isolated finding in the immunocompromised host. This condition is commonly mistaken with other diseases such as nail dystrophy, nail psoriasis and onychomycosis. We report two cases of unusual nail presentations that provide clues to diagnosis. Also, literature on unusual nail and dermatologic presentations was reviewed to recognize dermatologist consideration for appropriate treatment options.

We present a report of three cases with vesicles containing opalescent fluid grouped over the face and scattered on the trunk and limbs. Culture of the fluid aspirated from the vesicles grew Staphylococcus aureus in two of the three cases. The observation of vesicles grouped on the face in a sick febrile child should prompt the diagnosis of a severe staphylococcal septicemia.

Twenty-one patients older than 60 years of age with septic arthritis were reviewed over a ten-year period. The knee was the joint most commonly involved. Concurrent medical illnesses were noted in the majority of patients, with diabetes mellitus present in 24%. Seventy-one percent of patients had roentgenographic evidence of preexisting joint disease in the affected joint. A case of a 30-year-old woman who developed fever, diarrhoea and vomiting during the third and fourth days of menstruation, during which she was using tampons, is presented. The patient was admitted to hospital on the fifth day in shock , semicomatose, and with a generalized erythroderma. A presumptive diagnosis of toxic shock syndrome was made. Aggressive resuscitative and antibiotic therapy achieved a favourable outcome. An unused tampon from the same packet grew a Staphylococcus aureus of the same antibiogram and phage type as that isolated from the vaginal culture.
