Pediatric Cluster Headache Beyond Diagnostic Criteria: A Case Series and Review of Treatment Response.

Cluster headache is a headache disorder characterized by short-lasting, unilateral attacks in or around the eye accompanied by autonomic symptoms. As cluster headache in children is rare, it has been considered an adult disorder, causing significant diagnostic delays in children. Pediatric cases of cluster may not feature typical symptoms and can resemble migraine, so diagnoses can be difficult to make. In this case series, we report on four children (3 males and 1 female) diagnosed with cluster headache, including one who did not meet all International Classification of Headache Disorders, Th…

Exploring the potential of deep brain stimulation in managing cluster headache: a systematic review.

BACKGROUND: The debilitating nature of cluster headache (CH) is typified by intense, repeated cephalgia that are often referred to as "suicide headaches" owing to their severity. Individuals may not respond appropriately to therapeutic interventions despite various alternatives being readily available. This warrants an investigation into further management options. One of the promising solutions for CH has been identified as deep brain stimulation (DBS), which specifically targets the posterior hypothalamus. AIM: This systematic review aims to summarize the current literature on the safety an…

Clinical predictors of therapeutic failure of occipital nerve stimulation in refractory chronic cluster headache.

BACKGROUND: Occipital nerve stimulation (ONS) is a treatment with evidence in refractory chronic cluster headache (CCH). However, the variable response rate and cost make it necessary to investigate predictors of response. METHODS: This is a cross-sectional study conducted through the review of medical records of CCH patients from six hospitals in Madrid. Epidemiological and clinical variables were compared between patients with ONS failure and the rest. ONS failure was defined as the need for device withdrawal or switch off because of lack of response or adverse events. RESULTS: From a seri…

Treatment pattern and response for cluster headache in Korea: A prospective multicenter observation study.

BACKGROUND: Only limited data are available regarding the treatment status and response to cluster headache in an Asian population. Therefore, this study aimed to provide a real-world treatment pattern of cluster headache and the response rate of each treatment in an Asian population. METHODS: Patients with cluster headache were recruited between September 2016 and January 2019 from 16 hospitals in Korea. At the baseline visit, we surveyed the patients about their previous experience of cluster headache treatment, and acute and/or preventive treatments were prescribed at the physician's discr…

Cluster headache associated with oculomotor nerve palsy: A case report and review of literature.

Cluster headaches are a series of short, excruciating, and unilateral pain that happens for a short period at a time over days, weeks, or months and may awake people from sleep. It is situated behind, in, or around one eye only with possible radiation to the forehead at the same side. Cluster headache is characterized by many ocular features. However, extraocular muscle involvement is unusual in this type of headache. We present a case of chronic cluster headache associated with diplopia due to oculomotor nerve palsy in a 24-year-old man, which responded successfully to oral steroid. This pres…

Chronic Bilateral Sphenopalatine Ganglion Stimulation for Intractable Bilateral Chronic Cluster Headache: A Case Report.

UNLABELLED: A cluster headache (CH) is an excruciating pain centered on the periorbital region and is accompanied by autonomic symptoms. Despite the best currently available medical therapy, many patients still suffer from these headaches. Some patients also experience CH with side shift of attacks, which makes treatment more difficult. In light of the pathophysiological mechanism of CH, the sphenopalatine ganglion (SPG) is a promising therapeutic target for its management. SPG ablation and non-ablation techniques have been the preferred treatment for CH in recent decades. However, few article…

Altered hypothalamic functional connectivity in cluster headache: a longitudinal resting-state functional MRI study.

BACKGROUND: Neuroimaging studies implicate hypothalamic dysfunction in the pathogenesis of cluster headache (CH). Disruptions in non-traditional pain processing areas, including the cerebellum and visual cortex, have also been reported in CH. It is unknown whether the hypothalamus interacts significantly with these areas, and whether any such interactions vary between the 'in-bout' and 'out-of-bout' periods in CH. This study aimed to investigate the resting-state functional connectivity (FC) of the hypothalamus of patients with CH. METHODS: Using 3-T functional MRI, we conducted a seed-based …

Occipital nerve stimulation for intractable chronic cluster headache: new hope for a dreadful disease?

Chronic cluster headache (CCH) is one of the most painful primary headaches. A small percentage of CCH become intractable (iCCH) and is refractory to the majority of preventing drugs. Various invasive and sometimes destructive procedures have been tempted to help these patients, but none gave satisfactory results for the long term. Hypothalamic deep-brain stimulation (hDBS) has recently raised expectations with an average improvement of 50 to 70%, but is not a riskless procedure. Harmless methods were therefore warranted, and in this perspective occipital nerve stimulation (ONS) trials were un…

Cluster headache and internal carotid artery dissection: two cases and review of the literature.

We describe 2 patients with cluster headache attacks associated with a dissection of the ipsilateral internal carotid artery at the extra-intracranial passage. These cases highlight the need for extensive neuroradiological investigation in cluster headache patients when atypical features are present. We also performed a PubMed search to review the current literature data about this association.

Hypothalamic deep-brain stimulation: target and potential mechanism for the treatment of cluster headache.

Recently, functional imaging data have underscored the crucial role of the hypothalamus in trigemino-autonomic headaches, a group of severe primary headaches. This prompted the application of hypothalamic deep-brain stimulation (DBS), with the intention to preventing cluster headache (CH) attacks in selected severe therapy-refractory cases. To date, a total of 50 operated intractable CH patients, one patient with short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing and three with atypical facial pain, have been reported. However, it is not apparent why…

Two cases of symptomatic cluster-like headache suggest the importance of sympathetic/parasympathetic balance.

Despite several reports on symptomatic cluster-like headache, there is no clear explanation of how different lesions thought to be causative are related to cluster-like headache. On the basis of two additional cases of symptomatic cluster headache, we discuss the possibility that an acute imbalance of the autonomic nervous system, namely a net overactivity of the parasympathetic system, may be able to trigger these headache attacks in patients who probably have an additional individual predisposition to react with a cluster-like headache. Such an imbalance can be due to an increase in parasymp…

Cluster headache in women: relation with menstruation, use of oral contraceptives, pregnancy, and menopause.

In contrast with migraine, little is known about the relation between cluster headache and menstrual cycle, oral contraceptives, pregnancy, and menopause. A population based questionnaire study was performed among 224 female cluster headache patients, and the possible effect of hormonal influences on cluster headache attacks studied. For control data, a similar but adjusted questionnaire was sent to healthy volunteers and migraine patients. It was found that menstruation, use of oral contraceptives, pregnancy, and menopause had a much smaller influence on cluster headache attacks than in migra…

Case reports: sudden worsening of cluster headache: A signal of aneurysmal thrombosis and enlargement.

We report a 55-year-old man presenting with symptoms of cluster headache, including throbbing pain behind the left eye, tearing, and rhinorrhea. Magnetic resonance imaging and magnetic resonance angiography revealed no abnormalities. Two days of intravenous dihydroergotamine resolved his pain. His headaches were somewhat relieved with a treatment regimen of 100 mg of imipramine each night, 40 mg of propranolol twice a day, 250 mg of divalproex three times a day, and dihydroergotamine nasal spray for breakthrough headaches. Two months later, the severity of his pain increased dramatically. Repe…

Cluster headache: transcranial Doppler assessment of dynamic cerebral circulatory changes during hypocapnia and attack.

Transcranial Doppler ultrasound (TCD) investigations have been carried out in cluster headache patients (8 during remission and 6 during bout) and 14 healthy subjects, to assess cerebral vasomotor reactivity (VMR) to hypocapnia induced by voluntary hyperventilation. VMR was expressed as the relative change in blood flow velocity (V) (%) as a function of the reduction in end-tidal PCO2 (PETCO2) (kPa), i.e. V/P ETCO2. TCD with simultaneous PETCO2 monitoring, was also performed in 5 patients during spontaneous attacks. Prior to hyperventilation, there was bilaterally lower anterior cerebral arter…

Premonitory phase in primary headaches: shared and divergent mechanisms in migraine and cluster headache, the Junior Editorial Board Members' vision.

BACKGROUND AND AIM: Migraine and cluster headache are two highly disabling primary headache disorders, each characterized by distinct clinical phases. Both conditions feature a premonitory phase that precedes the onset of pain or the emergence of cluster bouts, and the symptoms occurring during this stage may provide critical insight into their underlying pathophysiology. This narrative review aims to synthesize current evidence on the premonitory phase of migraine and cluster headache, including its clinical manifestations, neurobiological mechanisms, and potential implications for early or p…
