Medical students' experiences of professional lapses and patient rights abuses in a South African health sciences faculty.

PURPOSE: To elicit South African medical students' experiences of witnessing patient rights abuses and professional lapses during their clinical training in order to inform an appropriate and effective response. METHOD: During June and July 2009 at the University of Cape Town Faculty of Health Sciences, the authors surveyed 223 fourth-, fifth-, and sixth-year medical students in selected clinical rotations concerning abuses they had observed. Volunteers were later interviewed individually. The authors coded interview transcripts for key themes using a constant-comparative grounded theory appr

How declines in fertility rates are informing responses within health systems: a protocol for a scoping review.

INTRODUCTION: Global total fertility rates (TFR) are declining by approximately 1% annually due to factors such as increasing use of contraceptives, urbanisation, a shift in social norms, educational and socioeconomic changes among others. The result is a growing proportion of the world population not meeting their family-building intentions, a shift in global demographic profiles, an ageing population with different social and health system needs and a reduced workforce and taxpayer base to support economic demands. How health systems across the globe are responding to this ‘fertility transit

When Science Outpaces Infrastructure: People, Process, and Technology Challenges for Precision Dosing in Children.

The implementation of novel precision medicine tools in pediatric medicine requires buy-in from multiple interest groups and a shift from traditional medical paradigms. In an effort to implement a precision drug-dosing project for an anti-coagulant drug in the neonatal intensive care unit (NICU) at a large children's hospital, a multi-specialty team made up of clinicians, scientists, private industry, and technology specialists formed and worked collaboratively over multiple years. Despite best efforts, the project's success was hindered by multiple foreseen and unforeseen barriers. Through th

"The most stress comes from witnessing the abuse of children" -challenges faced by research assistants in community-based research in Mtwara, Tanzania.

BACKGROUND: Field workers or research assistants (RAs) are commonly employed within community-based trials. Training provided for RAs, however, is often limited to the technical elements of research, with little-to-no training to help them navigate the ethical challenges they may encounter while working in community-based settings. The main objective of this study was to explore the challenges faced by RAs working as part of a novel housing initiative, to describe the impact these challenges had on their work and wellbeing, and to outline approaches taken by RAs in facing these challenges. Diagnostic accuracy of 18F-FDG PET/CT in muscle-invasive bladder cancer: rationale and design of the MIBC-PET study.

INTRODUCTION: Muscle-invasive bladder cancer (MIBC) is associated with poor prognosis. Current staging relies on CT and MRI, but both show limited sensitivity for nodal and distant metastases. [18F]FDG PET/CT has emerged as a promising tool for staging and response assessment, yet its role remains undefined. METHODS AND ANALYSIS: MIBC-PET is a prospective, monocentric, no-profit study with paired imaging comparison enrolling 60 patients with newly diagnosed, histologically confirmed high-grade MIBC. All patients undergo baseline staging with [18F]FDG PET/CT and contrast-enhanced CT (ceCT) of

Trials and tribulations: a qualitative exploration of researcher perspectives on navigating the challenges of health system implementation research.

OBJECTIVE: While researchers are crucial to the successful delivery of implementation trials, their perspectives are rarely captured in the evaluation of these trials. This qualitative study aimed to capture the experiences of a small research team coordinating a large, multisite implementation trial (the Hide and Seek Project) in the Australian healthcare setting. DESIGN: A qualitative approach was used, whereby members of the Hide and Seek Project research team were invited to take part in semistructured focus groups led by an external facilitator. The focus group guide was developed to exp

TRANSCENDENT (Transforming Research by Assessing Neuroinformatics across the Spectrum of Concussion by Embedding iNterdisciplinary Data-collection to Enable Novel Treatments): protocol for a prospective observational cohort study of concussion patients with embedded comparative effectiveness research within a network of learning health system concussion clinics in Canada.

INTRODUCTION: Concussion affects over 400 000 Canadians annually, with a range of causes and impacts on health-related quality of life. Research to date has disproportionately focused on athletes, military personnel and level I trauma centre patients, and may not be applicable to the broader community. The TRANSCENDENT Concussion Research Program aims to address patient- and clinician-identified research priorities, through the integration of clinical data from patients of all ages and injury mechanisms, patient-reported outcomes and objective biomarkers across factors of intersectionality. Clinicians' Experiences of Providing Compulsory Care for Youth With Anorexia Nervosa: A Qualitative Study.

OBJECTIVE: While compulsory treatment is shown to be potentially traumatic for patients, its impact on clinicians has been insufficiently studied. This study aimed to examine clinicians' experiences with providing compulsory nasogastric tube feeding for youth with severe anorexia nervosa, with particular attention to identifying factors contributing to its negative impacts and potential areas for improvement in clinical practice. METHOD: Twenty-three clinicians in various roles from four academic child and adolescent psychiatry clinics in the Netherlands shared their experiences and opinions

Cost-utility analysis of a multicomponent intervention for fibromyalgia syndrome in primary care versus usual clinical practice: study protocol for an economic evaluation of a randomised control trial.

INTRODUCTION: Fibromyalgia syndrome (FMS) imposes a high cost on society. The significant economic burden from the use of healthcare and, especially, social resources is a spur to revising the usual clinical care (UCC) and to improving treatment strategies. FMS has a deleterious effect on the quality of life (QOL) and productivity, which considerably increase the indirect costs to society. This study reports an economic evaluation comparing the cost and health benefits in a multicomponent intervention programme and UCC of patients with FMS who attend primary healthcare centres of the Gerència

Assessing the benefits on quality of life of a multicomponent intervention for fibromyalgia syndrome in primary care: patients' and health professionals' appraisals: a qualitative study protocol.

INTRODUCTION: Fibromyalgia syndrome (FMS) is a complex condition still scarcely understood and with ambiguity when prescribing treatment. Both patients and healthcare providers can supply valuable information for the development of new treatment strategies. The qualitative narrative analysis of participant's accounts is potentially helpful to reveal new insights about their opinions, needs, and experiences and, consequently, to model healthcare interventions accurately. International treatment guidelines suggest a promising future for multicomponent intervention (MI) approaches for FMS. This s

Validation of newly developed and redesigned key indicator methods for assessment of different working conditions with physical workloads based on mixed-methods design: a study protocol.

INTRODUCTION: The impact of work-related musculoskeletal disorders is considerable. The assessment of work tasks with physical workloads is crucial to estimate the work-related health risks of exposed employees. Three key indicator methods are available for risk assessment regarding manual lifting, holding and carrying of loads; manual pulling and pushing of loads; and manual handling operations. Three further KIMs for risk assessment regarding whole-body forces, awkward body postures and body movement have been developed de novo. In addition, the development of a newly drafted combined method

Invited commentary on Australian fetal alcohol spectrum disorder diagnostic guidelines.

The publication of Australian fetal alcohol spectrum disorder (FASD) diagnostic guidelines marks an important step forward in Australia's efforts to prevent FASD. But do we need yet another set of FASD guidelines? At the 5th International FASD Conference, the ever growing number of FASD diagnostic guidelines was identified as a core area of concern by leaders in FASD worldwide. All agreed we need to strive to adopt a single set of guidelines. It is essential that FASD diagnosis advance to incorporate new knowledge and technology. But to date, the field of FASD has seen multiple sets of guideli

Digital gender violence and family honor coercion: cyberbullying and digital blackmail among Palestinian women.

BACKGROUND: Cyberbullying and digital blackmail (ibtizaz) have emerged as serious threats to psychological well-being, especially for women in conservative and honor-based societies. In Palestine, these digital abuses intersect with patriarchal norms, cultural stigma, and the broader context of political occupation. While global research addresses cyberbullying's psychological impact, there remains limited empirical insight into how these phenomena manifest in occupied Palestinian territories and affect women subject to collective codes of honor, shame, and silence. AIMS: This study aims to f

Epigenetics of Endometrial Cancer: The Role of Chromatin Modifications and Medicolegal Implications.

Endometrial cancer (EC) is the most common gynecological malignancy in developed countries. Risk factors for EC include metabolic alterations (obesity, metabolic syndrome, insulin resistance), hormonal imbalance, age at menopause, reproductive factors, and inherited conditions, such as Lynch syndrome. For the inherited forms, several genes had been implicated in EC occurrence and development, such as POLE, MLH1, TP53, PTEN, PIK3CA, PIK3R1, CTNNB1, ARID1A, PPP2R1A, and FBXW7, all mutated at high frequency in EC patients. However, gene function impairment is not necessarily caused by mutations i

Navigating through turbulence in the nursing talent pipeline: A constructivist grounded theory of academic success in polytechnic nursing education.

BACKGROUND: Academic success for nursing students has traditionally focused on metrics like grade point averages and exam performance but fails to reflect key abilities such as clinical reasoning, ethical decision-making, and the effective transfer of knowledge to practice. Additionally, there is a paucity of literature investigating academic success in undergraduate nursing education within polytechnic institutions, despite their growing role in nursing education. AIM: The objective of this study was to explore the processes and strategies undergraduate nursing students in Canadian polytechn

[Encryption technique for linkable anonymizing].

Linkage of different records such as health insurance claims or medical records for the purpose of cohort studies or cancer registration usually requires matching with personal names and other personally identifiable data. The present study was conducted to examine the possibility of performing such privacy-sensitive procedures in a "linkable anonymizing" manner using encryption. While bidirectional communication entails encryption and deciphering, necessitating both senders and receivers sharing a common secret "key", record linkage entails only encryption and not deciphering because research

Oral antihypertensive agents in hypertensive pregnancies and adverse maternal and perinatal outcomes: study protocol for a target trial emulation using Dutch electronic health record data.

INTRODUCTION: Hypertensive disorders of pregnancy (HDP) are associated with maternal and neonatal morbidity. Although delivery is the definitive treatment, antihypertensive agents are often prescribed to control blood pressure to facilitate safe prolongation of pregnancy. International guidelines vary in their treatment recommendations and preferences for methyldopa, labetalol and nifedipine. Within the field of HDP, randomised trials are often underpowered and observational studies are limited in causal inference due to confounding. Therefore, we propose a target trial emulation with hospital

Decoding the dialogue: The vagus nerve at the interface of brain, body, and Tumor.

Cancer transcends a local disease by engaging in systemic crosstalk with the host's physiology. This review redefines the vagus nerve as a central bioelectric processor within a dynamic brain-body-tumor homeostatic network, orchestrating a complex biological dialogue between them. We synthesize evidence illustrating its specialized architecture for encoding tumor microenvironment signals across metabolic, immune, and endocrine interfaces. Functioning as a dynamic, context-dependent integrator, it maintains systemic homeostasis while paradoxically being susceptible to hijacking by tumors, under
