The prevalence and incidence of convulsive disorders in children.

Each year, about 150,000 children and adolescents in the United States will come to medical attention for evaluation of a newly occurring seizure disorder of some type. Between 2% and 4% of all children in Europe and the United States experience at least one convulsion associated with a febrile illness before the age of 5 years. The cumulative incidence of febrile convulsions among children ranges from about 1% in China to more than 8% in Japan and 14% in Guam. The peak incidence of a first febrile convulsion occurs in the second year of life. Between 0.5% and 1% of children and adolescents ex

[Neonatal convulsions: the current concepts and classification].

The authors presented a new classification of the neonatal convulsions, based on prolonged EEG registrations related to concomitant clinical or videorecording of fits. They focussed on the atypical neonatal convulsions and on the differential diagnosis between epileptic and nonepileptic seizures. Finally, they discussed the therapeutical consequences of these new conceptions about the conclusions in the neonate.

Febrile convulsions: factors and recurrence rate.

One hundred and forty-seven children admitted consecutively with their first febrile convulsion were studied to assess the influence of certain factors on the recurrence rate of seizures. The younger the age at first occurrence the more likely the recurrence rate. Those with moderate degree of pyrexia (39-39.9 degrees C) were found to be ten times more likely to have subsequent recurring convulsions compared to those with high degree of pyrexia (40 degrees C and above) or those with mild degree of pyrexia (38-38.9 degrees C). Furthermore, the recurrence rate was five times higher in first-born

Febrile convulsions in a rural and an urban population.

By means of personal interview of parents in their homes, using a questionnaire, the prevalence rate and some other factors associated with febrile convulsions, were determined in a rural and an urban population of Anambra State of Nigeria. Significantly different rates of 11.61% and 8.05% respectively were obtained for the rural and urban population surveyed. Other factors examined included age of onset and sex distribution, family history, recurrence rates, and severity of seizures. Clinical examination of affected children revealed that 2.9% of the children in the urban population had posti

Pre- and perinatal factors in febrile convulsions.

In a community based study, 110 children with febrile convulsions (FC) were identified prospectively. Pre- and perinatal risk factors were compared with 213 age and sex matched controls sampled from the community. During pregnancy, proteinuria and preeclampsia/eclampsia occurred more often in mothers of cases. Premature birth and bilirubinemia greater than 200 mumol/l were also more common in cases. There were no differences between cases and controls in occurrence of chronic illnesses in mothers, parents age at birth, birth order, and factors occurring during delivery such as type of anesthes

Viral infections and recurrences of febrile convulsions.

To determine whether complicated febrile seizures occur more often in children with a proven viral infection, we performed viral examinations on 144 children with febrile convulsions, of whom 112 had simple and 32 had complicated seizures. A diagnosis of virus infection was verified in 46% of the former patients and 53% of the latter. Three adenoviruses, one parainfluenza virus type 2 and one type 3, one respiratory syncytial virus, one echovirus type 11, one herpes simplex virus type 2, and one influenza B virus were isolated from the cerebrospinal fluid. A simple febrile convulsion occurred

Unsuspected bacterial infections in febrile convulsions.

In a 12-month prospective study in 1984, blood and urinary cultures were obtained as a routine from 307 children who presented with fever and convulsions to the Mater Misericordiae Children's Hospital, Brisbane, and the results were compared with data from 1981-1983 when cultures were not taken as a routine. In the prospective study, bacteraemia was found in 12 (4.3%) of 282 patients but was not suspected clinically in half of these; urinary-tract infection was found in seven (2.6%) of 272 patients and in six of these it was not suspected clinically. All 12 patients with unsuspected bacteraemi

The prognosis of convulsions during childhood shigellosis.

We examined the long-term outcome in 111 children who had convulsions during shigellosis and were followed for 3-18 years after the incident. No deaths or persistent motor deficits occurred as sequellae. Poor coordination of fine hand movements were noted in 3.3% of the 92 children who had no pre-existing neurological abnormality. Only 1 child developed epilepsy by the age of 8 years. Of the children 15.7% had recurrent febrile seizures. The only risk factor identified for febrile seizures following convulsions in shigellosis was a previous history (P less than 0.01). These observations sugges

Convulsions secondary to hyponatremia associated with labyrinthectomy.

A case of convulsions occurring 48 hours following transcanal labyrinthectomy for Meniere's disease in a 76-year-old woman is described to illustrate the unexpected onset of severe hyponatremia after surgery. This syndrome is believed to be produced by inappropriately increased levels of antidiuretic hormone in healthy women without any associated medical condition. To prevent severe central nervous system damage and death, early recognition and appropriate correction of this cation deficiency is necessary.

General convulsions and rhabdomyolysis. Case reports.

Two patients who developed rhabdomyolysis secondary to generalized seizures are presented. Minor traumas of skeletal muscles due to seizures may be associated with myoglobinuria and the development of acute renal failure. The diagnosis rhabdomyolysis may easily be confirmed by following serum creatine kinase. Myoglobinuria is present if the urine turns dark brown with a heavily positive dipstick for urinary hemoglobin while microscopy of urinary sediment contains but a few red blood cells. Darkly pigmented granular urinary casts may also be present. In order to prevent development of acute ren

Clinical spectrum and outcome of neonatal convulsions.

The clinical spectrum and outcome of neonatal convulsions within an obstetric hospital population were reviewed for the 5 years, 1978-82, inclusive. There were 156 convulsing neonates managed at the Mater Mothers Hospital (110 inborn, 46 outborn). The incidence of early neonatal convulsions for inborn babies was 3.0/1000 live births. Antenatal and perinatal risk factors were compared between the 156 convulsing infants and the 36,082 infants born during the same period who did not convulse. The leading risk factors for convulsions were prematurity, intra-uterine growth retardation, low 5 min Ap

Heredity in febrile convulsions: is it HLA-dependent?

One hundred children consecutively treated for simple febrile convulsions (FC) were investigated with respect to hereditary factors. Twenty-five of the children had parents or siblings with FC. HLA-typing was performed in all available members of ten families with one or two FC children and one FC parent, and in one family with two FC children but no FC parent. There was no clear association with a particular HLA haplotype in any of these families.

[Clinical analysis of immediate postoperative convulsion].

For the prevention and treatment of immediate postoperative convulsion which develops within 24 hours after neurosurgical operation, clinical analysis of consecutive 1293 cases in Kyoto University Hospital was done. Overall incidence was 3.1% and the incidence was highest in supratentorial craniotomy (5.7%). Previous history of convulsion was not contributory. In 77.5% of all cases, the convulsion was developed within 6 hours after operation and focal convulsion was more often than the generalized one. Status epilepticus was often developed. The incidence of convulsion was well correlated to t

Seasonal variation of febrile convulsion in Japan.

The 6-year incidence rates of febrile convulsions in all 3-year-old children in Fuchu (covering 95% of children, number examined 17,044) was 8.2%. The incidence was higher in boys than in girls (9.0%: 7.5%, P less than 0.001). The incidence rates varied with the month and year of birth, but the variations observed were slight. Two peak appearances of seasonal variation of the first febrile convulsion were found in November-January and in June-August. The former could be interpreted as a tendency to winter virus infection of the upper respiratory tract in children. The other peak in summer coul
