Are the current treatment strategies for facial nerve schwannoma appropriate also for complex cases?

OBJECTIVES: To describe the decision-making strategies for complex facial nerve schwannomas (FNSs). MATERIALS AND METHODS: Charts belonging to 103 consecutive patients with facial nerve tumors managed between 1990 and 2011 were examined retrospectively to identify complex FNSs. To be classified as complex, at least one of the following criteria had to be met: (1) FNS with large intraparotid tumor component and preoperative good facial nerve function (3 cases); (2) multiple-segment FNSs with extension to both the cerebellopontine angle and the middle cranial fossa in patients with preoperative

Absence and Duplicate Foramen Spinosum in the Same Patient: An Extremely Rare Variation.

The foramen spinosum, one of the important openings at the base of the cranium, is the opening through which the middle meningeal artery enters the cranium. The variations of the foramen spinosum should be well known to be an important landmark in middle fossa surgeries and to understand better the clinical conditions related to the middle meningeal artery passing through it. A total of 35 bones (32 cranial base and 3 separate sphenoid bones) of individuals of unknown age, sex, and ethnicity in the Laboratory of the Department of Anatomy, Gaziantep University, Faculty of Medicine were examined

How I do it: Simpson grade I resection in a medial and inner ridge sphenoid wing meningioma.

BACKGROUND: Extradural anterior clinoidectomy (EAC) and dural peeling of the lateral wall of the cavernous sinus (CS) are challenging skull base techniques that enhance exposure to anterior and middle cranial fossa lesions. Intimate knowledge of dural anatomy enables safe dissection and identification of critical neurovascular structures without cranial nerve deficit postoperatively. METHODS: In a patient with a middle/inner ridge sphenoid wing meningioma, EAC and targeted dural peeling allowed for a Simpson grade I resection. CONCLUSION: EAC and targeted dural peeling enable a feasible Simp

Malunion of the clavicle causes significant glenoid malposition: a quantitative anatomic investigation.

OBJECTIVE: An experimental cadaver model was used to assess the effects of a malunited fracture of the middle third of the clavicle on the functional anatomy of the shoulder joint. METHOD: Anatomic samples were prepared with simulated shortening and axial malposition of the clavicle. From these, alterations in glenoid fossa position were measured and depicted graphically. RESULTS: Healing of clavicle fractures with bony shortening leads to a ventromedialcaudal shift in glenoid fossa position. The following malpositions of the clavicle lead to the respective glenoid fossa positional changes:

Raeder's syndrome. A clinical review.

The term "Raeder's syndrome," which now seems to mean any painful postganglionic Horner's syndrome, has been used in the past to describe patients with a wide variety of underlying pathology, including such serious lesions as middle cranial fossa neoplasms and such benign conditions as unilateral vascular headache syndromes. The purpose of this review which is based on the literature and some recent experience with 41 cases of Raeder's syndrome, is to help clarify this syndrome and to aid the clinician in its evaluation and treatment. Patients with Raeder's syndrome have been divided into thre

Deep Temporal Muscle Pedicled Flap: A Novel Reconstruction Technique Following Transorbital Approach, Anatomic Study, and Surgical Implications.

BACKGROUND AND OBJECTIVES: The temporalis muscle flap has historically been used for repairing defects from orbital procedures and is commonly used in cranial, oral, and facial reconstructions. However, its application for reconstructing the skull base after a transorbital approach has not been explored. Reconstruction after a transorbital intradural approach is still object of preclinical and clinical investigation, with various techniques emerging recently. This study introduces a novel method suitable for extensive resections of the skull base, including intradural resections of the anterio

[Cerebral infarction in a boy with subdural abscess].

A 13-year-old boy was reported with a subdural abscess in the left anterior cranial fossa and an infarction in the lesion supplied by the left middle cerebral artery. Cranial computed tomography revealed a gradual increase of a low density area with surrounding capsule formation, and later another low density area which indicated in occlusion of the left middle cerebral artery. Cerebral angiography disclosed a stenosis at the trifurcated portion of the left internal carotid artery near the abscess. These findings suggested the occurrence of infarction due to angiitis of the internal carotid ar

Temporal bone meningioma involving the middle ear: A case report.

Meningioma is a common intracranial tumor involving the meninges. The localization of this type of tumor is rarely extracranial due to its typically low invasive properties. Furthermore, invasion of the middle ear is exceptional. The present study reported a case of meningioma extending into the middle ear from the middle cranial fossa through the tegmen tympani. The clinical and pathological characteristics, as well as the outcome of the patient, were described.

A novel classification of frontal bone fractures: The prognostic significance of vertical fracture trajectory and skull base extension.

PURPOSE: The broad spectrum of frontal bone fractures, including those with orbital and skull base extension, is poorly understood. We propose a novel classification scheme for frontal bone fractures. METHODS: Maxillofacial CT scans of trauma patients were reviewed over a five year period, and frontal bone fractures were classified: Type 1: Frontal sinus fracture without vertical extension. Type 2: Vertical fracture through the orbit without frontal sinus involvement. Type 3: Vertical fracture through the frontal sinus without orbit involvement. Type 4: Vertical fracture through the frontal s

Supraorbital vs Endo-Orbital Routes to the Lateral Skull Base: A Quantitative and Qualitative Anatomic Study.

BACKGROUND: Various extensions of the supraorbital approach reach the lateral and parasellar middle cranial fossa regions by removing the orbital rim and greater/lesser sphenoid wings. Recent proposals of a purely endoscopic ventral transorbital pathway to these regions heighten the need to compare these surgical windows. OBJECTIVE: To detail the lateral and parasellar middle cranial fossa regions and quantify exposures by 2 surgical windows (transcranial and transorbital) through anatomic study. METHODS: In 5 cadaveric specimens (10 sides), dissections consisted of 3 stages: stage 1 began w

Anatomy of petrous face.

The anatomy of the petrous face of the temporal bone, also known as the petrous pyramid, should be carefully examined to understand the location of, blood supply to, and venous drainage of meningiomas in this area (42%-59% of posterior fossa meningiomas). The petrous face is located in the skull base between the sphenoid and occipital bones. It consists of a base that joins the mastoid laterally and an apex that extends anteromedially to form the foramen lacerum. The anatomy can be divided into three surfaces: anterior/superior, posterior, and inferior. The anterior/superior surface is a conti

Temporomandibular joint cyst presenting as trigeminal neuropathy and middle fossa mass: case report.

OBJECTIVE: We describe the case of a temporomandibular joint cyst eroding into the middle fossa, initially causing compression of the trigeminal nerve and trigeminal neuropathy, and ultimately causing destruction of the second division of the nerve with facial numbness. CLINICAL PRESENTATION: A 50-year-old woman with rheumatoid arthritis on prednisone developed right-sided, initially lancinating facial pain that ultimately became dull in nature in the maxillary division of the trigeminal nerve. This change in pain was associated with the development of numbness in the same distribution. The f

Computed tomography and magnetic resonance imaging findings of intracranial and extracranial meningioma with perineural spread through the foramen rotundum: A case report.

We report a case of a 73-year-old woman presenting with intracranial and extracranial meningioma with perineural spread crossing the foramen rotundum and pterygopalatine fossa, primarily along the maxillary nerve. She visited our hospital with the chief complaint of the left cheek swelling. Subsequent magnetic resonance imaging revealed a well-demarcated mass containing intracranial (cavernous sinus, Meckel's cave, and middle cranial fossa) and extracranial components (pterygopalatine fossa, orbit, nasal cavity, ethmoid and sphenoid sinuses, infratemporal fossa, and buccal space) through the f

New Insights Into Cranial Base Ontogeny in Modern Humans Through Morphometric Analysis.

OBJECTIVES: The cranial base is located at the interface between the face, the brain, and the vertebral column. Understanding its growth is essential for elucidating key aspects of craniofacial development, neurodevelopment, and locomotor function. This study aims to examine the developmental trajectories of the three cranial fossae, anterior, middle, and posterior, in terms of both size and shape. MATERIALS AND METHODS: 203 CT scans of individuals aged from birth to 15 years were segmented and analyzed. Traditional morphometric analyses based on linear measurements were first conducted to as

Future of Endoscopic Ear Surgery.

Endoscopic ear surgery has gained popularity in recent years, becoming standard practice in otology centers around the world as an adjunct to conventional microscopic surgery and as a sole tool for limited disease. During the last years, technical improvements and growing expertise in the handling of the endoscope allowed introducing an exclusive endoscopic approach to the middle ear, lateral skull base, middle cranial fossa, and posterior fossa/cerebellopontine angle pathologies. Endoscopic instrumentation, techniques, and knowledge have improved during the last few years, and in the future,

Giant left middle fossa VII nerve schwannoma associated with amnesia.

The facial nerve is the third most frequent location of intracranial schwannomas, with facial paresis the most common sign.(1,2) A 77-year-old woman presented with amnesia; the Mini-Mental State Examination score was 26, with normal cranial nerve function. A giant multicystic VII nerve schwannoma was identified in the left middle fossa, with components of the tumor in the temporal bone facial canal, geniculate ganglion, and internal auditory canal (figure). Compression of the hippocampus may have accounted for the clinical presentation. The middle fossa component of the tumor was resected for

Facial nerve hemangioma of the geniculate ganglion: an endoscopic surgical approach.

Facial nerve hemangiomas are rare benign tumors arising from the venous plexus surrounding the facial nerve. Surgical management of these tumors is controversial. The goal of surgery is complete tumor removal with restoration of facial nerve function and preservation of hearing, wherever possible. The approaches most used are the translabyrinthine and middle cranial fossa approaches. In this report, we describe the first facial hemangioma treated with an endoscopic transcanal approach, combined with a retroauricular transmastoid minicraniotomy for closure of the dural defect. A great auricular

Surgical approaches to vestibular schwannomas: what the radiologist needs to know.

Vestibular schwannomas account for 85% of cerebellopontine angle tumors in adults and most commonly arise from the inferior division of the vestibular nerve. Surgical and imaging techniques have evolved to offer earlier detection and the potential for hearing preservation. Three main surgical techniques are currently being used for the removal of vestibular schwannomas: middle cranial fossa, suboccipital, and translabyrinthine approaches. Each surgical approach has unique advantages and limitations. For example, the middle cranial fossa and suboccipital approaches make hearing preservation pos
