Synthesis and antitumor activity of cyclophosphamide analogues. 4. Preparation, kinetic studies, and anticancer screening of "phenylketophosphamide" and similar compounds related to the cyclophosphamide metabolite aldophosphamide.

Phenyl ketone phosphorodiamidates [C6H5C(O)CH2CH2OP(O)NHR1NR2R3] were synthesized in conjunction with an ongoing investigation into the effects of substituents on the dynamical solution chemistry of the metabolites of cyclophosphamide (1a). In contrast to aldophosphamide (3a), which readily interconverts with its cyclic isomer 4-hydroxycyclophosphamide (2a), phenylketophosphamide (14a: R1 = H, R2 = R3 = CH2CH2Cl) exhibited an apparent "resistance" toward an intramolecular addition reaction such that 4-hydroxy-4-phenylcyclophosphamide (13a) could not be detected either spectroscopically (31P or…

Busulfan plus cyclophosphamide compared with total-body irradiation plus cyclophosphamide before marrow transplantation for myeloid leukemia: long-term follow-up of 4 randomized studies.

In the early 1990s, 4 randomized studies compared conditioning regimens before transplantation for leukemia with either cyclophosphamide (CY) and total-body irradiation (TBI), or busulfan (Bu) and CY. This study analyzed the long-term outcomes for 316 patients with chronic myeloid leukemia (CML) and 172 patients with acute myeloid leukemia (AML) who participated in these 4 trials, now with a mean follow-up of more than 7 years. Among patients with CML, no statistically significant difference in survival or disease-free survival emerged from testing the 2 regimens. The projected 10-year surviva…

Posttransplant cyclophosphamide in HLA-matched peripheral blood transplantation: what's next?

The use of posttransplant cyclophosphamide was initially pioneered as a means of permitting haploidentical transplantation across HLA barriers. This approach has now become a standard of care for the prevention of significant acute and chronic graft-versus-host disease (GVHD) after related and unrelated HLA-matched allogeneic peripheral blood stem cell transplant across a full spectrum of conditioning intensities. This article discusses recent advances, the mechanisms of action, and important unresolved questions in the prevention of GVHD that will help inform new prospective clinical studies.

Hepatoprotective effect of dipyridamole against cyclophosphamide-induced hepatotoxicity in rats.

BACKGROUND: Cyclophosphamide (Cyc) is an anticancer and immunomodulatory drug, but its use is associated with toxicity, including hepatotoxicity, mediated by oxidative stress (OS), lipid peroxidation, and inflammation. Hence, coadministration of antioxidants probably mitigates Cyc-induced hepatotoxicity. AIM: This study assessed the hepatoprotective effect of dipyridamole (Dipyr) against Cyc-induced hepatotoxicity in rats. METHODS: Thirty-six rats were divided randomly into six equal groups: normal control, Cyc only, Dipyr only (30 mg/kg), Cyc (200 mg/kg) with low-dose Dipyr (15 mg/kg), Cyc …

Quercetin and hesperidin attenuate cyclophosphamide-induced renal injury in rats.

Cyclophosphamide (CP) is an effective chemotherapeutic agent whose clinical use is limited by nephrotoxicity associated with oxidative stress and inflammation. This study investigated the potential nephroprotective effects of quercetin (QUE) and hesperidin (HSP) against CP-induced renal injury in rats. Thirty-six male Wistar albino rats were randomly assigned to six groups (n = 6): Control, QUE, HSP, CP, QUE + CP, and HSP + CP. QUE and HSP were administered orally at a dose of 100 mg/kg/day for 15 days. CP (200 mg/kg) was administered intraperitoneally on the final day to induce nephrotoxicity…

Plant-Derived Spinacetin Mitigates Cyclophosphamide-Induced Hemorrhagic Cystitis in Rats.

The purpose of our study was to assess if spinacetin (SPC), a flavonoid found in spinach, can alleviate the cyclophosphamide (CYP)-induced changes in cystometric and inflammatory parameters indicative of the development of hemorrhagic cystitis. The animal experiments were conducted in female Wistar rats. The cohort of 60 animals was grouped as follows: I-control, II-CYP group, III-SPC group, and IV-CYP + SPC group. The cystometry and biochemical analyses were performed after a fortnight of SPC administration. SPC was found to restore normal cystometric parameters in CYP-induced cystitis and, s…

Molecular mechanisms underlying cyclophosphamide-induced ovarian injury and protective strategies.

Cyclophosphamide (CP) is an anti-cancer medication that also treats chronic inflammatory illnesses caused by the immune system. Although CP is widely used, it can occasionally have limited therapeutic efficacy due to its significant combined toxicities. Ovarian damage caused by CP is a major problem for patients, and premature ovarian failure (POF) is a serious side effect of CP that commonly affects female patients. Mechanistic investigations have implicated oxidative stress, inflammatory responses, and apoptosis as critical components in the etiology of CP-induced POF, although the exact pro…

S100A8 induces cyclophosphamide-induced alopecia via NCF2/NOX2-mediated ferroptosis.

Chemotherapy-induced alopecia (CIA), commonly associated with agents such as cyclophosphamide (CYP), is a prevalent and distressing side effect of numerous chemotherapeutic treatments, significantly impacting patients' quality of life. S100A8, a calcium-binding protein involved in inflammatory responses and oxidative stress regulation, plays a pivotal role in cellular homeostasis. In this study, we investigated the involvement of S100A8 in ferroptosis within a CYP-induced CIA mouse model. We found that CYP treatment upregulated S100A8, NCF2, and NOX2 while reducing GPX4 levels in hair follicle…

Infectious Enterocolitis in Hematopoietic Cell Transplant with Post-Transplant Cyclophosphamide.

Despite the high incidence of diarrhea in hematopoietic cell transplant (HCT) and the frequent involvement of infections, evidence concerning patients receiving post-transplant cyclophosphamide (PTCy) as graft-versus-host disease (GVHD) prophylaxis in the molecular diagnostic era is limited. This study aimed to evaluate the characteristics, incidence, risk factors, and outcomes impact of infectious enterocolitis in patients with hematologic malignancies undergoing HCT from matched sibling, matched unrelated, and haploidentical donors using PTCy as GVHD prophylaxis. Retrospective analysis of in…

Fucoidan treats chemotherapy-induced alopecia and helps cyclophosphamide treat tumors.

Chemotherapy-induced alopecia (CIA) represents one of the most common side effects of cancer treatment. Currently, scalp cooling systems are utilized to treat CIA, but their safety and effectiveness remain limited. The objective of this study was to investigate the effect of fucoidan on CIA and to elucidate the possible mechanism of fucoidan in treating CIA. The results showed that when the dosage of fucoidan was 100 mg/kg·d, it could effectively alleviate CIA induced by cyclophosphamide and promote hair recovery. Altering the dosage affected the therapeutic effect. A lower dosage (50 mg/kg·d)…

Syringic acid improves cyclophosphamide-induced immunosuppression in a mouse model.

Syringic acid (SA), a naturally occurring phenolic substance present in many edible plants and fruits, has been shown to have potential in immunoenhancement applications. In this study, we investigated the immunomodulatory effects of SA in mitigating cyclophosphamide (CYP)-induced immunosuppression in BALB/c mice using doxycycline as a positive control. SA administration prevented immune organ atrophy and morphological changes in the thymus, spleen, and bone marrow induced by CYP treatment in mice while also showing a dose-dependent enhancement of thymus and spleen indices compared to mice tre…

Protective effects of oyster polypeptide on cyclophosphamide-induced immunosuppressed rats.

BACKGROUND: Oyster polypeptide (OP) is a mixture of oligopeptides extracted from oysters through enzyme lysis, separation, and purification. It is associated with immunomodulatory effects, but the underlying mechanisms are not known. This study therefore combined proton nuclear magnetic resonance (1H-NMR) urinary metabolomics and 16S rRNA gene sequencing of the gut microbiome to determine the immunoprotective mechanisms of OP in rats subjected to cyclophosphamide-induced immunosuppression. RESULTS: Oyster polypeptide restored the body weight and the structure of spleen and thymus in rats with…
