Stiff person syndrome in South Asia.

BACKGROUND: Stiff person syndrome is a highly disabling, progressive autoimmune disorder of the central nervous system characterized by musc … nt with intravenous immunoglobulins, immunosuppressive and muscle relaxant medications, regaining independent ambulation. CONCLUSIONS: Diagnosis of stiff person syndrome remains clinical, supported by electromyography and serology for glutamic acid decarboxylase antibodies, facilitated by a high index of clinical suspicion. An autoimmune basis lends stiff person synd …

Sarcopenia Is Highly Prevalent in Older Medical Patients With Mobility Limitation.

BACKGROUND: The association of sarcopenia with disability with ambulatory status is uncertain because most studies have targeted people who could walk independently. This study explored the prevalence of sarcopenia regardless of ambulatory status and the impact of ambulatory status on sarcopenia. MATERIALS AND METHODS: In total, 778 consecutive patients, aged ≥65 years and admitted to a hospital, were enrolled. Ambulatory status was divided into 4 grades according to mobility as described in the Barthel index. Sarcopenia was defined as a loss of appendicular muscle mass index (AMI) with bioel

Antidepressant properties of bioactive fractions from the extract of Crocus sativus L.

The aim of this study was to investigate the antidepressant properties of stigmas and corms of Crocus sativus L. The aqueous ethanol extract of C. sativus corms was fractionated on the basis of polarity. Among the different fractions, the petroleum ether fraction and dichloromethane fraction at doses of 150, 300, and 600 mg/kg showed significant antidepressant-like activities in dose-dependent manners, by means of behavioral models of depression. The immobility time in the forced swimming test and tail suspending test was significantly reduced by the two fractions, without accompanying changes

Repeated intrathecal administration of ropivacaine causes neurotoxicity in rats.

Previous studies suggest that ropivacaine causes the least neurotoxicity among local anaesthetics. Most data derive from a single injection of ropivacaine into the subarachnoid space. The histological changes and behavioural effects of repeated intrathecal administration have yet to be studied. We examined the possible neurotoxicity of multiple doses of intrathecal ropivacaine in rats. Rats received 0.12 ml/kg body weight ropivacaine in normal saline at concentrations of 0.25%, 0.5%, 0.75% and 1.0% at 90-minute intervals via an implanted intrathecal catheter (ID 0.12 mm, OD 0.35 mm) for 48 hou

Reference values and functional descriptions of transverse plane spinal dynamics during gait based on surface topography.

Spinal dynamics during gait have been of interest in research for many decades. Based on respective previous investigations, the pelvis is generally expected to be maximally forward rotated on the side of the reference leg at the beginning of each gait cycle and to reach its maximum counterrotation approximately at the end of the reference leg's stance phase. The pelvic-upper-thoracic-spine coordination converges towards an anti-phase movement pattern in high velocities during ambulation. The vertebral bodies around the seventh thoracic vertebra are considered to be an area of transition durin

Muscle strength and walking ability in diplegic cerebral palsy: implications for assessment and management.

Muscle weakness is a recognised problem in children with Cerebral Palsy (CP). Changes in the understanding of motor control, and progress in the treatment of spasticity, have led to a greater appreciation that spastic muscles are also weak. In recent years weakness has been identified in isolated muscle groups, but studies quantifying the degree and distribution of weakness in multiple muscles remain limited. This study evaluated isometric lower limb muscle strength in 50 ambulant children with CP/Spastic Diplegia (mean age 11 years 7 months) at GMFCS levels I (n=14), II (n=26) and III (n=10).

[Alcoholic cerebellar degeneration with pyramidal sign--in relation to alcoholic myelopathy].

Two cases of alcoholic cerebellar degeneration with pyramidal sign were reported. Patient 1 with alcohol dependence syndrome was a 46-year-old woman. After the alcohol abuse of about eight years, she complained of gait disturbance. The gait disturbance progressively worsened in about two months and she could not ambulate freely by herself. Neurological examination revealed nystagmus, ataxic and spastic gait, slight weakness and spasticity of the lower extremities, hyperreflexia of the extremities, bilateral Babinski's signs, and incoordination of the lower extremities. Examination of liver fun

Drone-delivered automated external defibrillators in prehospital emergency care: implementation evidence, bottlenecks, and conceptual integration with artificial intelligence and next-generation communications.

Out-of-hospital cardiac arrest (OHCA) remains a major global health challenge. Survival depends on immediate cardiopulmonary resuscitation and early defibrillation. Static public-access defibrillator programs often fail to reach private, rural, or otherwise underserved locations. Drone-delivered automated external defibrillators (AEDs) may overcome ground-traffic and geographic barriers, but rapid delivery does not always lead to timely treatment. This Perspective synthesizes simulation, economic, and real-world implementation evidence, and it separates device arrival from successful defibrill

Visual cue training to improve walking and turning after stroke: a study protocol for a multi-centre, single blind randomised pilot trial.

BACKGROUND: Visual information comprises one of the most salient sources of information used to control walking and the dependence on vision to maintain dynamic stability increases following a stroke. We hypothesize, therefore, that rehabilitation efforts incorporating visual cues may be effective in triggering recovery and adaptability of gait following stroke. This feasibility trial aims to estimate probable recruitment rate, effect size, treatment adherence and response to gait training with visual cues in contrast to conventional overground walking practice following stroke. METHODS/DESIG

Management of acute coronary syndromes in the community hospital without cardiac surgical capability: how can access to interventional therapy be improved?

Early coronary artery intervention is emerging as the treatment of choice for patients with high risk acute coronary syndromes (ACS). However, most patients with ACS are admitted to hospitals which do not have ready access to interventional therapy. Extending the benefits of early intervention to this population is problematic at such community hospitals, since this approach would require either emergency transfer to a tertiary center or the performance of angioplasty on-site at hospitals without cardiac surgical capability. A third solution, pre-hospital ambulance triage to interventional cen

Atlanto-occipital Assimilation with Basilar Invagination and Atlanto-occipital Dislocation Treated by Occipitocervical Fixation: A Rare Case Report.

INTRODUCTION: Atlanto-occipital assimilation is the most common osseous anomaly of the cervical-occipital junction. The incidence rate of occipitalization has been reported 1.42.5/1000 children. Most of the patients are asymptomatic initially. Clinical features usually appear after 3rd decade depending on the level and degree of spinal cord compression with surrounding vital structures such as vertebral artery involvement. Hence, early definitive diagnosis and subsequent successful treatment are necessary in such cases to prevent life-threatening complications. CASE REPORT: A 16-year-old male

Effect of Mobile Stroke Unit Dispatch on Process Parameters and Functional Outcomes in Patients With Acute Stroke: The B_PROUD-2.0 Study.

BACKGROUND AND OBJECTIVES: Prehospital stroke management on mobile stroke units (MSUs) shortens time to IV thrombolysis (IVT) and improves functional outcomes. Because IVT effects are time-dependent, optimizing workflows and dispatch-related processes may enhance MSU benefits. The B_PROUD-2.0 study aimed to determine the effect of additional MSU dispatch on functional outcomes in acute stroke patients under optimized MSU organization and service delivery. METHODS: In the nonrandomized, controlled B_PROUD-2.0 study (May 2019-April 2021) conducted in Berlin, Germany, MSUs were simultaneously di

Factors affecting hospital readmission heart failure patients in Japan: a multicenter retrospective cohort study.

The purpose of this study was to examine factors influencing readmission to hospital in patients with heart failure. This multicenter, retrospective cohort study analyzed 197 heart failure patients admitted to the research cooperation facilities between January 2017 and November 2017. We defined the readmission group as being readmitted to hospital in ≤ 6 months of discharge and the non-readmission group as > 6 months after discharge. Cox proportional hazards analysis was performed to explore the predictors of readmission. The incidence of readmission was calculated using Kaplan-Meier curves f

Fitness and mobility training in patients with Intensive Care Unit-acquired muscle weakness (FITonICU): study protocol for a randomised controlled trial.

BACKGROUND: Critical illness myopathy (CIM) and polyneuropathy (CIP) are a common complication of critical illness. Both cause intensive-care-unit-acquired (ICU-acquired) muscle weakness (ICUAW) which increases morbidity and delays rehabilitation and recovery of activities of daily living such as walking ability. Focused physical rehabilitation of people with ICUAW is, therefore, of great importance at both an individual and a societal level. A recent systematic Cochrane review found no randomised controlled trials (RCT), and thus no supporting evidence, for physical rehabilitation interventio

Frailty in Internal Medicine Inpatients: Screening and Multidisciplinary Management.

Frailty is a clinical syndrome characterized by reduced physiological reserve and increased vulnerability to adverse outcomes. It is highly prevalent among hospitalized patients in Internal Medicine, especially older adults, and is associated with increased risk of complications, functional decline, institutionalization, and mortality. We conducted a narrative literature review across PubMed, Scopus, and Web of Science for studies published until 2025. The review included validated screening tools, inpatient interventions, and their impact on clinical outcomes. Limitations inherent to narrativ

Functional Recovery Following Post-vaccination Neurological Syndrome With Demyelinating Findings on Magnetic Resonance Imaging (MRI): A Case Report.

Rare neurological syndromes have been reported following severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination, although a causal relationship remains uncertain. We report the case of a 62-year-old woman who developed progressive neurological impairment temporally associated with BNT162b2 (Pfizer-BioNTech) COVID-19 vaccination, resulting in wheelchair dependence. Brain magnetic resonance imaging (MRI) demonstrated multiple periventricular T2-weighted/fluid-attenuated inversion recovery (T2/FLAIR) hyperintense lesions, including lesions oriented perpendicular to the lateral v
