A 52-year old man presented with recurrent Herpes simplex of the thigh and buttock of 30 years duration. The skin eruption was preceded by pain and sciatica. Surgical excision of the skin area involved modified the site of recurrence. During an attack, the patient developed severe pain and hypoaesthesia in the left half of his chest. The skin lesions were unmodified, and a type 2 Herpes simplex virus was isolated from a vesicle. A clinical examination performed 5 weeks later showed reduced sensitivity to pin prick in the previously painful D5 to D12 territory. Three points are of interest in t

BACKGROUND: We identified substantial heterogeneity in the perioperative management of antithrombotic drugs in skin surgery in Germany in 2012 and 2017 in two cross-sectional surveys. The first national guideline on this subject was published in 2014 and updated in 2021. We sought to identify whether the management of these drugs had changed.
METHODS: We sent a paper-based survey to 1115 dermatologists throughout Germany asking them about their perioperative management of antithrombotic drugs in skin surgery, as well as their familiarity with the guideline.
RESULTS: We received responses fro

BACKGROUND: Pain assessment plays an important role in dermatologic surgery. The numeric rating scale (NRS), visual analog scale (VAS), verbal rating scale (VRS), and Faces Pain Scale (FPS) are commonly used scales for pain measurement. Conflicting evidence exists regarding the use of each. Prompt pain recognition and treatment during procedures result in higher patient satisfaction.
OBJECTIVE: Determine the most applicable scale for acute pain measurement in dermatologic surgery.
MATERIALS AND METHODS: This systematic review was performed in accordance with PRISMA-Supplemental Digital Conte

Pyoderma gangrenosum (PG) classically presents with an acute inflammatory stage, characterized by rapid evolution of painful ulcerations. The pathergy associated with PG lesions complicates disease management. Although PG is commonly treated with immunosuppression, some patients have refractory noninflammatory ulcers. In this subpopulation, there are case reports of successful surgical treatment. However, there is no consensus on optimal perioperative treatment for patients with PG undergoing surgery of any kind, PG related or otherwise. Therefore, we conducted a comprehensive literature revie

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory disease of the skin that is a significant source of impaired quality-of-life for affected individuals. For patients with refractory disease, radical resection of all involved tissue may offer the only potential option for a cure. However, high rates of recurrence after wide local excision have led investigators to seek alternative therapeutic combinations. We present our experience and a literature review.
METHODS: A comprehensive literature review (1982-2016) was performed on PubMed and Ovid MEDLINE, using key words relevant

Precancerous skin lesions and carcinomas in situ of the skin represent the early stages of epithelial skin tumors. There is no invasive tumor growth, so the basement membrane is completely intact. These lesions show a wide variation of clinical and histological appearances on the skin or mucosa. The precancerous and carcinoma in situ lesions that are described in this text are actinic keratosis, actinic cheilitis, cutaneous horns, arsenical keratosis, tar-induced dermatosis, X-ray irradiation-related keratosis, Bowen's disease, erythroplasia of Queyrat, bowenoid papulosis, intraepithelial neop

Excisional biopsies of rounded lesions are performed daily by surgeons. The objective of this article is to establish a surgical technique for direct closure of circular and elliptical defects without the need for additional excision of dog-ears. A four-step technique based on multiple cutaneous and subcutaneous "figure-of-8" sutures is presented. When correctly placed, these sutures can equally distribute the excess tissue along the scar and alleviate dog-ears. The results of the 65 presented cases show that this technique yields short scars and saves healthy skin. A significant reduction of

A patient suffering from skin leishmaniasis is described. Clinical diagnosis was confirmed by histologic findings and isolation of Leishmania tropica. Since antibiotic therapy was ineffective, the patient was exposed to argon laser for the coagulation of nodular elements. Six sessions of laser therapy under local anesthesia with intervals of 4-5 days were carried out, this resulting in complete clinical cure.

Background/Objectives: Keratitis-ichthyosis-deafness (KID) syndrome is an exceptionally rare congenital multisystem disorder, with an estimated prevalence below 1:1,000,000 and fewer than 100 reported cases worldwide. It is characterized by hyperkeratosis, alopecia, nail dystrophy, hearing loss, and ocular involvement. While dermatological management is well described, surgical experience with wound treatment in KID syndrome remains extremely limited. The objective of this report is to describe the surgical management and outcomes of a patient with chronic lower-limb wounds associated with KID

BACKGROUND: When treating a patient with a unilateral cleft lip, the goal is to try and achieve a result that is the least perceivable at conversational distance. The result should be a balanced and symmetric lip with scars that are hidden or are placed in natural anatomical lines. Several methods have been described to repair the lip. The most popular repairs at present are the rotation-advancement or its modifications and the various versions of the straight-line repair.
METHODS: In the author's unit, a method has evolved entitled the symmetric philtral column repair. In this method, the au

The reverse sural flap has often been used for cutaneous coverage of the distal region of the leg and ankle. When the flap is performed in 2 stages, the vascular pedicle is exteriorized and later resected. Our goal was to assess the reverse sural flap performed in 2 stages regarding its viability and low morbidity along the flap-donor area. Eleven patients with cutaneous coverage loss found in the area between the distal third of the leg and ankle underwent cutaneous coverage surgery with a reverse-flow sural flap with an exteriorized pedicle, without violation of the skin between the base of

BACKGROUND: Wound tension reduction is still a challenge to surgeons. Over the years, many techniques have been proposed to avoid this issue. In this paper, we present a new suture technique.
OBJECTIVES: To investigate the tension-reduction effectiveness of the modified subcutaneous buried horizontal mattress suture compared with the vertical buried mattress suture technique.
METHODS: Two suture techniques, the vertical buried mattress suture (group A) and the modified subcutaneous buried horizontal mattress suture (group B), were performed on paired samples of symmetrical skin flaps. An equ

Different techniques can be used to repair paralytic labial commissurae. Ideally, microsurgical procedures or dynamic suspension methods can correct kinetic paralysis but in certain patients (elderly subjects, poor general health, precarious postsurgical rehabilitation) such procedures may represent an excessively aggressive procedure. We therefore used static suspension of the paralytic labial commissurae based on desepidermization of a nasogeneal flap. This simple technique is easy to perform and is our method of choice for static suspension because the flap pulls the paralysed commissurae u

For the augmentation of maxillo-facial soft tissue deficiencies, microsurgical 'pure' fat transplants have proven effective. In this paper, we present the 'de-epithelialized' scapular flap and discuss its significance in comparison with other microvascular fat transfers. Consistency of transplant vessels and minimal dependence of volume on the overall nutritional condition are its specific advantages. The transplant can be securely fixed at the retained corium.
