1,476/2,320 of our cases of atopic dermatitis (AD) start in the first six months of life. The diagnosis is usually easy, but at this age it is sometimes more difficult, mainly because of lack of or rare evidence of scratching, but also because of the brief clinical history that does not allow observation of the characteristic chronic and relapsing course. Knowledge of clinics, but specially of the management of this entity is important, since references to this respect are not up-to-date in spite of the importance of this disease. Newborns adn infants present this condition, due to the fact that vesical and anal sphincters are still beyond control and because the diaper is one of the most important clothing articles in babies and often in preschool or schoolage children complaining of some related pathological condition.

Diaper dermatitis may result from prolonged skin contact with wetness and bacteria. Ammonia plays no apparent role in the generation of diaper dermatitis. Candida albicans frequently contaminates a diaper dermatitis and should be considered present in any diaper dermatitis and should be considered present in any diaper dermatitis known to be present for longer than three days. Topical fluorinated glucocorteroids, boric acid, and mercury-containing preparations should be avoided in the diaper area because of their toxicity.

A cosmetically acceptable lotion containing a silicone protectant, a keratolytic, a bacteriocidal agent and antipruritic substances, was clinically tested in 208 persons with various dermatoses.Twenty-four-hour closed patch tests on humans and intradermal tests in laboratory animals indicate the lotion not to be a sensitizer. Subacute and chronic housewives' eczema and contact dermatitis of the hands, uncomplicated "diaper rash," periaural dermatitis due to excessive moisture, and certain hyperkeratotic dermatoses responded satisfactorily to the use of the lotion.

A 2-week-old infant had grouped pustules on the right hand, wrist, and antecubital flexure that appeared after the first week of life.
