Diaper dermatitis with psoriasiform ID eruption.

Diaper dermatitis with psoriasiform ID eruption has a distinct clinical presentation. Its etiology and relationship to psoriasis remain uncertain. Previous reports have shown histologic features of subacute to chronic dermatitis. Two cases are presented in which a biopsy of secondary lesions showed features characteristic of psoriasis. It is possible that such cases represent those patients with a psoriatic diathesis.

A Strategy to Reduce Critical Cardiorespiratory Alarms due to Intermittent Enteral Feeding of Preterm Neonates in Intensive Care.

BACKGROUND: Many preterm infants require enteral feeding as they cannot coordinate sucking, swallowing, and breathing. In enteral feeding, milk feeds are delivered through a small feeding tube passed via the nose or mouth into the stomach. Intermittent milk feeds may either be administered using a syringe to gently push milk into the infant's stomach (push feed) or milk can be poured into a syringe attached to the tube and allowed to drip in by gravity (gravity feed). This practice of enteral feeding is common in neonatal intensive care units. There is, however, no evidence in the literature t

Efficacy of petrolatum jelly for the prevention of diaper rash: a randomized clinical trial.

PURPOSE: To assess the efficacy of petrolatum jelly for the prevention of diaper rash and to evaluate the relationship between the occurrence of diaper rash and certain treatments and type of nutrition. DESIGN AND METHODS: A randomized clinical trial was carried out with two parallel groups (n = 213) at the neonatal care unit of Donostia University Hospital in San Sebastián, Guipuzcoa Province, Spain. RESULTS: There was a lower incidence of diaper rash in the experimental group with petrolatum jelly (17.1%) than the control group (22.2%), but the difference was not statistically significant

Infantile psoriasis.

We report 14 infants younger than 2 years of age with psoriasis vulgaris in order to call attention to the existence of infantile psoriasis. Three of the patients were noted to have psoriasis in the neonatal period; the onset may have been prenatal in one. In 13 of the 14 patients the site of onset was determined by the isomorphic effect (Köbners phenomenon). Although nail pitting is usually said to be infrequent in childhood psoriasis, 11 (79%) of our patients had typical psoriatic nail changes. Psoriasis must be considered in the differential diagnosis of papulosquamous disorders in infants.

Incontinence-associated dermatitis: Not just babies.

Irritant diaper dermatitis (IDD) is frequently observed in young children but can affect individuals throughout their lifespan. The broader term incontinence-associated dermatitis (IAD) is used to describe inflammatory skin reactions from prolonged exposure to urine and/or feces. This review compares IAD and IDD in pediatric and geriatric populations, focusing on their pathogenesis, clinical considerations, and management. Both conditions involve prolonged bodily fluid exposure disrupting the skin barrier, making infants and older adults particularly susceptible due to their age-related skin c

Effect of skin to skin contact on father infant bonding in a randomized controlled trial.

This study aimed to determine the effect of skin-to-skin contact on father-infant bonding. This randomized controlled experimental study included fathers of 165 newborns delivered at the obstetrics units of Çanakkale Onsekiz Mart University Research Hospital and Çanakkale Mehmet Akif Ersoy State Hospital. The study will be conducted from June 2023 to April 2024. Fathers were randomly allocated to three groups: early skin-to-skin contact (n = 55), frequent skin-to-skin contact (n = 55), and standard care (n = 55). First data were collected at baseline using a questionnaire. At the third month,

Are twitches, startles, and body movements pain indicators in extremely low birth weight infants?

OBJECTIVE: The goal of this study was to examine whether body activity such as postural, trunk, and limb movements may be potential pain cues in preterm infants. DESIGN: Convenience sample. SETTING: Level III neonatal intensive care unit (NICU). PATIENTS: Extremely low birth weight (< or = 1,000 g) preterm infants (n = 64) undergoing routine NICU medical care. OUTCOME MEASURES: Procedures likely to differ in evoking distress (i.e., endotracheal suctioning, chest physical therapy, diaper change, or nasogastric feed) were observed. Behaviors were recorded at bedside using the Neonatal Indivi

Performance of an artificial intelligence model for classifying stool consistency in infants.

Introduction. Classifying stool consistency in infants is essential in pediatric gastroenterology. The Brussels Infant and Toddler Stool Scale (BITSS) is the visual standard, but it exhibits interobserver variability. Artificial intelligence (AI) offers an objective alternative to reduce subjectivity. Objective. To evaluate the agreement between an AI model and the classification performed by pediatric gastroenterologists using BITSS. Population and methods. An observational, cross-sectional, retrospective study based on a photo database of stool samples from infants under 12 months of age in

Different Potent Glucocorticoids, Different Routes of Exposure but the Same Result: Iatrogenic Cushing’s Syndrome and Adrenal Insufficiency.

OBJECTIVE: Potent glucocorticoids (GC) cause iatrogenic Cushing’s syndrome (ICS) due to suppression of hypothalamo-pituitary-adrenal (HPA) axis and may progress to adrenal insufficiency (AI). The aim was to review the clinical and laboratory findings of patients with ICS and to investigate other serious side effects. METHODS: The possibility of AI was investigated by low-dose adrenocorticotrophic hormone test. Hydrocortisone was started in patients with adrenal failure. RESULTS: Fourteen patients (five boys) with ages ranging from 0.19 to 11.89 years were included. The duration of GC exposur

The 2003 national pediatric pressure ulcer and skin breakdown prevalence survey: a multisite study.

OBJECTIVE: The purpose of this study was to document the prevalence of pressure ulcers and other types of skin breakdown in hospitalized children. DESIGN: This descriptive study included documentation of findings from chart reviews and physical assessments of children. SETTING AND SUBJECTS: Nine children's hospitals from throughout the United States participated for a total sample of 1064 children. Subjects were inpatients in the children's hospitals between the ages of neonate to 17 years. INSTRUMENTS: The data collection tools included the interrater reliability quiz, the patient data col

Fluid Balance in the Critically Ill Child Section: "How Bad Is Fluid in Neonates?".

Fluid overload (FO) in neonates is understudied, and its management requires nuanced care and an understanding of the complexity of neonatal fluid dynamics. Recent studies suggest neonates are susceptible to developing FO, and neonatal fluid balance is impacted by multiple factors including functional renal immaturity in the newborn period, physiologic postnatal diuresis and weight loss, and pathologies that require fluid administration. FO also has a deleterious impact on other organ systems, particularly the lung, and appears to impact survival. However, assessing fluid balance in the postna

An atypical form of pediatric pityriasis rosea of the diaper area: Pityriasis marginata et circinata of Vidal.

Pityriasis rosea is a common, self-limited disease which in its typical form should not raise diagnostic doubts. Nevertheless, its atypical presentations can be a challenge for the clinician, and hence it is important to be aware of them. Recognition of these forms may enable a correct diagnosis to be made and avoid unnecessary procedures. We present two patients for whom we believe atypical PR was the most likely the diagnosis.

Dermatophytosis of the diaper area.

Six cases of dermatophytosis of the diaper area due to either Epidermophyton floccosum or Trichophyton rubrum are described herein. The clinical and laboratory features of these patients and those reported in the literature are highlighted. Dermatophytosis should be considered in the differential diagnosis of rashes in the diaper area. Most standard remedies for diaper rash will not be effective or may exacerbate diaper dermatophytosis. Recognition of this condition by the clinician will lead to earlier rational treatment and may avoid the delay caused by therapy.
