Incidentally diagnosed non-dissecting thoracic aortic aneurysm in a dog with suspected aortic valve abnormality.

A thoracic aortic aneurysm (TAA) is a relatively common cardiovascular disorder in humans. However, it is rarely reported in dogs. To date, several reported canine cases are associated with clinical signs and severe complications, such as aortic dissection and rupture. Herein, we report an incidentally diagnosed TAA in an asymptomatic elderly dog with suspected aortic valve abnormality. A 12-year-old spayed female Shiba Inu was referred for the evaluation of anorexia and vomiting. It was subsequently diagnosed with gallbladder mucocele. Thoracic radiography revealed cranial mediastinal widenin

[Branched endovascular aortic repair of a contained rupture in chronic aortic dissection].

A late complication of aortic dissection is the dilatation of the weakened aortic wall. The only urgent endovascular alternative to high-risk open surgery in the treatment of postdissection aneurysms is branched endovascular aortic repair (BEVAR), which is extremely difficult due to the intimal membrane in the aorta lumen and the compressed true lumen. In this case report, we present the first application of BEVAR for aortic dissection in Hungary. Our 76-year-old male patient underwent type B aortic dissection 13 years before, for which supraaortic debranching was followed by thoracic stent gr

Total arch and descending thoracic aortic replacement for massive hemoptysis requiring CPR caused by intrapulmonary penetration of chronic dissecting aortic aneurysm: a case report.

BACKGROUND: Intrapulmonary penetration of the thoracic aorta is a rare, life-threatening complication of a chronic dissecting aortic aneurysm. It causes massive hemoptysis requiring prompt intervention to prevent fatal airway bleeding. A surgical approach that enables diverse surgical maneuvers and intraoperative organ protection is crucial. CASE PRESENTATION: A 62-year-old man, who underwent graft replacement of the ascending aorta for an acute type A aortic dissection 20 months before, developed massive hemoptysis and cardiac arrest. The hemoptysis was secondary to an aortopulmonary fistula

Bilateral Approach for Thoracoscopic Esophagectomy in a Patient with Esophageal Cancer and Solitary Posterior Thoracic Para-aortic Lymph Node Metastasis.

We report a successful dissection of metastatic posterior thoracic para-aortic lymph node (No. 112aoP) via bilateral thoracoscopic surgery. With the anesthetized patient (a 73-year-old Japanese woman) in the prone position, two working ports were inserted for the left-side approach, and artificial pneumothorax was created. Thoracoscopic examination revealed a swollen LN posterior to the descending aorta. Fat and metastatic LNs posterior to the aorta were dissected from the aortic arch level to the diaphragm while preserving intercostal arteries. For the right-side approach, two working ports w

The effect of false lumen procedures during thoracic endovascular aortic repair in patients with chronic DeBakey type IIIB dissections.

OBJECTIVE: Although thoracic endovascular aortic repair (TEVAR) is commonly used for chronic DeBakey type IIIB (CDIIIB) dissections, aortic remodeling outcomes after the procedure have been unsatisfactory. Persistent retrograde flow to the false lumen (FL) through re-entry tears commonly causes treatment failure. The aim of this study was to clarify the safety and effect of the FL procedure (FLP) for aortic remodeling in patients with CDIIIB dissections. METHODS: From 2012 to 2016, there were 73 patients who underwent TEVAR for CDIIIB dissections. The surgery, accompanied by the FLP, was perf

The STABILISE technique to address malperfusion on acute-subacute type B aortic dissections.

Endovascular treatment is the current standard of care for complicated acute and subacute type B aortic dissection. Closure of the primary entry tear with thoracic endovascular aneurysm repair (TEVAR) is often insufficient to induce complete false lumen thrombosis and a positive aortic remodeling. Moreover, TEVAR does not solve all the cases of malperfusion. The Provisional ExTension to Induce COmplete ATtachment (PETTICOAT) technique (deploying self-expandable bare metal stents in the true lumen in addition to TEVAR) can re-expand the true lumen, stabilize the lamella and promote aortic remod

Thoracic Endovascular Aortic Repair (TEVAR) for Grade III Traumatic Thoracic Aortic Injury.

The thoracic endovascular aortic repair (TEVAR) procedure has become a preferred option for managing traumatic lesions of the thoracic aorta due to its ability to intervene in a minimally invasive manner and reduce the risk of complications. This approach allows rapid patient stabilization by avoiding open surgery and facilitates the management of critical conditions, such as pseudoaneurysms of the descending aorta. Aortic contusion injury caused by trauma and deceleration is an important and serious cause of mortality related to trauma, since most of those who suffer it die at the site of the

A novel wire-guided prefenestration technique for patients with type B aortic dissection: A case report.

RATIONALE: Acute type B aortic dissection (ABAD) is a fatal and severe cardiovascular disease. There are various strategies for dissection involving the left subclavian artery, but limited by the variety and cost of stents, the treatment brings certain obstacles. The aim of this study is to evaluate the effectiveness and safety of the wire-guided prefenestration technique for treating left subclavian artery involvement in patients with arterial dissection. PATIENT CONCERNS: A 48-year-old man was transferred to our hospital due to persistent chest and back pain that had lasted for 6 hours. DI

Dissecting aneurysm in a patient with autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is primarily associated with renal failure, but it also causes systemic diseases, including cysts of other systemic organs and cerebral or visceral aneurysm. To make matters worse, life-threatening aortic diseases are associated with ADPKD in some cases. However, only a few reports of ADPKD-associated with thoracic aortic dissection have been published. Herein, we present a case of dissecting aneurysm in a patient with hypertension and ADPKD. He had been followed up for type B aortic dissection for six years. Preoperative creatinine level ra

TEVAR-Only Management of Abdominal Aortic Aneurysm Rupture Caused by Acute Type B Aortic Dissection after EVAR: A Case Report.

Type B aortic dissection after endovascular aneurysm repair (EVAR) is a rare late complication and may cause rupture of a previously stable abdominal aortic aneurysm (AAA). A 79-year-old male developed acute Stanford type B aortic dissection 12 years after EVAR, with distal extension into an infrarenal AAA resulting in rupture and circulatory collapse. A large primary entry tear was located in the descending thoracic aorta. Emergency thoracic endovascular aortic repair (TEVAR) was performed to seal the entry tear, achieving rapid hemodynamic stabilization without abdominal intervention. This c

[A case report of surgical treatment of acute aortic dissection (DeBakey III B) in association with thoracic aortic aneurysm].

This is a case report of 63-year-old man suffering from DeBakey III B acute dissection in association with thoracic aortic aneurysm. He had been following up for hypertension and thoracic aortic aneurysm. He was brought to the hospital by city ambulance complaining of sudden onset of severe back pain. Emergency operation was carried out. It revealed aneurysm of 90 mm in diameter located just distal to the aortic arch and an intimal tear or entry of the dissection located distal to the left subclavian artery. A low porosity Dacron graft was interposed between the distal aortic arch and middle p

aCTA structured reporting of aortic dissection and updated knowledge required by radiologists.

The development of Computed Tomography Angiography (CTA) has greatly enhanced the diagnosis of aortic dissection (AD). However, there is currently no comprehensive reporting requirement for the diagnosis of AD. Structured reporting templates are becoming increasingly common in radiology and other medical fields. Drawing on the standardized writing methods of structured reports for different diseases, the authors propose a structured reporting template for the CTA diagnosis of AD and suggest details for the template while also exploring several hot topics related to AD.

De novo acute type B aortic dissection in two patients with previous infrarenal endovascular aortic aneurysm repair with EndoAnchors.

Acute aortic dissection in the immediate postoperative period after endovascular abdominal aortic aneurysm repair (EVAR) has been linked to technical factors such as excessive endograft oversizing or aortic wall injuries during the procedure. In contrast, dissections that occur later are more likely to be de novo. Regardless of their etiology, aortic dissection can extend into the abdominal aorta, causing collapse and occlusion of the endograft with devastating complications. To the best of our knowledge, no studies have reported on aortic dissection in EVAR patients in whom EndoAnchors (Medtr

[Challenges and strategies in managing distal aneurysms after endovascular repair of aortic dissection].

Aortic dissection is a life-threatening cardiovascular condition, with thoracic endovascular aortic repair (TEVAR) emerging as the preferred treatment for Stanford type B aortic dissection. TEVAR facilitates thrombosis of the false lumen and remodeling of the true lumen by sealing the initial entry tear. While distal re-entry tears are often left untreated, which can result in the formation and progression of distal aneurysms. Current surgical techniques for managing distal aneurysms each possess distinct advantages and limitations, while their complex anatomical characteristics present signif

Mechano-biology in the thoracic aortic aneurysm: a review and case study.

An aortic aneurysm is a permanent and localized dilatation of the aorta resulting from an irreversible loss of structural integrity of the aortic wall. The infrarenal segment of the abdominal aorta is the most common site of aneurysms; however, they are also common in the ascending and descending thoracic aorta. Many cases remain undetected because thoracic aortic aneurysms (TAAs) are usually asymptomatic until complications such as aortic dissection or rupture occurs. Clinical estimates of rupture potential and dissection risk, and thus interventional planning for TAAs, are currently based pr
