Diagnostic Errors in the Acutely Dizzy Patient-Lessons Learned.

Acute vertigo or dizziness is a frequent presentation to the emergency department (ED), making up between 2.1% and 4.4% of all consultations. Given the nature of the ED where the priority is triage, diagnostic delays and misdiagnoses are common, with as many as a third of vertebrobasilar strokes presenting with acute vertigo or dizziness being missed. Here, we review diagnostic errors identified in the evaluation and treatment of the acutely dizzy patient and discuss strategies to overcome them. Lessons learned include focusing on structured history taking, asking about timing and triggers to …

Vertigo and dizziness due to vertebrobasilar TIA: a prospective study.

PURPOSE: Prospective studies on vascular vertigo and dizziness (VVD) due to vertebrobasilar transient ischemic attack (VBTIA) have been sparse. This study aimed to characterize clinical features, response to treatments, and prognostic factors of VVD due to VBTIA using a cohort established in 2021. METHODS: We recruited 103 patients (58 female individuals, 56.3%), with a mean age of 70.9 ± 9.3 years (range = 37-85), between January 2021 and January 2024. All patients met the diagnostic criteria of "Probable transient VVD" published by the Bárány Society. The mean interval from symptom onset to…

Daytime Somnolence and Sleep Apnea Are Associated With Dizziness in the Elderly.

OBJECTIVE: Dizziness is a debilitating multifactorial disorder commonly affecting the elderly. Daytime somnolence and sleep apnea have been linked to dizziness, but previous studies were limited by small sample sizes. The purpose of this study was to examine the relationship between dizziness and daytime somnolence and sleep-disordered breathing, in a nationally representative sample of elderly adults, while adjusting for possible confounders and mediators. STUDY DESIGN: Data from the National Health and Nutrition Examination Study (NHANES; 2017-2020 prepandemic data) were analyzed in a cross…

Post-concussive Dizziness: A Review and Clinical Approach to the Patient.

Dizziness is a frequent complaint after head trauma. Among patients who suffer a concussion (mild traumatic brain injury or mTBI), dizziness is second only to headache in symptom frequency. The differential diagnosis of post-concussive dizziness (PCD) can be divided into non-vestibular, central vestibular and peripheral vestibular causes with growing recognition that patients frequently exhibit both central and peripheral findings on vestibular testing. Symptoms that traditionally have been ascribed to central vestibular dysfunction may be due to peripheral dysfunction. Further, our ability to…

Development and validation of the dizziness symptoms questionnaire in Thai-outpatients.

INTRODUCTION: "Dizziness" is a common complaint in clinical practice that can occur with anyone. However, since the symptom is caused by a wide range of disorders, a general clinician usually faces some difficulty to detect the cause. OBJECTIVE: This study aimed to formulate and validate a simple instrument that can be used to screen and predict the most likely cause of dizziness in Thai outpatients. METHODS: This study was divided into two phases. Phase I included 41 patients diagnosed with common causes of dizziness to determine the algorithm and construct the "structural algorithm questio…

Dizziness Handicap and Its Contributing Factors in Patients With Migraine.

OBJECTIVE: Dizziness is frequently reported in patients with migraine. However, its assessment is often neglected in clinical practice. We investigated the dizziness handicap experienced by patients with migraine and its contributing factors. METHODS: A total of 361 migraine outpatients (270 women, 91 men; mean age 39.01 ± 11.2 years) were given the Korean Dizziness Handicap Inventory (KDHI) questionnaire (total score: 100 points; cutoff: 29). The Headache Impact Test, Generalized Anxiety Disorder-7, Patient Health Questionnaire-9, and Pittsburgh Sleep Quality Index were also administered. Cl…

Nonspecific dizziness as an unusual presentation of neurocysticercosis: A case report.

RATIONALE: Neurocysticercosis (NCC) can cause lesions across the central nervous system, leading to varying clinical manifestations. While the presentation of nonspecific symptom is rare, they are easy to ignore. The present report documents a case of NCC that manifested as persistent dizziness. PATIENT CONCERNS: A Chinese woman visited the hospital on account of dizziness, the severity of which had increased gradually over the month prior. DIAGNOSES: Head computed tomography and magnetic resonance imaging (MRI) revealed hydrocephalus. Cervical MRI revealed an abnormal object in the spinal c…

Acute Diagnosis and Management of Stroke Presenting Dizziness or Vertigo.

Stroke involving the brainstem and cerebellum frequently presents acute vestibular syndrome. Although vascular vertigo is known to usually accompany other neurologic symptoms and signs, isolated vertigo from small infarcts involving the cerebellum or brainstem has been increasingly recognized. Bedside neuro-otologic examination can reliably differentiate acute vestibular syndrome due to stroke from more benign inner ear disease. Sometimes acute isolated audiovestibular loss may herald impending infarction in the territory of the anterior inferior cerebellar artery. Accurate identification of i

Solving the puzzle of strong French coffee, palpitation and dizziness.

A 38-year-old gentleman presented to the emergency department at the University Hospital in Geneva with nausea, rapid palpitation and presyncope. This was followed by a poor night's sleep, a day of strenuous walking in the city and drinking a large quantity of strong French coffee. 12-lead ECG revealed tachycardia with slightly increased QRS duration of right bundle branch block pattern and left axis deviation. Without the electrophysiological (EP) study, the patient was diagnosed with supraventricular tachycardia. On return to Edinburgh, he was investigated under the EP study, which confirmed

Differentiation of Ménière's disease and migraine-associated dizziness: a review.

A significant body of literature exists demonstrating a relationship between migraine disorders and dizziness. In the characterizations of the migraine-associated dizziness, the signs and symptoms show overlap with those characterizing Meniere's disease. There has been literature, beginning with Ménière himself, suggesting a relationship between Ménière's disease and migraine-associated dizziness. This implicates a possible underlying link in pathogenesis. This review article presents a discussion of the overlap in signs and symptoms between the two disorders. Suggestions for differentiating b…
