A workshop in June 2005 ("Preventing Organ and Tissue Allograft-Transmitted Infection: Priorities for Public Health Intervention") identified gaps in organ and tissue safety in the US. Participants developed a series of allograft safety initiatives. "The Organ and Tissue Safety Workshop 2007: Advances and Challenges" assessed progress and identified priorities for future interventions. Awareness of the challenges of allograft-associated disease transmission has increased. The Transplantation Transmission Sentinel Network will enhance communication surrounding allograft-associated disease trans

Almost one third of kidney donation candidates are incompatible (HLA and/or ABO) with their directed recipient. Kidney paired donation allows potential donors to be exchanged and gives access to a compatible kidney transplant. The Bioethics Law of 2011 authorised kidney paired donation in France with reciprocity between 2 incompatible "donor-recipient" pairs. A limited number of transplants have been performed due to a too restricted authorization compared to other European practices. This study presents the perspectives of the new Bioethics Law, enacted in 2021, which increases the authorised

INTRODUCTION: The aim of this study was to determine the prevalence of anatomic variations (renal, vascular and urological) and acquired renal pathologies in living kidney donor candidates (LKDC).
METHODS: This is a retrospective study of all LKDC referred to our center between April 2003 and September 2014. Of the 491 LKDC, 189 were initially excluded for medical reasons (n=140) or others reasons (n=49), without undergoing a radiological assessment. In total, 302 had a radiological assessment (angio-CT or MRI) in anticipation of the donation and 226/302 (73.5%) could donate a kidney.

In this paper, we have reviewed the literature and report on kidney donors that are currently used at relatively low rates. Kidneys from donors with acute kidney injury (AKI) seem to have outcomes equivalent to those from donors without AKI, provided one can rule out significant cortical necrosis. Kidneys from donors with preexisting diabetes or hypertension may have marginally lower aggregate survival but still provide patients with a significant benefit over remaining on the wait list. The Kidney Donor Profile Index derives only an aggregate association with survival with a very modest C sta

INTRODUCTION: Worldwide, organ shortage is a major limiting factor to transplantations. One possible way to face graft scarcity is splitting full livers into hemilivers; this procedure would allow transplantation in 2 adult recipients with the use of a single organ from a deceased donor.
OBJECTIVE: The goal of this study was to describe an adult-to-adult split liver operative protocol and share it between centers interested in exploring this procedure.
MATERIALS AND METHODS: A literature review was first conducted to elaborate on the present protocol; second, selection criteria for suitable

This article discusses the Pittsburgh experience with the pulmonary donor and provides guidelines for the maintenance and selection of appropriate donor lungs. Criteria for the selection of the pulmonary donor include absence of radiographic abnormality, minimal ventilation-perfusion mismatch, and an absence of identifiable infection. Because early thoracic infections result in a high mortality, donors are excluded if white cells, fungi, or bacteria are noted in samples obtained from tracheal aspirates or bronchoscopic examination. A number of techniques for the procurement of donor lungs are

BACKGROUND: Donor-derived infection remains a risk in solid organ transplantation. Clinical practice guidelines inform best practices for donor evaluation and acceptance, although they vary across centers and regions. This study aimed to evaluate the quality and consistency of guidelines for the evaluation of infections in solid organ donors and to summarize their recommendations.
METHODS: We searched electronic databases and websites of professional organizations for clinical practice guidelines and consensus statements on evaluating solid organ donors for the presence and risk of infection.

Blood donor deferral management is a critically important aspect of the safety of the blood supply. Since 2020, several key changes in regulated deferrals have occurred. The most significant change has been the shift to the individual risk assessment model for HIV risk mitigation. The implementation of this had several unique challenges. How blood centers approached the implementation of the individual risk assessment model and other changes of blood donor deferrals since 2020 are discussed.

The number of patients waiting for kidney transplants from deceased organ donors continues to increase. In this context, non-transplantation of acceptable kidneys is especially regrettable. Here, we review successful transplantation of deceased donor kidneys with anatomic abnormalities, intrinsic kidney diseases, and other ostensibly problematic conditions. These scenarios will be encountered infrequently and, with limited time to decide, uncertainty often results in organ refusal. In general, anatomic abnormalities can be overcome, kidney diseases remit in recipients, and systemic donor condi

The evaluation of living kidney donor candidates is a complex and lengthy process. Donor candidates face geographic and socioeconomic barriers to completing donor evaluation. Inequities in access to living donations persist. With a growing demand for kidney transplants and a shortage of living donors, transplant centers are more permissive of accepting less-than-ideal donor candidates. Donors have an increased lifetime risk of kidney failure, but the absolute risk increase is small. Efforts are needed to support donor candidates to complete donor nephrectomy safely and efficiently and receive

The kidney donor risk index (KDRI), standardized as the kidney donor profile index (KDPI), estimates graft failure risk for organ allocation and includes a coefficient for the Black donor race that could create disparities. This study used the Scientific Registry of Transplant Recipients data to recalculate KDRI coefficients with and without the Black race variable for deceased donor kidney transplants from 1995 to 2005 (n = 69 244). The recalculated coefficients were applied to deceased kidney donors from 2015 to 2021 (n = 72 926) to calculate KDPI. Removing the Black race variable had a negl

BACKGROUND: The French health authorities are considering expanding the current selective hepatitis E virus (HEV)-RNA testing procedure to include all donations in order to further reduce transfusion-transmitted HEV infection. Data obtained from blood donors (BDs) tested for HEV-RNA between 2015 and 2021 were used to assess the most efficient nucleic acid testing (NAT) strategy.
MATERIALS AND METHODS: Viral loads (VLs) and the plasma volume of blood components, as well as an HEV-RNA dose of 3.85 log IU as the infectious threshold and an assay with a 95% limit of detection (LOD) at 17 IU/mL, w

The Organ Procurement and Transplantation Network implemented the Collaborative Improvement and Innovation Network (COIIN) to improve the use of donors with kidney donor profile index >50%. COIIN recruited 2 separate cohorts of kidney transplant programs. Cohort A included 19 programs of 44 applicants (January 1, 2017, to September 30, 2017), and cohort B included 39 programs of 47 applicants (October 1, 2017, to June 30, 2018). We investigated the effect of COIIN on kidney yield (number of kidneys transplanted from donors from whom any organ was recovered), offer acceptance, deceased donor tr

BACKGROUND: Point-of-care testing (POCT) devices for determining pre-donation haemoglobin (Hb) concentrations mark the advent of advanced technology for blood banks. POCT devices have undergone several improvements including changes in testing methodology and size of device, befitting the needs of blood donors and blood banks in terms of safety and quality of blood components. This study was planned to evaluate the suitability of non-invasive and invasive POCT devices for blood donor Hb screening.
MATERIAL AND METHODS: Pre-donation Hb in apparently healthy blood donors was measured by a non-i

BACKGROUND: Travel to malaria risk areas such as Mexico is a common source of donor deferral in Canada. On February 21st, 2011 the deferrable regions in Mexico were revised to permit donation if donors travelled to the state of Quintana Roo, Mexico, a popular ocean-side resort area.
MATERIALS AND METHODS: Canadian travel data and malaria deferral rates since 2007 were plotted to examine trends. Deferral records in one centre were accessed from January to April, 2011 to tabulate travel destinations of deferred donors immediately before and after the change.
RESULTS: Travel to Mexico and the C

BACKGROUND: The minimum Hb for blood donation varies from nation to nation. This study assessed the impact of blood donation on donors' iron stores based on different Hb levels. An estimation of drop in the blood collection was made with the new suggested Hb cut-off value.
MATERIALS AND METHODS: 2017 male donors were studied. A questionnaire was filled out to gather demographic data, history of donation and risk factors of iron loss. Their blood samples were analyzed for RBC parameters, serum iron, TIBC, and ferritin level. The iron status of all first-time and regular donors was determined f
