Primary synovial chondromatosis of the elbow.

Very few cases of primary synovial chondromatosis of the elbow have been reported in the literature. This is a study concerning the late outcome of primary synovial chondromatosis in the elbows of 12 patients, 10 men and 2 women, with a mean follow-up of 16 years 10 months. The average age at the time of the initial complaint was 29 years. The histories of 10 patients revealed elbow strain as a consequence of work-related activities. Surgery included removal of loose bodies and partial synovectomy in all patients. A moderate to severe osteoarthritis was found preoperatively in 5 of 12 patients…

Fracture and dislocation of snowboarder's elbow.

BACKGROUND: This study focuses on the analysis of snowboarding versus skiing injuries, especially fracture, dislocation, or both, of the elbow, based on 7 years of medical records and roentgenograms of patients injured at a ski-snowboard area, Mt. Zao National Park, and demonstrates the precise characteristics of snowboard injury in the elbow region. METHODS: A retrospective study of 1,445 injured snowboarders and 10,152 injured skiers was undertaken to assess both snowboarding and skiing injuries. Sixty-four cases of snowboarding injuries and 152 cases of skiing injuries were available for p…

Ulnar nerve strains at the elbow.

Twenty unembalmed intact cadavers had strain measurements taken of the ulnar nerve at 5 positions around the elbow and at varying degrees of elbow flexion. A microstrain gauge was implanted directly into the nerve through a superficial incision. Maximum strains in the ulnar nerve occurred at maximum elbow flexion and directly behind the medial epicondyle; meaningfully less strain was seen at lesser degrees of flexion and positions distal and proximal to the epicondyle. A wide range of maximal strains (0% to 14%) was found. Twenty-five percent of the ulnar nerves showed strains of greater than …

MR Arthrography of the Elbow and Wrist.

This article describes the present status and future directions of magnetic resonance arthrography (MRA) of the elbow and the wrist. The indications are evolving and not yet clearly defined. MRA of the elbow is useful for evaluation of collateral ligaments, the articular surfaces, and suspected loose bodies. MRA of the wrist currently is typically reserved for difficult cases. A description and a comparison of direct and indirect MRA techniques is provided. Direct MRA produces reliable joint enhancement with capsular distension. Indirect MRA does not require a radiologist experienced with arth…

Irreducible lateral dislocation of the elbow.

A rare case of an irreducible post-traumatic lateral dislocation of elbow is presented. The mechanism of injury was fall on a flexed elbow with trauma on its medial aspect resulting in pronation of the forearm. At open reduction, the brachialis muscle was in the form of a tight band which prevented reduction. The ulnar nerve was entrapped in the joint.

MR imaging of the elbow. An update.

MR imaging provides clinically useful information in assessing the elbow joint. Superior depiction of muscles, ligaments, and tendons as well as the ability to visualize nerves, bone marrow, and hyaline cartilage directly are advantages of MR imaging relative to conventional imaging techniques. These features of MR imaging may help to establish the cause of elbow pain by accurately depicting the presence and extent of bone and soft-tissue pathology. Ongoing improvements in surface coil design and newer pulse sequences have resulted in higher quality MR images of the elbow that can be obtained …

Magnetic resonance imaging of the elbow.

Magnetic resonance imaging (MRI) provides useful information regarding the elbow joint. Many abnormalities seen in the elbow are a result of trauma, often from sports such as baseball and tennis. Elbow problems are frequently related to the medial tension-lateral compression phenomenon where repeated valgus stress produces flexor-pronator strain, ulnar collateral ligament sprain, ulnar traction spurring, and ulnar neuropathy. The lateral compression causes osteochondritis dissecans of the capitellum and radial head, degenerative arthritis, and loose bodies. Other elbow abnormalities seen on MR…

Elbow injuries to the throwing athlete.

Elbow injuries in the throwing athlete are common. Because of the tremendous medial tensile forces and lateral compressive forces borne by the elbow, there is a wide array of injuries that occur. A detailed history and physical examination are necessary for an accurate diagnosis. Many of the injuries discussed can be treated successfully with conservative measures followed by aggressive physical therapy. Prompt recognition and early treatment is mandatory. In those conditions in which conservative treatment tends to fail, the detailed surgical options have been discussed. It is important to se…

MR imaging of the adult elbow.

Magnetic resonance (MR) imaging can provide important diagnostic information in the evaluation of the adult elbow. Optimal imaging technique should include the use of proper positioning, surface coils, and appropriate sequences and imaging planes as indicated by the suspected abnormalities. A familiarity with the anatomy of the normal elbow is crucial to identifying pathologic entities correctly and avoiding diagnostic pitfalls. Disruption of the collateral ligaments can best be demonstrated with thin-section coronal gradient-echo MR images. Injuries to the flexor and extensor muscle groups, b…

Arthroscopy for limitation of motion of the elbow.

Twenty-five patients with limitation of motion of the elbow joint caused by the intraarticular pathologies were treated with the following arthroscopic procedures: removal of loose bodies, excision of osteophytes, anterior capsular release, abrasion arthroplasty, and partial excision of the radial head. The extension of the elbow improved by 7 degrees, from a preoperative average of 21 degrees to a postoperative average of 14 degrees. The flexion of the elbow improved by 17 degrees, from a preoperative average of 113 degrees to a postoperative average of 130 degrees. The total range of motion …

Elbow injuries in athletes. A review.

Elbow injuries are becoming more common as increasing numbers of people participate in throwing and racquet sports. The understanding and treatment of elbow injuries is becoming more sophisticated in conjunction with better noninvasive and invasive diagnostic techniques. The majority of injuries to the elbow in the athlete are chronic, overuse injuries. These injuries are the result of repetitive intrinsic or extrinsic overload, or both, resulting in microrupture of soft tissue such as ligament or tendon. In children, apophyses, being the weakest link in the immature musculoskeletal system, ar…

Physical examination of the thrower's elbow.

The physical examination of the thrower's elbow presents the clinician with the clinical challenge of differentially diagnosing specific pathologies. The examination should include a thorough history and a well-organized physical examination, which relies on an extensive knowledge of the functional anatomy of the elbow. The components of an elbow examination include inspection/observation, palpation of bony and soft tissues, range of motion assessment, resisted muscle testing (both manual and mechanical), neurologic testing, and special tests. The special tests commonly performed on the throwe…

Compression plate arthrodesis of the elbow.

We reviewed 15 patients with an arthrodesis of the elbow using an AO compression-plate technique, after an average follow-up of 24 months. The most common indication was an open, infected high-energy injury with associated bone loss. Arthrodesis was successful in all but one patient in whom severe deep infection necessitated amputation. Eight patients were treated with the metal partly exposed in an infected wound. After removal of the metal, all wounds healed secondarily and none had clinical or radiographic signs of sepsis at latest review. Compression-plate arthrodesis of the elbow is a gen…

[Osteochondrosis dissecans of the elbow joint].

Thirty-one elbow joints in 29 patients (28 males, only 1 female), operated on for osteochondritis dissecans, were available for follow-up after an average of 10 years. The mean age at diagnosis was 24.4 years, but 11 patients were younger than 20. In 23 out of 29 patients the right elbow was involved, including 2 cases with bilateral involvement. In 20 cases removing the free bodies was the only surgical procedure performed, but in 2 cases it was combined with resection of the radial head. At follow-up, only one-third of the patients had no complaints. Nevertheless, daily activity was not rest…

Posterolateral rotatory instability of the elbow.

Recurrent posterolateral rotatory instability of the elbow is an apparently undescribed clinical condition that is difficult to diagnose. We treated five patients, ranging in age from five to forty years, who had such a lesion and in whom the instability could be demonstrated only by what we call the posterolateral rotatory-instability test. This test involves supination of the forearm and application of a valgus moment and an axial compression force to the elbow while it is flexed from full extension. The elbow is reduced in full extension and must be subluxated as it is flexed in order to ob…
