Analysis of Fracture Characteristic and Medial Collateral Ligament Injury Relationships in Terrible Triad Elbow Injuries.

PURPOSE: The objectives of this study were to identify the relationship between fracture characteristics and medial collateral ligament (MCL) injuries in terrible triad injuries of the elbow. METHODS: Between 2010 and 2018, 60 patients who underwent surgery for terrible triad elbow injuries were retrospectively reviewed. Using magnetic resonance imaging (MRI) assessments, patients were stratified into those who had intact or low-grade partial MCL tears (low-grade MRI-MCL group) and those who had high-grade partial or full-thickness MCL tears (high-grade MRI-MCL group). We also analyzed patien

Survival and outcome of total elbow arthroplasty for distal humeral fracture at long term follow-up.

Background Total elbow arthroplasty is an accepted procedure for the treatment of acute comminuted distal humeral fractures in elderly. Few long-term outcomes are available. The purpose of this study was to examine long-term clinical and radiological outcomes of prosthesis performed (January 2002-June 2015) for complex intra- fractures of the distal humerus (AO/C) in patients older than 65 years with low functional demands. Materials and Methods Further inclusion criteria were: the availability of a clinical and radiological follow-up of minimum 5 years and pattern of closed fracture. Demograp

[Effects on the elbow of radial head resection following isolated radial head fracture in young patients].

BACKGROUND: Radial head resection has been a widely used procedure in cases of non-reconstructable radial head fracture. Some authors propose the use of radial head arthroplasty. The purpose of this study was to evaluate the long-term outcome of radial head resection for isolated fracture, without instability at the time of the fracture, and its radiological and functional repercussions at the elbow level. METHODS: We reviewed 44 clinical records with acute resection of the radial head following isolated radial head fracture, with absence of associated injuries, younger than forty, with a min

Elbow dislocation with ipsilateral diaphyseal forearm bone fracture: A rare injury report with literature review.

Dislocation of the elbow along with shaft fractures of both bones of the ipsilateral forearm is a rare injury though elbow dislocation or fracture of the forearm bones may occur separately. Such injuries need a concentric reduction of the dislocation and an anatomical fixation of forearm bones for optimal functional outcomes. We report a case of elbow dislocation with fracture of the lateral condyle of the humerus along with fractures of shafts of the radius and ulna in a 44-year-old female. Closed reduction of the elbow and operative stabilization of all fractures were done with good clinical

A new fracture model for "terrible triad" injuries of the elbow: influence of forearm rotation on injury patterns.

OBJECTIVE: The purpose of this study was to evaluate the influence of forearm rotation on failure patterns of the elbow under axial loads. METHODS: Fourteen upper extremities were resected mid-humerus and mounted on a custom apparatus, which allowed rotation of the ulna, radius, and humerus about a fixed wrist while loading in axial compression. Seven specimens were loaded to failure with the forearm in pronation and 7 in supination. RESULTS: Six of the 7 elbows axially loaded in pronation resulted in fractures of the radial head and coronoid with posterior dislocation (terrible triad). Six

The use of a "reverse" axis (axillary-interscalene) block in a patient presenting with fractures of the left shoulder and elbow.

A patient presented for surgery to repair a fractured left shoulder and elbow and requested regional anesthesia. Most upper extremity operations require a single brachial plexus nerve block. The position of the two fractures however required the use of two separate approaches, an interscalene and an axillary approach.

[Posterior approach to the elbow preserving the extensor apparatus. Its value in the osteosynthesis of humerus plate fractures].

The authors describe a median posterior approach of the elbow with preservation of the triceps mechanism in continuity. Such an approach provides an excellent exposure of the elbow and of the lower part of the humeral shaft which can be used for the osteosynthesis of complex supra and intercondylar fractures. This approach can also be extended to the midshaft of the humerus, the radial head and the ulnar shaft. The repair is easy and strong enough to allow a rapid rehabilitation.

Factors Influencing Time to Return of Elbow Motion in Completely Displaced and Flexion Type Pediatric Supracondylar Humeral Fractures.

BACKGROUND: Factors influencing the time to return of elbow motion have not been reported in extension-type III/IV and flexion-type supracondylar humeral (SCH) fractures. This study aims to identify factors influencing the time to return of elbow motion following operative pinning of these injuries. METHODS: An IRB-approved retrospective study was conducted of patients <18 years old treated at a large tertiary children's hospital from January 1, 2013 to July 31, 2022 for operatively pinned, completely displaced, and flexion-type SCH fractures. We excluded other SCH fracture types, patients un

Exploring the incidence of infection in children undergoing open reduction for supracondylar elbow fractures without perioperative prophylactic antibiotics.

BACKGROUND: Elbow fractures are among the most common types of fractures in children; an incision is inevitable when open surgery is performed. Prophylactic antibiotics was widely used in paediatric elbow fractures. This study is to investigate factors associated with postoperative infection in children undergoing open reduction and internal fixation with percutaneous pin fixation of supracondylar humeral fractures (SHFs). All patients did not receive prophylactic antibiotics during perioperative period. METHODS: The clinical data of children with SHF who underwent open reduction and percutan…

Financial incentives and work relative value unit comparison of fixation versus total elbow arthroplasty for distal humerus fractures.

BACKGROUND: Distal humerus fractures (DHFs) are challenging orthopedic injuries requiring tailored management. Surgical options primarily include open reduction and internal fixation (ORIF) and total elbow arthroplasty (TEA) for non-reconstructable fractures. While clinical considerations typically guide treatment, recent studies have shown better surgical incentives for arthroplasty compared to ORIF. This study aims to investigate compensation differences across treatment options for DHFs and to evaluate the potential association of financial incentives with clinical decisions in orthopedic s

Residual articular incongruity is not a predictor of poor functional outcomes in operatively treated intra-articular elbow fractures.

BACKGROUND: Intra-articular and comminuted elbow fractures are complex injuries that present a tremendous surgical challenge. The goal of surgical fracture reduction is an anatomical restoration of the articular surface to restore physiological biomechanics of the elbow joint and to regain an unrestricted elbow function. However, these fractures are often accompanied by residual articular step-off, gap displacement, and intra-articular loose bodies. To date, the potential correlation between the quality of fracture reduction and the postoperative functional outcomes has not been investigated f…

Total elbow arthroplasty following complex fractures of the distal humerus: results in patients over 65 years of age.

BACKGROUND: The treatment of complex distal humerus fractures is challenging and is often associated with unsatisfactory results. Anatomic reduction and stable internal fixation is difficult to obtain, especially in elderly osteoporotic patients. For these reasons, total elbow arthroplasty (TEA) has recently evolved as a valid procedure in selected cases following these injuries. The aim of this study was to analyze outcomes of TEA for the treatment of complex distal humerus fractures in 20 low-demanding patients over 65 years of age. MATERIALS AND METHODS: All patients, at a mean follow-up o

Conversion to below-elbow cast after 3 weeks is safe for diaphyseal both-bone forearm fractures in children.

BACKGROUND: It is unclear whether it is safe to convert above-elbow cast (AEC) to below-elbow cast (BEC) in a child who has sustained a displaced diaphyseal both-bone forearm fracture that is stable after reduction. In this multicenter study, we wanted to answer the question: does early conversion to BEC cause similar forearm rotation to that after treatment with AEC alone? CHILDREN AND METHODS: Children were randomly allocated to 6 weeks of AEC, or 3 weeks of AEC followed by 3 weeks of BEC. The primary outcome was limitation of pronation/supination after 6 months. The secondary outcomes were

A Pediatric Floating Elbow Associating Flexion Type Supracondylar Fracture with Both Bone Forearm Fracture And Ulnar Nerve Irritation: A Case Report.

INTRODUCTION: Pediatric floating elbow in the setting of flexion type-supracondylar fracture is exceptional. Most reported cases in the literature include the more common variant of supracondylar fracture which is the extension type. We report a rare case of pediatric floating elbow associating flexion type supracondylar fracture with both bone forearm fracture and ulnar nerve irritation. CASE REPORT: A 5-year-old boy presenting to the emergency department after sustaining a fall from 3 m height. At presentation, he had an S-shaped deformity of the left upper limb, along with ecchymosis of th
