Acid-Base and Electrolyte Disorders: Potassium Disorders.

A normal serum potassium level of 3.5 to 5.0 mEq/L is maintained via potassium ingestion, excretion, and distribution between intra- and extracellular fluid. Potassium balance is essential for maintenance of normal resting cell membrane potential in excitatory tissues. Abnormalities in serum potassium, whether low or high, can cause life-threatening complications due to cardiac, respiratory, or neurologic compromise. Hypokalemia results from renal or gastrointestinal losses, or transcellular shifts. In the absence of an identified cause, evaluation of urinary potassium excretion and acid-base

Acid-Base and Electrolyte Disorders: Calcium Disorders.

Calcium is an essential element for normal physiologic function. Normal serum calcium is maintained within a close range of 8.4 to 10.4 mg/dL, and acute or severe changes can result in serious cardiac, musculoskeletal, and neurologic consequences. Normal calcium levels are maintained by interactions of parathyroid hormone and vitamin D. Parathyroid hormone regulates calcium levels via bone resorption, renal calcium reabsorption, and enhanced calcium absorption in the small intestine. Causes of hypocalcemia include hypoparathyroidism (often postsurgical), severe chronic kidney disease, and vita

Fluid and Electrolyte Therapy in the Patient with Kidney Injury.

In small animals with kidney injuries, fluid therapy is the cornerstone of supportive care. Although fluids do not repair kidney damage, appropriate therapy can stabilize the patient, correct fluid, electrolyte, and acid-base imbalances, and provide vital support. The primary goals are to rescue the animal, optimize systemic perfusion and renal hemodynamics, and restore hydration without causing fluid overload. Isotonic crystalloids are the fluids typically administered first, but fluid therapy of the animal with kidney injury commonly must be individually adjusted to meet unpredictable and ch

Nutmeg Poisoning With Electrolyte Abnormalities: A Case Report.

Nutmeg is a commonly used spice that contains myristicin, a compound with anticholinergic and psychoactive properties. Excessive consumption can lead to toxicity, causing psychiatric and anticholinergic symptoms such as tachycardia, dizziness, dry mouth, tremors, and nausea. Nutmeg poisoning is rare and not widely recognized. This report describes the case of a 33-year-old woman who accidentally ingested a toxic amount of nutmeg while cooking. She presented with symptoms of dry mouth, tremors, dizziness, and anxiety and was subsequently diagnosed with nutmeg poisoning. Blood tests revealed ele

The Preparation of Electrolyte Hydrogels with the Water Solubilization of Polybenzoxazine.

Polybenzoxazine (PBZ) exhibits excellent heat resistance, and PBZ derivatives have been designed and synthesized to achieve high performance. However, the application range of PBZ is limited by the strong interactions between molecular chains and its low solubility in organic solvents, thereby limiting its processability. This study focused on the benzoxazine structure as the molecular backbone of new hydrogel materials that can be applied as electrolyte materials and prepared functional gel materials. Here, we prepared hydrogels by water-solubilizing PBZ derivatives, which typically exhibit l

Electrolyte Disorders in Mitochondrial Cytopathies: A Systematic Review.

SIGNIFICANCE STATEMENT: Several recent studies identified mitochondrial mutations in patients with Gitelman or Fanconi syndrome. Mitochondrial cytopathies are generally not considered in the diagnostic workup of patients with electrolyte disorders. In this systematic review, we investigated the presence of electrolyte disorders in patients with mitochondrial cytopathies to determine the relevance of mitochondrial mutation screening in this population. Our analysis demonstrates that electrolyte disorders are commonly reported in mitochondrial cytopathies, often as presenting symptoms. Consequen

Colonic Fluid and Electrolyte Transport 2022: An Update.

Colonic epithelial cells are responsible for maintaining a delicate balance between luminal secretion and the absorption of fluids and ions. This review aims to discuss and update the model of colonic electrolyte secretion and absorption via the cystic fibrosis transmembrane regulator (CFTR), epithelial sodium channel (ENaC), Na-K-Cl cotransporters (NKCC1 and 2), Na-H exchangers (NHE1-4), colonic H,KATPase, and several other key components involved in multi-level transepithelial ion transport. Developments in our understanding of the activity, regulation, localization, and relationships of the

Reproductive Consequences of Electrolyte Disturbances in Domestic Animals.

Electrolyte balance is essential to maintain homeostasis in the body. The most crucial electrolytes are sodium (Na+), potassium (K+), magnesium (Mg2+), chloride (Cl-), and calcium (Ca2+). These ions maintain the volume of body fluids, and blood pressure, participate in muscle contractions, and nerve conduction, and are important in enzymatic reactions. The balance is mainly ensured by the kidneys, which are an important organ that regulates the volume and composition of urine, together with which excess electrolytes are excreted. They are also important in the reproductive system, where they p…

Fluid and Electrolyte Management in the Neonate: Potassium and Phosphorus.

Maintenance of electrolyte balance in the neonate is challenging in the context of illness or prematurity. Imbalances in potassium and phosphorus can occur in neonates, ranging from benign and clinically insignificant to those that can be life-threatening. An individualized approach to fluid and electrolyte management is based on the neonate's gestational age, day of life, maintenance needs, deficits, and ongoing losses. This article reviews normal and abnormal potassium and phosphorus values along with management strategies. Using a case scenario, the reader can apply concepts outlined in thi

Trimethoprim-associated electrolyte and acid-base abnormalities.

INTRODUCTION: The antimicrobial trimethoprim is structurally related to potassium-sparing diuretics and may consequently lead to derangements in electrolyte and acid-base balance. Since no report so far analyzed the literature documenting individual cases with electrolyte and acid-base derangements induced by trimethoprim, a systematic review was carried out. EVIDENCE ACQUISITION: We retained 53 reports documenting 68 cases (42 males and 26 females 23 to 96 years of age) of electrolyte or acid-base derangements occurring on trimethoprim for about 5 days. EVIDENCE SYNTHESIS: One hundred five

Pharmacologic Approaches to Electrolyte Abnormalities in Heart Failure.

Electrolyte abnormalities are common in heart failure and can arise from a variety of etiologies. Neurohormonal activation from ventricular dysfunction, renal dysfunction, and heart failure medications can perturb electrolyte homeostasis which impact both heart failure-related morbidity and mortality. These include disturbances in serum sodium, chloride, acid-base, and potassium homeostasis. Pharmacological treatments differ for each electrolyte abnormality and vary from older, established treatments like the vaptans or acetazolamide, to experimental or theoretical treatments like hypertonic s

[Electrolyte disturbances in geriatric patients with focus on hyponatremia].

Disturbances of water and electrolyte balance are commonly encountered in older patients due to a multitude of physiological changes and preexisting morbidities with hyponatremia being the most common disorder. Even mild chronic hyponatremia can lead to cognitive deficits and gait instability and is associated with an increased rate of falls and fractures. Additionally, experimental and epidemiological data suggest that hyponatremia promotes bone resorption and therefore increases the risk of osteoporosis. Furthermore, osteoporosis and sarcopenia can be stimulated by hypomagnesemia. Hypernatre
