Limits of normal for pressure sensitivity in the fistula test.

In patients with perilymphatic fistula (PLF), nystagmus may sometimes be elicited by application of pressure to the external ear canal. The extent to which the normal population also exhibits such 'pressure sensitivity' is presently unknown. Our goal was to determine the limits of normal pressure sensitivity and to quantify the performance of the fistula test. Our subjects consisted of 13 normal controls and 7 patients with a history of pressure sensitivity who later underwent exploratory tympanotomy. We measured nystagmus prior to and following pressurization of the external ear canal. Pressu

Ventilatory effects of specific carotid body hypocapnia in dogs during wakefulness and sleep.

We used extracorporeal perfusion of the vascularly isolated carotid sinus region to determine the effects of specific carotid body chemoreceptor hypocapnia-alkalosis on ventilatory control in the unanesthetized dog. Eight female dogs were studied during wakefulness, non-rapid-eye-movement (NREM) sleep, and rapid eye movement (REM) sleep. Carotid body perfusions lasted from 1 to 2 min, and each trial was preceded by a 1-min control period. Two levels of carotid body hypocapnia were employed, approximately 7 and 14 Torr below eupneic levels in a given dog. We found that 1) During wakefulness and

Sleep and cranial dystonia.

A nocturnal polygraphic study was performed on 10 patients with cranial dystonia (blepharospasm (BS) and oromandibular dystonia (OMD)). All patients showed impaired sleep efficiency and reduced slow and REM sleep, more marked in subjects with severe dystonia. Abnormal muscular activity decreased progressively with deeper sleep and during the first hours of the night, without disappearing. A disordered hypnic++ pattern and impaired motor control even during sleep are typical features in cranial dystonia.

Termination of central sleep apnea episodes by upper airway stimulation using intermittent positive pressure ventilation.

A 52-year-old man presented with pure central sleep apnea syndrome that failed to respond to nasal continuous positive airway pressure (NCPAP) therapy. Intermittent positive pressure stimulation via a nasal mask was instituted using a portable positive pressure ventilator. We found marked reduction in the respiratory disturbance index, number of desaturations, and the number of arousals with this mode of therapy.

The effects of gamma-hydroxybutyrate on the sleep of narcolepsy patients: a double-blind study.

The effects of gamma-hydroxybutyrate (GHB: 25 mg/kg h.s. and 3 h later) vs. placebo on objectively evaluated nighttime sleep and daytime sleepiness in narcolepsy were evaluated in a double-blind, counterbalanced crossover design. Twenty narcolepsy patients were given an overnight polysomnogram (PSG), followed by a daytime multiple sleep latency test (MSLT) at baseline and on the 1st and 29th days of GHB and placebo treatment. The overnight PSGs indicated that the narcolepsy patients had the following significant results during GHB versus placebo treatment: decreased stage 1 (p = 0.012), increa

Incomplete congenital stationary night blindness: electroretinogram c-wave and electrooculogram light rise.

Three cases of congenital stationary night blindness are reported. In all patients a negative electroretinogram was recorded by single bright flash stimulation and changed to positive on sequential reduction of the stimulus intensities. Oscillatory potentials were recognized. Biphasic dark adaptation curves and moderate elevation of the relative logarithmic final threshold of dark adaptation were also found. Values of critical flicker fusion frequency were reduced to a mild or moderate extent. VEP latency was prolonged beyond the normal range in two cases. In all three the electroretinogram c-

Sustained blepharoclonus upon eye closure.

A unique case of sustained, rhythmical contractions of both orbicularis oculi induced by voluntary eye closure is described. It was observed in a 25-year-old man who had suffered a severe head trauma. The lid phenomenon was synchronous with macro square-wave jerks. Sustained blepharoclonus upon eye closure is likely to be related to dysfunction in the cerebellar system.

Variations in gravitoinertial force level affect the gain of the vestibulo-ocular reflex: implications for the etiology of space motion sickness.

Recordings of horizontal nystagmus were obtained on 16 male subjects exposed to repeated patterns of horizontal angular acceleration, constant velocity rotation, and sudden-stop deceleration in the laboratory and in the free-fall and high-force periods of parabolic flight. Nystagmus intensity was a clear function of gravitoinertial force level: slow phase velocity and beat frequency increased during exposure to high force levels and decreased in free-fall compared to values obtained at I G. These findings indicate that the gain of the vestibulo-ocular reflex decreases in free-fall. This fact l

Influence of diffuse brain stimulation (DBS) on human sleep. II. Sleep-induced periodic breathing with apnoea.

The influence of diffuse brain stimulation (DBS), administered just before sleep onset, on the number of apnoeas and of sleep stage changes during the first part of the succeeding night sleep to the end of the first sleep cycle has been studied in 7 patients with sleep-induced periodic breathing with apnorea. When compared with the corresponding period of the baseline night, both phenomena are increased significantly. These results indicate that DBS influences slow periodic sleep phenomena. It is suggested that the target area for this influence might be mesencephalo-pontine reticular structur

[Clinical comparison of perrotatory and postrotatory nystagmus characteristics (author's transl)].

Rotatory stimulation is a physiological stimulation of the vestibular canalicular organs. In every day's neurootological routine it is applied in sinusoidal, triangular and trapezoidal shape. The rotatory intensity damping test (RIDT) applies a perrotatory and postrotatory test combination of a trapezoidal stimulus pattern. Thus the perrotatory as well as the postrotatory nystagmus can be recorded during several phases of the after phenomena. In terms of acceleration/time plots the supraliminal perrotatory acceleration exhibits a rectangular or step-like stimulus characteristic. The supramaxim

[Pseudocaloric nystagmus (author's transl)].

A pseudocaloric nystagmus is, in its strict sense, a spontaneous nystagmus activated by the unspecific stimulus of syringing the auditory canal. 40 patients with a unilateral, peripheral-vestibular defect were examined electronystagmographically. With most patients the 30 degrees caloric stimulation gave an incorrect result of the degree of hypoexcitability with all parameters (duration, maximum frequency and maximum intensity). This incorrect result was most pronounced with patients after neurectomy of the vestibular nerve, who frequently had a heavily reduced reaction of the healthy ear. For

Methysergide in the treatment of narcolepsy.

Five patients with narcolepsy (four with the allied symptom of cataplexy) were treated with the serotonin antagonist methysergide. All patients had as good control of their sleep attacks while on methysergide therapy as on a control period of dextroamphetamine therapy. The cataplexy was less well controlled by methysergide than by dextroamphetamine, but improved when compared to a period without medication. Two patients developed severe calf claudication while on methysergide.

Fourier Synchrosqueezing Transform-ICA-EMD Framework Based EOG-Biometric Sustainable and Continuous Authentication via Voluntary Eye Blinking Activities.

In recent years, limited works on EOG (electrooculography)-based biometric authentication systems have been carried out with eye movements or eye blinking activities in the current literature. EOGs have permanent and unique traits that can separate one individual from another. In this work, we have investigated FSST (Fourier Synchrosqueezing Transform)-ICA (Independent Component Analysis)-EMD (Empirical Mode Decomposition) robust framework-based EOG-biometric authentication (one-versus-others verification) performances using ensembled RNN (Recurrent Neural Network) deep models voluntary eye bl…

Multi-head noise regression for single-channel EEG: estimating ocular and muscle contamination to guide artifact removal.

Objective.Electroencephalography (EEG) is often contaminated by ocular electrooculographic (EOG) and muscle electromyographic (EMG) artifacts, yet many pipelines apply uniform denoising, risking distortion of clean neural activity. We propose a two-head, single-channel regressor that estimates EOG and EMG noise-to-signal ratio (NSR, dB) from short segments and test whether it can guide selective artifact reduction, including downstream brain-computer interface decoding.Approach.Using EEGdenoiseNet clean EEG and artifact exemplars, we synthesized 2 s single-channel mixtures with known EOG/EMG N

Narcissism and risky decisions: a neurophysiological approach.

Narcissists are prone to risky decision-making, but why? This study tested-via behavioral and event-related potential (ERP) measures-two accounts: deficiencies in error monitoring and deficiencies in action updating. High and low narcissists were engaged in a monetary gambling task by choosing between a high-risk and a low-risk option while the electroencephalogram (EEG) was being recorded. Two ERP components relevant to outcome evaluation-feedback-related negativity (FRN) and P3-were analyzed, with the FRN serving as an index of error monitoring and the P3 as an index of action updating. Polysomnographic pattern recognition for automated classification of sleep-waking states in infants.

A robust, automated pattern recognition system for polysomnography data targeted to the sleep-waking state and stage identification is presented. Five patterns were searched for: slow-delta and theta wave predominance in the background electro-encephalogram (EEG) activity; presence of sleep spindles in the EEG; presence of rapid eye movements in an electro-oculogram; and presence of muscle tone in an electromyogram. The performance of the automated system was measured indirectly by evaluating sleep staging, based on the experts' accepted methodology, to relate the detected patterns in infants
