To identify areas endemic for Brugia timori infection, a field survey was carried out in 2001 on Alor, East Nusa Tenggara Timor, Indonesia. Elephantiasis was reported on this island by villagers as a major health problem. Bancroftian filariasis was detected in four villages in the coastal area, whereas B. timori was identified in four rice-farming villages. No mixed infections with both species were found. In the highland village Mainang (elevation = 880 m), 586 individuals were examined for B. timori infection and 157 (27%) microfilaria carriers were detected. The prevalence of microfilaremic…

The results of a 16-year, parasitological and clinical, follow-up study on bancroftian filariasis in three villages in north-eastern Tanzania are reported and analysed with respect to the long-term development of microfilaraemia and disease. The study revealed an extraordinary static situation, both at the community level and at the individual level. No significant differences were observed in any of the three villages, with respect to total or age-specific prevalences of microfilaraemia, hydrocele in males aged > or = 20 years or leg elephantiasis in all subjects aged > or = 20 years, when th…

Lymphatic filariasis (LF) is the second most common parasitic disease worldwide, after malaria. It should always be considered in the differential diagnosis for military personnel returning from disease-endemic areas. Numerous outbreaks of LF have been reported in military deployments from World War II. In contrast to the presentation of LF in indigenous populations, which often involves such uncommon complications as elephantiasis and hydrocele, the clinical presentation of LF in military personnel can vary widely and is often vague and nondescript. Common symptoms are pain and swelling of th…

Lymphatic filariasis (LF), or elephantiasis, is a neglected tropical disease caused by filarial worms, primarily Wuchereria bancrofti, transmitted through mosquito bites. It often begins in childhood but may not show symptoms until later, leaving many individuals asymptomatic for long periods. LF disrupts the lymphatic system, causing severe swelling in the limbs and genitals, leading to deformities and disabilities. The World Health Organization estimates that around 51 million people are affected globally, with 36 million suffering from chronic conditions like lymphedema and hydrocele. In 20…

Lymphatic filariasis is caused by nematodes Wuchereria bancrofti, Brugia malayi and Brugia timori. Lymphatic filariasis is a spectrum of illness and can manifest as, asymptomatic microfilaraemia, acute lymphatic filariasis (lymphangitis and lymphoedema), chronic lymphoedema, elephantiasis, hydrocele, tropical pulmonary eosinophilia and some systemic manifestations which involves joint, heart, kidney, nerve, etc. We here present a case of huge splenomegaly caused by lymphatic filariasis which is a rare presentation and only few cases had been reported in the world literature so far. After treat…

Helminth (worm) infections are major public health problems that have important socioeconomic consequences for the more than 2 billion infected individuals. Chronicity (their hallmark) can lead to anemia (in hookworm infection), river blindness (onchcerciasis), cirrhosis (schistosomiasis), and elephantiasis (lymphatic filariasis). Although there have been many studies examining innate immune responses (including TLR expression and function) in response to intracellular pathogens, fewer have examined the interaction of the multicellular helminth parasites and the innate immune system. This revi…

Two types of tumefactions can be distinguished: diffuse, without an actually measurable tumour, and circumscribed, most often corresponding to benign tumours. Among diffuse tumefactions, the author diseases prolapse, inguinal hernias, hematomas, oedemas, vulvar varices, abcesses of the vulva and rare cases of elephantiasis. Each case is described and management is discussed. In a second section, where only large tumours can be termed tumescent, the author discusses cystic formations (serous, mucous and epidermal) and solid formations (fibromas, lipomas, acrochordons, neurofibromas, giant condy…

Elephantiasis verrucosa nostrum occurs when lymphatic obstruction of sufficient magnitude causes back pressure in the lymphatic channels forceful enough to produce superficial vessel dilation which has a "pebbly" or cobble-stone appearance. Unchecked this can progress to a marked verrucous or mossy appearance, frequently complicated by infection, with swelling and gross deformity. The patient presented here was both post-phlebitic and post-lymphatic with significant deformity. He showed dramatic improvement on a regimen of antibiotics and elevation followed by decompression and routine use of …

Lymphatic filariasis is a parasitic disease transmitted by mosquitoes that carry minute, thread-like worms. There are usually no signs or symptoms to be seen in patients with this disease for the most part. Some people, however, develop a condition called elephantiasis, which is marked by severe swelling in the arms, legs, breasts, or genitals. We here descibe an interesting case of a 40-year-old alcoholic brought to the emergency department with an alleged history of injury and incidentally found to have microfilariasis after a thorough blood smear screening. This case illustrates the need fo…

Filarial nematodes are parasitic worms that cause some of the most devastating of all tropical diseases such as elephantiasis and river blindness. Studies on the inflammatory pathogenesis of filarial disease have shown that endotoxin-like activity derived from endosymbiotic Wolbachia bacteria is the major inflammatory stimulus of filarial nematodes. Wolbachia appear to have evolved as essential symbionts of their filarial nematode hosts. Antibiotic depletion of bacteria shows that they are required for normal fertility and development of the worm and may even protect the parasites from host im…

BACKGROUND: Aspergillus species are widely distributed in nature; however, cutaneous infections due to A. terreus are particularly rare. In this report, we describe a very uncommon case of primary cutaneous aspergillosis presenting as cauliflower-like skin lesions caused by A. terreus in a patient with preexisting bilateral filarial elephantiasis of the legs.
MATERIALS AND METHODS: Histopathological examination of the lesions revealed narrow, septate, branching, acute-angled hyphae with delicate chitinous walls and multinucleated giant cells in the dermis.
RESULTS: The fungal isolate was ide…

Our understanding of the pathogenesis of filarial lymphedema, although evolving, is still limited. Recurrent bacterial infections play a major role in the progression of lymphedema to elephantiasis, but the host and parasite factors that trigger disease development are not known. Field studies in Haiti show that lymphedema and host responses to parasite antigens cluster in families, consistent with the hypothesis that host genes influence lymphedema susceptibility. The recent recognition that filarial parasites harbor the endosymbiotic bacteria, Wolbachia, also raises questions about the poten…

A cross-sectional survey was undertaken to determine the prevalence of disease due to lymphatic filariasis in Khurda district of Orissa, India. The total disease attributable to filariasis was significantly higher in males (14.79%) than females (10.04%). However, elephantiasis is more prevalent in females, and adenolymphangitis is more prevalent in males than their counterparts. The prevalences of various forms of the disease are age dependent in both sexes. About one-seventh of men and women of higher age groups suffered from chronic debilitation forms of the disease. The study suggests that …

Filarial lymphoedema is a tropical parasitic worm illness caused mostly by Wuchereria bancrofti. This disease impacts millions of individuals in endemic areas, causing significant impairment and financial discomfort. Elephantiasis is a chronic lymphatic drainage failure causing swelling in the legs and genitalia. It is a chronic condition characterized by the buildup of lymphatic water, causing pain and reduced limb activity. Genetic abnormalities, trauma, surgery, infection, or cancer cause the disease. Treatment options include antiparasitic drugs, surgical interventions, and nonsurgical int…

BACKGROUND: Lymphatic Filariasis (LF) is a vector-borne neglected tropical disease caused by the filarial nematode parasites that can lead to the disfiguring swelling of the limbs (lymphedema or elephantiasis for late stage) and/or genitalia (hydrocele) in men. Growing evidence suggests that not only are filarial lymphedema patients confronted with huge societal stigma and discrimination, but also experience acute filarial attacks accompanied by swelling of the affected part(s), fever, wounds and peeling of the skin of affected limbs(s). However, the extent to which seasonal variation influenc…
