BACKGROUND: Complex emergencies are situations of violence, nutritional insecurity and population displacement of increasing frequency and intensity which pose a serious public health problem to the affected populations. Communicable diseases, especially waterborne diseases as cholera, cause up to 75% of morbidity and mortality in complex emergencies. The objective is to analyze the cholera epidemics in complex emergencies and cholera risk factors associated with the emergency.
METHODS: This descriptive study analyzes cholera epidemics in complex emergencies in affected countries between 2000…

Acute complications arising in abdominopelvic malignancies represent a unique subset of patients presenting to the emergency room. The acute presentation can be due to complications occurring in the tumor itself or visceral or vascular structures harboring the tumor. Multidetector computed tomography (MDCT) is the investigation of choice in the workup of these patients and enables appropriate and timely management. Management of the complication depends primarily on the extent of the underlying malignancy and the involvement of other viscera. The purpose of this article is to depict the imagin

Skin disease is one of the most common reasons dogs and cats are taken to the veterinarian. While many dermatologic conditions cause mild, localized signs, some, such as erythema multiforme, toxic epidermal necrolysis, cutaneous vasculitis, cutaneous drug eruptions, and thermal burns, can cause severe cutaneous signs and may have serious systemic consequences. These patients may present on an emergency basis and require intensive monitoring, diagnostics, and care. Lack of familiarity with these conditions may delay diagnosis and appropriate treatment. Pyotraumatic dermatitis may also prompt ow

Rheumatic disease can affect and severely damage vital organs and thus cause acute emergencies and life-threatening complications. As systemic diseases they can cause any presenting complaint commonly encountered in emergency medicine. Because of their relative rarity in general practice, a high level of vigilance is required in order to recognize an emergency caused by an underlying rheumatic disease in individual cases. The most important rheumatological emergencies comprise septic arthritis, gout, atlantoaxial subluxation, renal crisis and digital ulcers in systemic sclerosis, amaurosis fug…

Fever is defined as a rectal temperature greater than 38.0 degrees C (>100.4 degrees F). A recently documented fever at home should be considered the same as a fever in the ED and should be managed similarly. All febrile infants younger than 28 days should receive a "full sepsis workup" and be admitted for parenteral antibiotic therapy. Clinical and laboratory criteria can be used to identify a low-risk population of febrile infants aged 1 to 4 months who have not received 2 doses of conjugate vaccines for bacterial meningitis. Children with sickle cell disease are at high risk and require spe

Most biliary emergencies can be classified as either infectious or obstructive. Infectious complications include acute cholecystitis and cholangitis. Many of these can be treated either surgically or endoscopically, but in some instances, less-invasive percutaneous techniques can be utilized to successfully treat these conditions. Obstructive complications, especially in the setting of liver transplant, can be serious if not treated quickly. Percutaneous drainage is sometimes the only acceptable treatment alternative for these patients.

BACKGROUND: The increasing prevalence of cardiovascular diseases in subsaharan Africa and their related mortality and morbidity have been established. A large number of them present as emergencies. The purpose of this first multicentric study was to assess the causes, management and outcome of cardiovascular emergencies in savannah and forest environments.
METHODS: A total of 665 patients were included from seven participating centers in seven countries: 417 were classified as belonging to the savannah zone (Dakar, Nouakchott, Ouagadougou, N'djamena) whereas 248 patients were from the forest …

Already the major cause of mortality in the United States, cardio-vascular emergencies will become increasingly prevalent in the future as the geriatric population doubles. This article discusses five cardiovascular emergencies: acute coronary syndrome, congestive heart failure, dysrythmias, aortic dissection, and ruptured abdominal aortic aneurysm. The discussion focuses on the differences in presentation, management, and outcomes that characterize each disease amongst the elderly. As a rule, the elderly have significantly worse outcomes than younger patients.

Grave shortages of blood occur in over 80% of the countries in the world. These result from cultural taboos against donation, inadequate funding of the local transfusion service, high incidence of transfusion transmissible diseases, and rarity of a particular blood group, such as Rhesus negative in Asia, or any combination of these factors. Someone visiting such a country, requiring a transfusion, may, by receiving local blood, deprive the local community of a scarce resource. To resolve this problem the Blood Care Foundation has developed a fast reliable service to provide blood to travellers

When a person experiences arrhythmia (i.e., cessation of effective heart contractions), he or she can lose a pulse and become unconscious. Urgent medical care then is needed to improve outcomes and prevent sudden cardiac death. This care should include an electrocardiogram to identify the underlying arrhythmia and guide treatment. It is important for healthcare professionals to understand the role of medications in the treatment of potentially fatal arrhythmias. They should be familiar with the treatment algorithms for cardiac emergencies (as published by the American Heart Association) such a…

Most severe eye diseases and injuries ultimately require intervention by an ophthalmologist. The urgency of referral depends on various factors, including level of vision loss, duration of symptoms, and presence of comorbid diseases. Of special importance are five acute eye problems in which emergency management by primary care physicians can be critical to visual outcome: high-velocity injuries, chemical injuries, acute angle-closure glaucoma, arteritic ischemic optic neuropathy, and central retinal artery occlusion. This paper will focus on emergencies in glaucoma, especially primary and sec…

Several neurological conditions may present to the emergency department (ED) with airway compromise or respiratory failure. The severity of respiratory involvement in these patients may not always be obvious. Proper pulmonary management can significantly reduce the respiratory complications associated with the morbidity and mortality of these patients. Rapid sequence intubation (RSI) is the method of choice for definitive airway management in the ED and is used for the majority of intubations. The unique clinical circumstances of each patient dictates which pharmacological agents can be used f

MRI often is mandatory in the diagnostic work-up of visual loss, visual field alterations and oculomotor problems. It is performed emergently in patients with painful diplopia associated to mydriasis, to exclude aneurysm, or in patients with painful Horner syndrome to exclude dissection of the internal carotid artery. CT scan in emergency remains useful in case of acute lateral hemianopsia or acute post traumatic visual loss. Progressive neuro-ophthalmological symptoms may require imaging examination in a short delay to define the therapeutic strategy: monocular transient blindness (dissection

The practitioner is very frequently confronted by emergencies in drug-addicted patients also having psychiatric symptomatology. In this article the authors will address emergencies related to alcohol (notably intoxication, pre-DTs and the encephalopathies); emergencies related to cannabis (notably intoxication, psychotic states and panic attacks); and emergencies related to other psycho-active substances (overdoses, drug-withdrawal, psychiatric complications related to cocaine or amphetamines). In the domain of drug addiction, as in psychiatry, the practitioner must give as much importance to …

The use of MR imaging in the emergency setting is evolving. Clear indications include situations in need of contrast media when iodinated contrast cannot be administered or to facilitate assessments in pregnant patients and children when exposure to ionizing radiation is considered unacceptable. The availability of rapid, motion-immune sequences now makes MR imaging a feasible study in less cooperative patients extending the range of patients for whom a diagnostic study can be achieved. Capitalizing on the unique benefits of MR imaging there is optimism that MR imaging can eliminate test redun
