"Are all referrals necessary?" Experiences and perceptions of maternity healthcare providers on emergency intrapartum referrals in Dar es Salaam, Tanzania.

BACKGROUND: Intrapartum continuity of care to reduce maternal morbidity and mortality relies heavily on a functional and effective referral system between tiers of care. Capacity building of providers in managing intrapartum referrals is expected to improve the efficiency of the referral system, but this does not always work in practice. This study explored the experiences and perceptions of maternity healthcare providers on emergency intrapartum referrals in Dar es Salaam, Tanzania. METHODS: An exploratory qualitative study was conducted at Amana Regional Referral Hospital and Muhimbili Nati

A Participatory Curricula for Community Health Workers and Supervisors to Increase HIV Health Outcomes.

Community Health Workers (CHWs) are becoming essential members of the HIV workforce as emerging evidence demonstrates their effectiveness in engaging people with HIV into care and treatment. In 2018, among the estimated 37,000 persons who received an HIV diagnosis, the majority were from racial ethnic minority communities. CHWs serve as a bridge between the community and health care system and have the potential to address structural inequities and reduce the stigma, discrimination and other barriers that prevent people with HIV from seeking and staying in care and treatment. Effective CHW int

Building momentum for local action on problem gambling in Leeds and Yorkshire.

Problem gambling has not been a priority within either Leeds City Council or partnership plans. However, financial inclusion, licencing and public health teams have been able to develop a cross-Council approach to problem gambling. This has been aided by an upfront payment plus annual payments to the Council that have been part of the licencing agreement for a new casino. As a result, research has been commissioned on local prevalence. This showed a higher rate of problem gamblers (1.8%) than national estimates with a similar level to nationally of those 'at risk'. The research also showed tha

Planning Dementia-Capable Group Homes for Adults With Intellectual Disability: Insights From a 15-Year Longitudinal Study.

Adults with intellectual disability (ID) experience elevated risk of developing dementia, including earlier onset in some conditions such as Down syndrome. As increasing numbers of adults with ID reach older age, community service systems are confronted with the need to adapt residential supports originally designed for lifelong developmental assistance to accommodate progressive neurodegenerative conditions. One emerging response has been the development of small, community-based dementia-capable group homes that integrate specialized care practices within familiar neighborhood environments.

Falls Do Not Just Break Bones--They Break Lives: World Spinal Cord Injury Day (September 5, 2025) and Indian Perspectives.

Spinal cord injury (SCI) is a catastrophic condition that often results in lifelong disability, loss of independence, and profound social and economic consequences. While road traffic accidents remain a major cause, falls are increasingly emerging as a significant and preventable contributor to SCI, particularly in India. The theme of World SCI Day 2025--"Fall Prevention, Spinal Cord Protection"--highlights the urgent need to address this issue through proactive measures. Effective fall prevention requires a multipronged approach that combines home and community safety interventions, medical s

Rising quantitative productivity and shifting readership in academic publishing: Bibliometric insights from monkeypox literature.

The sudden spread of the monkeypox virus has been accompanied by an increase in the scientific interest in the virus. More than 1,400 PubMed-indexed documents have been authored by about 5,800 unique authors, averaging around 120 publications per month. This sheer rise in the number led us to explore the content published in the literature. We discovered more than 30% of the documents are Quantitative Productivity (QP) i.e., papers that illustrate the emerging trends of parachute concerns, modified salami tactics, cyclic recycling, and excellence in redundancy. In addition, we found few common

[Knowledge of the population about leading symptoms of cardiovascular emergencies and the responsibility and accessibility of medical facilities in emergencies : Results of the KZEN study in Western Palatinate].

BACKGROUND: The Westpfalz is a mainly rural region in the southwestern part of the German state of Rhineland-Palatinate with 527,000 inhabitants and demonstrates a higher than average cardiovascular mortality compared to the rest of Germany. The reasons are not known. Our study attempted to investigate whether significant deficits in knowledge of the population on cardiovascular emergencies, the accessibility of emergency medical services (EMS) or the different responsibilities and abilities of the medical facilities could be held responsible for this. These factors are of the utmost importanc

Opinions on conscientious objection to induced abortion among Finnish medical and nursing students and professionals.

BACKGROUND: Conscientious objection (CO) to participating in induced abortion is not present in the Finnish health care system or legislation unlike in many other European countries. METHODS: We conducted a questionnaire survey with the 1(st)- and the last-year medical and nursing students and professionals (548 respondents; response rate 66-100%) including several aspects of the abortion process and their relation to CO in 2013. RESULTS: The male medical respondents chose later time points of pregnancy than the nursing respondents when considering when the embryo/fetus "becomes a person". O

Decision making in recovery-oriented mental health care.

OBJECTIVE: Patient-centered communication has been linked to patient satisfaction, treatment adherence and outcomes. Shared decision making (SDM) has been advocated as an important and ethically essential aspect of patient-centered care, but SDM has received relatively little attention in mental health care, despite studies indicating that consumers want to be involved in making decisions. This is particularly important in a recovery-oriented system, where consumers are active participants in their treatment and rehabilitation. Because medication management is a key component of recovery from

Exploring Pre-hospital Emergency Care Challenges and Interventions in Singapore: A Qualitative Environmental Scan.

ObjectiveEmergency Departments in Singapore face increasing demands for acute care. Possibly due to factors such as an ageing population with increasing burden of complex chronic illnesses. However, a significant proportion of ED attendances are potentially avoidable cases which may not require emergent care. It is timely to consider approaches for Pre-hospital Emergency Care (PEC) redesign that direct patients towards the most appropriate care settings, thereby mitigating demand for emergency care. By gathering stakeholders' perspectives, findings of this study could help to propose and evalu

Does a pre-hospital emergency pathway improve early diagnosis and referral in suspected stroke patients?--Study protocol of a cluster randomised trial [ISRCTN41456865].

BACKGROUND: Early interventions proved to be able to improve prognosis in acute stroke patients. Prompt identification of symptoms, organised timely and efficient transportation towards appropriate facilities, become essential part of effective treatment. The implementation of an evidence based pre-hospital stroke care pathway may be a method for achieving the organizational standards required to grant appropriate care. We performed a systematic search for studies evaluating the effect of pre-hospital and emergency interventions for suspected stroke patients and we found that there seems to be

Elder Mistreatment Documentation by Prehospital Clinicians: An Analysis of the National Emergency Medical Services Information System Database.

OBJECTIVES: Elder mistreatment (EM), encompassing abuse and neglect, is a significant public health issue, affecting up to 10% of community-dwelling older adults annually. Elder mistreatment is a growing concern with a higher prevalence in institutional settings and substantial associated healthcare costs. Prehospital clinicians (PHCs) such as emergency medical technicians and paramedics are uniquely positioned to detect and report EM during their interactions with older adults in their homes. The objective of the study is to describe the rate and characteristics of EM documented by PHCs using

Leveling of triggers: a comprehensive summative content analysis of factors contributing to physical violence in emergency medical services.

BACKGROUND: The literature has identified numerous factors that contribute to workplace violence, ranging from environmental stressors to interpersonal conflict. However, a gap remains in our understanding of the specific factors associated with physical violence, particularly concerning its frequency and perceived significance. METHODS: A summative content analysis was conducted via the electronic survey platform Porsline in Iran for data collection. In April 2024, EMTs working in urban, road, and air bases in western Iran participated in the study. In total, 358 EMTs were selected via conve

Care pathways in informal and popular health subsystems in the experience of people living with HIV, Rio de Janeiro, Brazil, between 2021 and 2022.

INTRODUCTION: Four decades after the emergence of the Human Immunodeficiency Virus (HIV) infection, health systems still face the challenge of implementing effective policies to reduce the incidence of infection, mortality and problems such as stigma faced by people living with HIV (PLHIV). The objective of the study is to analyze the care pathways followed by PLHIV in the informal and popular health subsystems. METHODOLOGY: Qualitative study of 45 PLHIV treated at specialized Polyclinics in a city in the state of Rio de Janeiro, Brazil. Individual interviews were conducted from December 2021

'Working to stay healthy', health-seeking behaviour in Bangladesh's urban slums: a qualitative study.

BACKGROUND: Kamrangirchar and Hazaribagh are the largest slum areas in Dhaka, Bangladesh. In 2013, Médecins Sans Frontières initiated an urban healthcare programme in these areas providing services for factory workers and responding to the sexual and reproductive health needs of young women. Little in-depth information is available on perceptions of health and health seeking behaviour in this population. We aimed to provide a better understanding of community perceptions toward health and health care in order to inform programme strategies. METHODS: In-depth interviews were conducted with wom

Designing for generalism, courage, and systems: a curriculum scholarship account of an integrated family medicine and primary care block.

BACKGROUND: Pre-clerkship Family Medicine teaching often covers content well but does too little with experiential immersion, structured reflection, and the open engagement with clinical uncertainty that defines generalist work in practice. We describe the partnership-based redesign of the COM353 Family Medicine and Primary Care block at Alfaisal University, Riyadh, with student-reported outcomes from the first delivered cohort. The paper is offered as a worked account of how academic-academic and academic-service partnerships can themselves become curriculum. PEDAGOGICAL FRAMEWORK: The redes

Qualitative Research of Violent Incidents Toward Young Paramedics in the Czech Republic.

INTRODUCTION: Prehospital and emergency medical services (EMS) providers are usually the first to respond to an individual's urgent health needs, sometimes in emotionally charged circumstances. Because violence toward EMS providers in the Czech Republic is often overlooked and under-reported, we do not have a complete understanding of the extent of such violence, nor do we have recommendations from EMS professional organizations on how to resolve this problem in prehospital emergency medicine. METHODS: We conducted this study to explore the process of violence against EMS providers, using the

Extended-Release Buprenorphine Administration by Emergency Medical Services Paramedics: A Case Series.

BACKGROUND: Medications for opioid use disorder (MOUD), including buprenorphine, reduce mortality and improve treatment retention. However, daily sublingual formulations may be impractical for patients with unstable housing, limited healthcare access, or interrupted care. Extended-release buprenorphine (XR-BUP) provides sustained therapeutic levels without daily dosing and may overcome barriers to adherence. Emergency Medical Services (EMS), both 911 and Mobile Integrated Healthcare (MIH) models, are uniquely positioned to initiate or maintain MOUD among high-risk individuals in community sett

Dual disorders: an overview.

The coexistence of substance use disorders (SUD) and other mental disorders - commonly referred to as dual disorders (DD) - is highly prevalent and clinically significant. Although various terms have been used over time (e.g., psychiatric comorbidity, dual pathology), the lack of a standardised definition has created inconsistencies in diagnosis, communication, research, and treatment. Epidemiological studies estimate that 30-80% of individuals with SUD have co-occurring psychiatric disorders, with prevalence depending on the substance involved, gender, age, and the population studied (general
