Endodontic anesthesia in mandibular molars: a clinical study.

Sixty-one mandibular molar teeth with clinically manifest pulpitis, which required endodontic therapy, were studied. Twenty-seven subjects received standard inferior alveolar nerve block (IANB) with 2% lidocaine HCI with 1:100,000 epinephrine and 34 subjects received IANB with 3% mepivacaine with no vasoconstrictor. Pulpal anesthesia was assessed with dichlorodifluormethane (DDM). Subjects who gave a positive response to DDM were given a periodontal ligament injection with 2% lidocaine with 1:100,000 epinephrine. This study showed that 3% mepivacaine HCI is as effective as 2% lidocaine HCI in

Incidence of periradicular pathoses in endodontic treatment failures.

Biopsy reports from 150 periradicular tissue specimens obtained from teeth refractory to nonsurgical endodontic therapy were reviewed. The specimens were submitted by postdoctoral dental students in the Department of Endodontics, and the biopsy reports were prepared by oral pathologists at the University of Texas Health Science Center at San Antonio. The study found that 59.3% of the periradicular lesions were granulomas, 22% cysts, 12% scars, and 6.7% other pathoses. The majority (56%) of endodontically treated cases which failed to heal were recognized within 2 yr after the completion of the…

[Lack of healing following adequate endodontic treatment].

When an endodontic treatment is to be redone, generally the X-ray reveals some form of poor or inadequate filling. Most endodontic cases fail because of inadequate endodontic treatment (cleaning, shaping, filling). However, sometimes good clinical treatment isn't followed by healing! This article sums up the possible reasons to consider. In order to improve the success rate of the endodontic treatments, practical hints are given regarding the non-surgical as well as the surgical (re)treatments.

[Endodontic treatment of a "peculiar" upper central incisor].

The Author presents a clinical case of a maxillary central incisor where the periapical radiograph revealed a process of fusion, an internal resorption, the presence of a "dens invaginatus" and an important periapical radiolucency. In this tooth was located, treated and filled three different root canals.

Restorative and endodontic management of an anomalous mandibular molar.

Morphologic variations of dental hard tissue should be recognized and considered during restorative procedures. Overlooking anomalies increases the potential for clinical complications. Amalgam was used erroneously in a patient when the presence of a paramolar structure was overlooked. The clinical complications that resulted from overlooking the anomaly led to restorative, endodontic, and periodontal failures. The present report describes the use of conservative endodontics and a cast restoration, taking into account the presence of the paramolar structure, to correct the case.

Radiographic evaluation of intraosseous implants of endodontic materials.

Following several case reports of cyst development after endodontic treatment of decidous teeth, the osseous response to formaldehyde- or phenol-based materials was evaluated by a standardized radiographic technique. Endodontic medicaments contained in Teflon carriers were implanted between feline deciduous canine and molar teeth and the permanent successional teeth. Persistent radiolucency surrounding formocresol implants and sequestration of Kri-paste implants were noted in contrast to apparent bony healing around the zinc oxide-eugenol controls and those implants containing glutaraldehyde.

Relationship of intracanal medicaments to endodontic flare-ups.

The purpose of the study was to compare the effect of three intracanal medicaments on the incidence of post-instrumentation flare-ups. All teeth were instrumented to a predetermined minimum size using a 0.5% solution of sodium hypochlorite as the irrigant. Formocresol, Ledermix, and calcium hydroxide were placed in strict sequence irrespective of the presence or absence of symptoms or radiographic signs of apical periodontitis. The patients were given written post-operative instructions and a prescription for 600 mg ibuprofen to be taken if mild to moderate pain developed. If severe pain and/o

Chronic facial pain associated with endodontic therapy.

Retrospective study was done on 118 patients who underwent nonsurgical endodontics and then surgical endodontics. Seventy-nine patients had pain before nonsurgical endodontics, and an additional 39 were pain free. After the open surgical procedure, six patients (5%) had continual pain and were considered failures. Three patients (2.5%) were thought to have had posttraumatic dysesthesia, and the other three (2.5%) were considered to have phantom tooth pain. Possible mechanisms for each chronic pain type are discussed.

An evaluation of tricalcium phosphate as a treatment for endodontic perforations.

In the rat, perforations of maxillary molars were created and treated with either tricalcium phosphate (Synthograft) or Cavit. At four time intervals (1 day, 1 wk, 2 wk, and 1 month), four evaluative factors (inflammation, bone resorption, cementum and dentin resorption, and epithelial proliferation) were analyzed and compared for the two materials. For the individual time periods, there were no statistically significant differences between the two materials. However, when all four time periods were combined, there was a statistically significant better result for tricalcium phosphate than for

Draining fistulas associated with an endodontically treated tooth.

A case of periapical actinomycosis is presented. Although apparently uncommon, actinomycosis should be considered when a chronic periapical lesion, often in the maxilla, is associated with an endodontically treated tooth or a previous history of trauma. Histologic examination of the tissue or the presence of "sulfur granules" in the exudate will usually establish the diagnosis. Treatment consists of local curettage and antibiotic therapy.

Impact of Contracted Endodontic Access Cavities on the Fracture Resistance of Endodontically Treated Teeth After Mechanical Aging by Simulated Chewing Forces.

INTRODUCTION: This in vitro study aimed to comparatively evaluate the fracture resistance of contracted endodontic cavities (CECs) versus traditional endodontic cavities (TECs) in mandibular molars after subjecting the samples to a chewing simulator. METHODS: A total of 24 freshly extracted human mandibular molars were included in the study. Teeth with intact crowns and mature root apices that were devoid of caries, attrition, restorations, and cracks were selected and randomly assigned to 3 groups (n = 8) as follows: Group 1: TECs, Group 2: CECs, and Group 3: intact teeth (control group). Periapical and Endodontic Status Scale for Endodontically Treated Teeth and Their Association with Maxillary Sinus Abnormalities: A Cone-beam Computed Tomographic Study.

INTRODUCTION: This retrospective cross-sectional study investigated the applicability of the periapical and endodontic status scale (PESS) to determine the association of endodontically treated teeth with maxillary sinus (MS) abnormalities through cone-beam computed tomographic imaging. METHODS: A total of 631 endodontically treated teeth were analyzed. MS abnormalities were classified as mucosal thickening, sinus polyp, antral pseudocyst, nonspecific opacification, periostitis, and antral calcification. The PESS was used to evaluate the quality of endodontic treatment as well as periapical t
