Kimura disease presenting as epiglottic masses: A case report.

BackgroundKimura disease presents considerable diagnostic and therapeutic challenges because of its rarity, polymorphic early manifestations, recalcitrant clinical course, high recurrence rate, and potential for multiorgan involvement.Case presentationA Chinese woman in her late 40s presented with a 3-year history of foreign body sensation in the pharynx and a 6-month history of dysarthria. Laryngoscopy revealed a smooth mass on the epiglottis. Laboratory investigations demonstrated elevated peripheral blood eosinophil counts and serum immunoglobulin E levels. The mass, together with the epigl

A giant congenital epiglottic cyst in a neonate: a case report.

Congenital epiglottic cysts, though rare, represent a recognized etiology of upper airway obstruction in neonates and infants. Airway management of large epiglottic cysts presents significant challenges due to the inherent risk of catastrophic airway compromise. We present a case of difficult airway management in a neonate with a giant congenital epiglottic cyst, detailing an effective technique for glottic visualization and successful endotracheal intubation.

Rare Epiglottic Granulomatosis: Diagnostic Challenges and Therapeutic Implications.

We report a rare case of epiglottic granulomatosis with polyangiitis (GPA) in a 72-year-old woman presenting with 5 years of progressive dysphagia and dyspnea. Initial laboratory studies revealed leukocytosis (White Blood Cells 14.2 × 10³/µL), eosinophilia (8%), elevated inflammatory markers (C-Reactive Protein 42 mg/L, Erythrocyte Sedimentation Rate 78 mm/hour), and Proteinase 3 Anti-Neutrophil Cytoplasmic Antibody (PR3-ANCA) positivity (65 IU/mL). Laryngoscopy identified supraglottic edema with a right-sided mass, confirmed by Computed Tomography as localized epiglottic thickening without ca

A Large Cohort Analysis of Epiglottic Phenotypes and Pharyngeal Residue.

OBJECTIVE: To describe the phenotypic characteristics of the epiglottis at rest and their impact on vallecular residue. METHODS: Videofluoroscopic studies (VFSS) were pooled from 2 Laryngology practices, and Image J was used to measure epiglottic anatomic features at rest. Studies were rated by the MBSImp and presence of vallecular residue following swallow of thin and puree boluses. A conditional inference tree analysis was performed to isolate which epiglottic parameters were risk factors for presence of vallecular reside followed by logistic regression. RESULTS: The majority of patients h

Predicting epiglottic collapse in patients with obstructive sleep apnoea.

Obstructive sleep apnoea (OSA) is characterised by pharyngeal obstruction occurring at different sites. Endoscopic studies reveal that epiglottic collapse renders patients at higher risk of failed oral appliance therapy or accentuated collapse on continuous positive airway pressure. Diagnosing epiglottic collapse currently requires invasive studies (imaging and endoscopy). As an alternative, we propose that epiglottic collapse can be detected from the distinct airflow patterns it produces during sleep.23 OSA patients underwent natural sleep endoscopy. 1232 breaths were scored as epiglottic/non

Invasive epiglottic aspergillosis: A case report and literature review.

Invasive aspergillosis is a life-threatening infection in immunocompromised hosts and occurs most frequently in the lungs. Invasive laryngeal aspergillosis is extremely rare. Due to the potential progression of invasive aspergillosis, antifungal therapy must be started immediately in cases involving clinical suspicion of the disease. A 65-year-old male with agranulocytosis complained of sore throat and dysphagia. His epiglottis was covered with caseating granulomatous lesions and the tissue was easily disrupted. A histopathological examination showed an aggressive invasion of Aspergillus speci

[Individualized partial laryngectomy during surgery of epiglottic carcinoma].

OBJECTIVE: To investigate an individualized operative modality of partial laryngectomy for suprahyoid epiglottis centered epiglottic carcinoma. METHOD: Eleven cases of epiglottic squamous carcinoma which primarily focused in the suprahyoid region were included. Preoperative CT indicated that obvious margin with low density existed in the pre-epiglottic space in 3 cases, and pre-epiglottic space were involved in 8 cases, among which the arytenoid region were involved in 2 cases. An individualized partial laryngectomy was manipulated beyond the safe margin, an arytenoid epiglottic fold-plasty w

Fatal epiglottic abscess after radiotherapy for laryngeal carcinoma.

Acute epiglottitis is a rare condition, especially in adults, largely owing to the widespread vaccination against Haemophilus influenzae. Traumatic epiglottitis as a result of thermal or caustic insults is documented. Epiglottic abscess formation is described as a sequela of epiglottitis in some cases. The development of epiglottic abscess from epiglottitis secondary to radiotherapy has previously been described in the literature; however, there are no reports of fatalities due to epiglottic abscess after radiotherapy. We present an unusual case of an adult sudden death due to epiglottic absce

Epiglottic prolapse induced by lighted stylet tracheal intubation.

We report a case of epiglottic prolapse induced by lighted stylet tracheal intubation perceived by following upper gastrointestinal endoscopy. A 68-year-old male was to undergo endoscopic mucosal resection (EMR) under general anesthesia for a superficial orolarynx cancer spreading over the root of the tongue. Because the mucosal change was so minimal, intubation was performed with a lighted stylet instead of a direct laryngoscope, to prevent its metal blade spoiling the delicate endoscopic findings. After intubation, endoscopy revealed that the epiglottis folded down completely into the laryng

Outcomes studies of epiglottic and base of tongue prolapse in children.

OBJECTIVES: The purpose of this study was to compare previously reported flexible fiberoptic laryngoscopy (FFL) findings of a grading system for children with epiglottic and base of tongue (EBT) prolapse with findings at follow-up FFL. Surgical outcomes and tracheotomy decannulation are also reported. STUDY DESIGN: : Retrospective medical record review. METHODS: Fourteen children with EBT prolapse had transnasal FFL in the supine position on at least two occasions. Findings were graded for initial versus most recent FFL. The previously published EBT prolapse grading system was reapplied. Mea

Epiglottic and base-of-tongue prolapse in children: grading and management.

OBJECTIVES: A distinct entity of airway obstruction from epiglottic and base-of-tongue (EBT) prolapse in the pediatric population is defined. Laryngopharyngeal findings, swallowing dysfunction, and gastroesophageal reflux disease are described in a group of children with EBT prolapse. A new grading system is also presented. STUDY DESIGN: A prospective study was conducted of laryngopharyngeal findings in children with EBT prolapse, a description of a new grading system, and review of the pediatric literature. METHODS: Fourteen children with EBT prolapse were prospectively studied with flexibl

Epiglottic tuberculosis in a patient treated with steroids for Addison's disease.

Isolated epiglottic tuberculosis (TBC) is uncommon and has rarely been described. We report the case of a 40-year-old man with tuberculous involvement of the epiglottis and primary adrenal insufficiency. Endoscopic examination showed a severely swollen epiglottis with granulomatous and partially necrotic mucosa. The patient has been treated with glucocorticoids for four years due to primary adrenocortical insufficiency. We therefore assume that tuferculous involvement of epiglottis is due to the reactivation of pulmonary TBC. We also discuss differential diagnosis and management of epiglottic

[Epiglottic abscess as a rare reason for airway obstruction in adults].

Acute epiglottitis in adults is a potentially life-threatening disease of increasing incidence. Although pharyngitis is the most common cause of sore throat in the adult, acute epiglottitis must be considered in the differential diagnosis when there is unrelenting throat pain and minimal objective signs of pharyngitis. We report the case of a 45-year old man with acute epiglottitis and occlusion of the upper airways due to an epiglottic abscess. A brief discussion of the diagnosis and treatment of adult epiglottitis is presented. Patients with acute painful dysphagia should be considered to ha

Acute epiglottitis as a cause of airway obstruction in an adult patient.

Epiglottitis in adults is a dangerous infectious disease with a rising incidence and potential fatal complications as illustrated in this case report. Like in children, skilled and aggressive airway protection with appropriate antibiotic therapy have been effective in reducing mortality.
