Legg-Calvé-Perthes disease and slipped capital femoral epiphysis: major developmental causes of femoroacetabular impingement.

Problematic femoroacetabular impingement frequently is seen following Legg-Calvé-Perthes disease (LCPD) in young children and following slipped capital femoral epiphysis (SCFE) in older children and adolescents. Although symptoms may be mild in adolescents and young adults, chondral damage following LCPD and SCFE deformity is cumulative and irreversible, which has led to a recent emphasis on the consideration of early treatment. The surgical dislocation approach and improved MRI and three-dimensional CT have revealed common patterns of deformity and structural damage. The surgical dislocation

Primary hyperparathyroidism presenting with bilateral slipped capital femur epiphysiolysis. A pictorial essay.

It has become rare to find the typical radiological manifestations of primary hyperparathyroidism like generalized osteoporosis, brown tumors and advanced bone resorption because of the generalized usage of biochemical screening techniques. We present a 17-year-old patient with a parathyroid adenoma resulting in these typical skeletal manifestation throughout the skeleton combined with secondary bilateral slipped capital femur epiphysiolysis.

[Slipped capital femoral epiphysis].

Slipped capital femoral epiphysis (SCFE) is a common hip disorder in adolescence and should be diagnosed and treated surgically as soon as possible. The etiology, biomechanical, biochemical and hereditary factors are still under investigation. The classification of SCFE is based on the acuteness, clinical and radiomorphological findings. Avascular necrosis of the epiphysis (AVN) and chondrolysis occur more often in operated than in non-operated patients. Medium and long-term sequelae of SCFE are loss of function and degenerative joint disease due to femoroacetabular impingement (FAI) or conseq

Grade-III slipped capital femoral epiphysis with unstability: a report of three cases.

We reviewed the cases of three patients with an unstable grade-III slipped capital femoral epiphysis treated between 2001 and 2003. Clinical records and imaging studies were reviewed for patient's history, anatomic features of the slip, definitive treatment and clinical outcome. The duration of the follow-up ranged from twenty-four to fourty-eight months.

[Bilateral idiopathic femur head epiphyseolysis ("slipped epiphysis") in cats: literature review and case description].

One case of a bilateral idiopathic slipped capital femoral epiphysis--"slipped epiphysis"--in a cat is described. The similarities and the differences between the cases in human and small animal medicine concerning the incidence, the hypotheses of the pathogenesis and the treatment options are discussed.

Pathogenesis of slipped capital femoral epiphysis: current concepts.

Since Ambrose Paré in 1572 initially described what is believed to be the first case of slipped capital femoral epiphysis (SCFE), a virtual plethora of unproven theories has evolved in the ensuing years. The most currently popular theories embrace a framework of biomechanical events versus biochemical events having impact at the time of puberty. The zone of slipping always occurs primarily through the zone of hypertrophy in a corrugated undulating fashion. All investigative studies show that the growth plate at the time of puberty is weakened in SCFE. The final common pathway appears to be a m

[Epiphysiolysis of the femoral head--can surgery prevent later arthritis?].

Slipped capital femoral epiphysis is one of the few orthopaedic emergencies. All orthopaedic surgeons agree that operation is the treatment of choice. In early stages, complete healing is possible. Fixation in situ is sufficient. If there is more severe dislocation of the head of the femur, the results become worse. More sophisticated and more difficult interventions aim at correction of the deformity, which may be a preliminary stage ['prearthrosis'] of osteoarthritis later in life. According to the extent of the slip, the surgeon may choose among different operations. The decision in a parti

Distal humeral epiphyseal separation in a young athlete: a case report.

Elbow pain in the young throwing athlete can have numerous etiologies. Skeletally immature athletes commonly sustain epiphyseal injuries secondary to trauma and/or repetitive activity. The case of a 7-year-old Little League pitcher with the diagnosis of distal humeral epiphyseal separation is presented along with a review of the current literature.

Treatment of slipped capital femoral epiphysis. Spica-cast immobilization.

Thirty-two patients (thirty-seven hips) who had a so-called acute-on-chronic or chronic slipped capital femoral epiphysis were treated with traction for relief of symptoms and then with immobilization in a spica cast for eight to sixteen weeks. The disappearance on radiographs of a metaphyseal juxtaphyseal radiolucency, rather than closure of the physis, was used as the criterion for removing the cast. In one (3 per cent) of the thirty-seven hips, the slip progressed; possibly this could have been prevented by keeping the cast on for a longer period of time. Narrowing of the cartilage space wa

Rotational injury of the distal tibial physis.

Rotational displacement of the distal tibial physis occurred in a 12-year-old boy. A review of the literature indicates that long-term follow-up is necessary to determine whether premature physeal closure will occur.

Epiphyseal separation in scurvy.

The case of a 10-month-old boy with scurvy and epiphyseal displacement of both the distal femur and the right proximal tibia is presented. He was treated without reduction of the displacement.

Slipped capital femoral epiphysis: still an emergency.

Two case histories are presented in which acute progression of slipped capital femoral epiphysis occurred after a radiographic diagnosis of chronic slipped capital femoral epiphysis had been made. The importance of recognizing the potential for acute progression of a slipped epiphysis and the need for prompt orthopedic intervention are emphasized.

Spontaneous bilateral distal femoral physiolysis due to scurvy.

A bilateral distal femoral epiphysiolysis was observed in a 28-month-old child suffering from scurvy. We explain the mechanisms of the separation of the epiphysis due to loss of strength in the calcification zone of the cartilage columns.
