[Diagnostic value of brainstem auditory evoked potentials (author's transl)].

The present work is an evaluation of brainstem auditory evoked potentials (BAEP) as a tool for neurological diagnosis. Filtered alternate clicks with monaural stimulation and ipsilateral recording between the vertex and the ipsilateral mastoid have been used. Normative data of the laboratory established in 50 normal subjects, mean age 35 years, and criteria for interpretation are given. 155 patients had or were suspected of having multiple sclerosis. In definite multiple sclerosis (33 patients), BAEP were abnormal in 67 p. 100 of the cases, among them the 4 patients with an internuclear ophtal…

Visual evoked potentials and the visuogram in multiple sclerosis.

Visual evoked potential (VEP) latency to a sinusoidal grating pattern was measured in each eye of 103 patients with multiple sclerosis (MS) and compared with results in a control group of 56 patients hospitalized for other neurological conditions. Of the 50 patients classified as having definite MS, 90% showed prolonged latency (over 131 msec) in one or both eyes. In each eye of 24 of the MS patients, psychophysical measurement of the detectability of grating patterns was obtained. This test was abnormal in 11 of 13 patients with definite MS, 3 of 4 with probable MS, and 5 of 7 with possible M

Laser evoked potentials in patients with trigeminal disease: the absence of Adelta potentials does not unmask C-fibre potentials.

OBJECTIVE: Although laser stimuli activate both Adelta- and C-fibres, the corresponding laser evoked potentials (LEPs) remain restricted to the Adelta-fibre input. Previous studies found C-LEPs after limb stimulation only in subjects with block or clinical impairment of Adelta-fibres. In this study, we aimed at verifying whether in the trigeminal territory the impairment of Adelta-fibres unmasks the C-LEP. METHODS: By collecting retrospectively LEPs recorded in 370 patients, we analyzed the results from 150 trigeminal divisions with absent Adelta-LEPs. RESULTS: We found signals that were con…

Transesophageal motor-evoked potentials, a novel method induced by transesophageal spinal cord stimulation, are less sensitive to anesthetics than transcranial motor-evoked potentials.

PURPOSE: Intraoperative neurologic monitoring can be useful, but transcranial motor evoked potentials (TcMEPs) are sensitive to anesthetic agents. We compared the effects of anesthetics on the newly developed transesophageal motor evoked potentials (TeMEPs) with those on TcMEPs. METHODS: Eleven pigs (25.6 ± 0.8 kg) were anesthetized by desflurane inhalation, remifentanil was maintained at 0.5 µg/kg/min until the end of the experiment. End-tidal desflurane concentration was then maintained at 7, 4, 10, and 13%, and TcMEPs and TeMEPs were measured at each concentration. Desflurane was then disc…

Coupling between visual evoked cerebral blood flow velocity responses and visual evoked potentials in migraneurs.

Neurovascular coupling may be altered in migraneurs. Therefore, visual evoked potentials (VEP) and visually evoked cerebral blood flow velocity responses (VEFR) were simultaneously recorded in 30 healthy controls and 30 migraneurs interictally using a checkerboard stimulus with visual contrasts of 1%, 10% and 100%. The VEFR were measured in the posterior cerebral artery using transcranial Doppler and VEP were recorded from occipital leads. We found an increase in VEFR and VEP in both the healthy and migraneur groups (P < 0.01). VEFR were significantly higher in migraneurs (P < 0.01), while VEP…

Diagnosis of brachial plexus traction lesions by sensory nerve action potentials and somatosensory evoked potentials.

Forty-two patients with unilateral brachial plexus traction lesions were investigated by recording sensory nerve action potentials (SNAPs) from the lower arm and somatosensory evoked potentials (SEPs) from the clavicle, the cervical spine and the scalp overlying the contralateral somatosensory cortex, in response to electrical stimulation of peripheral nerves. The median and radial nerves were assumed to derive principally from the C6 and C7 roots, and the ulnar nerve from the C8 and T1 roots. Combination of SEP and SNAP findings suggested a location for the lesion (preganglionic, postganglion…

Cold-evoked potentials in clinical practice: A head-to-head contrast with laser-evoked responses.

BACKGROUND: Innocuous cooling of the skin activates cold-specific Aδ fibres, and hence, the recording of cold-evoked potentials (CEPs) may improve the objective assessment of human thermo-nociceptive function. While the feasibility of CEP recordings in healthy humans has been reported, their reliability and diagnostic use in clinical conditions have not been documented. METHODS: Here, we report the results of CEP recordings in 60 consecutive patients with suspected neuropathic pain, compared with laser-evoked potentials (LEPs) which are the gold standard for thermo-algesic instrumental assess…

Vestibular evoked myogenic potentials (VEMPs) evoked by air- and bone-conducted stimuli in vestibular neuritis.

OBJECTIVE: To compare and characterise abnormalities for short latency vestibular evoked myogenic potentials (VEMPs) elicited by air- (AC) and two differing types of bone-conducted (BC) stimuli during vestibular neuritis (VN). METHODS: AC (500Hz short tone bursts) and two BC stimuli (500Hz at the forehead and impulses at the mastoids) were used to evoke cervical and ocular potentials (cVEMPs and oVEMPs) in VN patients (n=22) and healthy subjects. RESULTS: More abnormalities were observed for the oVEMP than the cVEMP when using either AC 500Hz or BC 500Hz. The AC stimulus showed slightly more

Transcranial Motor-Evoked Potentials Provide More Reliable Intraoperative Neuromonitoring Than Somatosensory-Evoked Potentials in Charcot-Marie-Tooth Patients Undergoing Periacetabular Osteotomy for Hip Dysplasia.

BACKGROUND: Charcot-Marie-Tooth (CMT) disease is a common inherited progressive sensorimotor neuropathy which can complicate intraoperative monitoring (IOM). Transcranial motor-evoked potentials (TcMEPs) have been found to be effective in obtaining IOM in CMT patients undergoing spine surgery. The aim of this study was to examine the effectiveness of TcMEPs and somatosensory-evoked potentials (SSEPs) in IOM in CMT patients undergoing periacetabular osteotomies (PAOs) for hip dysplasia. METHODS: A retrospective case series was conducted in all CMT patients aged <19 years at a single institutio…

Pain-Related Evoked Potentials in Hereditary Transthyretin Amyloidosis With Polyneuropathy.

INTRODUCTION/AIMS: Pain-related evoked potentials with concentric electrodes (PrEP-CE) provide a method for screening for small fiber neuropathy, which is usually the first manifestation of hereditary transthyretin amyloid polyneuropathy (ATTR v-PN). Despite that, this technique has not yet been fully explored in this condition. The current study aimed to compare PrEP-CE responses among patients with ATTRv-PN, presymptomatic carriers of TTR mutations, and healthy controls. METHODS: We recruited 17 patients with ATTRv-PN, 12 presymptomatic TTR mutation carriers (pre-ATTRv-PN), and 13 healthy c…

Variability in sensory processing and evoked potentials in Rett syndrome.

BACKGROUND: Rett syndrome (RTT), a rare neurodevelopmental disorder caused primarily by pathogenic variants in the MECP2 gene, is characterized by severe cognitive, motor, and autonomic impairments. Atypical sensory processing-including co-occurring hypo- and hyper-responsivity-is a core yet poorly understood feature. While evoked potentials (EPs) show delayed and attenuated sensory responses in RTT, the underlying mechanisms of these impairments remain unclear. Inter-trial phase coherence (ITPC), which quantifies trial-by-trial neural response consistency, offers a promising functional biomar…

Motion-onset visually evoked potentials are amplified in the deaf.

The loss of a sensory modality triggers a phenomenon known as cross-modal plasticity, where areas of the brain responsible for the lost sensory modality are reorganized and repurposed to the benefit of the remaining modalities. After perinatal or congenital deafness, superior visual motion detection abilities have been psychophysically identified in both humans and cats, and this advantage has been causally demonstrated to be mediated by reorganized auditory cortex. In our study, we investigated visually evoked potentials (VEPs) in response to motion-onset stimuli of varying speeds in both hea…

Beyond Seizures: A Comprehensive Review of Giant Somatosensory Evoked Potentials.

Giant somatosensory evoked potentials (gSEPs) are abnormally high-amplitude cortical responses to peripheral nerve stimulation, traditionally regarded as electrophysiological hallmarks of progressive myoclonic epilepsies (PMEs). However, accumulating evidence shows their presence in a broader range of non-epileptic conditions, including focal lesions, metabolic encephalopathies, neurodegenerative diseases, and even functional disorders. This review offers a comprehensive analysis of the physiological mechanisms, diagnostic criteria, and clinical significance of gSEPs, integrating data from bot…

Массетер Vestibular Evoked Myogenic Potenties (M-VEMPs) в Vestibular Neuritis.
