Aortic valve replacement to a patient with high titer of cold agglutinin.

A 60-year-old man was admitted to our hospital because of aortic stenosis with a peak pressure gradient of 61 mmHg, moderate aortic regurgitation, and a dilatation of the ascending aorta of 50 mm in diameter, which had grown 5 mm in 2 years. Because of severe aortic stenosis with a bicuspid valve and fast progression of the ascending aorta in size, replacements of both the aortic valve and the ascending aorta were planned. He had experienced severe acute renal failure with hemolysis because of cold agglutinin one year before the operation. The hemoglobin had decreased to 4.3 g/dL during hemolyt

[A case of cervical and mediastinal lymph nodes tuberculosis, tuberculous pleurisy, spinal caries and cold abscess in the anterior chest wall].

A 61-year-old woman with schizophrenia that had been treated in a psychiatric hospital was admitted to our hospital because of subileus and back pain. Though subileus was improved, she had a sudden attack of fever 7 days later and developed right pleural effusion, a cold abscess in the anterior chest wall and swelling of a thumb-sized right cervical lymph node which broke through the skin. We made a diagnosis of cervical and mediastinal lymph nodes tuberculosis, tuberculous pleurisy, spinal caries and cold abscess in the anterior chest wall due to the biopsy findings of the specimen taken from

Restoration of thermoregulatory response to body cooling by cooling hands and feet.

Deep body temperature fell progressively by 0.5-1.4 degrees C during 3-h immersions in 29 degrees C water. Both in unacclimatized volunteers and, to a lesser degree, in divers in cold-water training, cooling the hands and feet for 1 h in 12 degrees C water during such immersion caused sensation of cold, shivering, and rise in metabolic rate; it caused body temperature to rise in unacclimatized subjects and halted its fall in divers. Tissue conductances generally fell a little in divers but rose in unacclimatized subjects, probably because of muscle blood flow associated with the greater shiver

Lobotomies and Botulism Bombs: Beckett's Trilogy and the Cold War.

The article argues that Beckett's Trilogy stages the effects of a lobotomy operation on a potentially politically subversive writer, and that the consequences of the operation can be traced in both the retreat of the narrator(s) of the Trilogy into the mind and into comatose mental states and in the detail of the operation itself, based on the 'icepick' lobotomies performed by neurologist Walter Freeman in the late 1940s and early 1950s. To write about extreme psychiatric situations in the post-war period is necessarily to invoke the political uses of psychosurgery with which this article enga

Airway injury during high-level exercise.

Airway epithelial cells act as a physical barrier against environmental toxins and injury, and modulate inflammation and the immune response. As such, maintenance of their integrity is critical. Evidence is accumulating to suggest that exercise can cause injury to the airway epithelium. This seems the case particularly for competitive athletes performing high-level exercise, or when exercise takes place in extreme environmental conditions such as in cold dry air or in polluted air. Dehydration of the small airways and increased forces exerted on to the airway surface during severe hyperpnoea a

[Travelers exposed to extreme temperatures].

Sudden introduction of the unadapted human into extreme environments can result in serious, sometimes fatal, reactions. Most complications are due either to failure of thermoregulatory system or consecutive to the physiological responses to those environmental conditions. In addition to a number of minor diseases, cold can cause two major accidents, i.e., hypothermia and frostbite which can be enhanced at altitude. Moreover, the main disease in altitude conditions is represented by the acute mountain sickness which can lead to acute pulmonary and cerebral edema. Heat can cause heatstroke, dehy

Disseminated intravascular coagulation due to IgM-mediated autoimmune hemolytic anemia.

Disseminated intravascular coagulation (DIC) due to red cell hemolysis has been previously attributed to transfusion-related hemolytic reactions, but not to autoimmune hemolytic anemia. We report a case of DIC in a child with complement-fixing IgM-mediated cold-agglutinin autoimmune hemolysis, which resulted in arterial thrombosis and gangrene of the upper and lower extremities.

Actinomycosis of the Forearm: An Unusual Presentation of an Early Event.

Actinomycosis is a rare chronic infectious disease, most commonly affecting the cervicofacial, pulmonary, or genitourinary areas. It is caused by the Actinomyces spp. bacteria, which are facultative anaerobes and gram-positive, nonsporing rods. The disease is characterized by the formation of cold abscesses or fistulas, as well as granulomatous tissue, reactive fibrosis, and necrosis, which can resemble local malignancy. Cutaneous actinomycosis in the trunk or extremities is extremely rare and is usually related to previous surgical procedures or trauma. However, we report one such rare case o

[Closed femoral fracture complicated by blunt injury and posttraumatic occlusion of the superficial femoral artery--a case review].

The diagnosis and the treatment of an unusual posttraumatic occlusion of a femoral superficial artery during skeletal traction has been described. Angiography of the arteries of lower extremities was done because a sharp pain, cold, parestesia of the foot and absent pulsies had been found. The occlusion of the right femoral superficial artery was seen in the place of the compression of the artery by a fragment of femur. The fracture of the femur was stabilized with Ender's rods immediately and the injured part of the artery was resected and replaced by a venous graft. The healing was without a

Macro- and microcirculatory assessment of cold sensitivity after traumatic finger amputation and microsurgical replantation.

INTRODUCTION: Finger replantations after traumatic amputation are associated with good prognosis and acceptable functional results. However, cold sensitivity is a common and sometimes disabling sequelae after digital replantation. The exact causes of cold intolerance are still unclear; neural as well as vascular mechanisms have been discussed. We examined the macro- and microvascular performance of replanted fingers using high-resolution color-coded sonography for the assessment of skin vessel density of the fingertips as well as nailfold capillary microscopy and laser Doppler anemometry. Fabrication of an ion-enhanced low-temperature tolerant graphene/PAA/KCl hydrogel and its application for skin sensors.

Flexible sensors based on conductive hydrogels show great potential in wearable displays and smart devices. However, a water-based hydrogel inevitably freezes or loses its conductivity under extremely cold temperatures, leading to inadequate fulfillment of sensor performance. Herein, a well-designed strategy is proposed for fabricating a low-temperature-tolerant water-based hydrogel for sensor applications. By immersing a multi-crosslinking graphene(GO)/polyacrylic acid (PAA)-Fe3+ hydrogel into a KCl solution, an ion-enhanced conductive (GO/PAA/KCl) hydrogel is obtained with excellent conducti

The miR166-SlHB15A regulatory module controls ovule development and parthenocarpic fruit set under adverse temperatures in tomato.

Fruit set is inhibited by adverse temperatures, with consequences on yield. We isolated a tomato mutant producing fruits under non-permissive hot temperatures and identified the causal gene as SlHB15A, belonging to class III homeodomain leucine-zipper transcription factors. SlHB15A loss-of-function mutants display aberrant ovule development that mimics transcriptional changes occurring in fertilized ovules and leads to parthenocarpic fruit set under optimal and non-permissive temperatures, in field and greenhouse conditions. Under cold growing conditions, SlHB15A is subjected to conditional ha

Palmar cold threshold test and median nerve electrophysiology in carpal tunnel compression neuropathy.

The diagnosis of median nerve compression neuropathy at the carpal tunnel is usually confirmed by clinical electrophysiology. The classical findings of a significantly slowed median nerve conduction velocity for both sensory and motor fibres, with a prolonged distal motor latency and a reduced amplitude compared to age-related norms are unambiguous, but these criteria are often present only in part. In such cases another quantitative indicator of compression neuropathy would be extremely helpful. The present study aimed to test whether measurement of warm and cold sensory acuity in cases of pu

Feature Selection and Intelligent Classification of Dietary Nature Based on Tibetan Medicine Pharmacological Theory and Nutritional Components.

BACKGROUND: The cold, hot, and neutral natures are core concepts in Tibetan dietary therapy and guide traditional food classification and dietary use. However, their identification has largely depended on empirical judgment, and quantitative standards based on modern nutritional features remain limited. OBJECTIVES: This study aimed to develop an interpretable machine learning framework integrating feature selection, multimodel classification, and SHapley Additive exPlanations (SHAP) analysis to identify nutritional drivers of Tibetan dietary nature and support prediction for currently unlabel

Short-term impact of air pollution, noise and temperature on emergency hospital admissions in Madrid (Spain) due to liver and gallbladder diseases.

BACKGROUND: Very few epidemiological studies have explored the environmental and meteorological risk factors that influence liver diseases and gallbladder disorders, and no studies have addressed the specific case of Spain. METHODS: This is a retrospective ecological study conducted during 2013-2018. We analysed emergency admissions in the central area of the Region of Madrid for the following causes: Liver and gallbladder diseases (L&GB) (ICD-10: K70-K81); disorders of gallbladder (DGB) (ICD 10: K80-K81); liver disease (LD) (ICD 10: K70-K77); alcoholic liver disease (ALD) (ICD-10: K70); vira

Intracranial hypertension in a boy with Mycoplasma pneumoniae infection: the first report.

We describe a 7-y-old boy who developed idiopathic intracranial hypertension (IIH) during the convalescent phase of Mycoplasma pneumoniae (M. pneumoniae) infection. Anti-M. pneumoniae antibody and cold hemagglutinin titer were extremely high during his course, and corticosteroid administration was noticeably effective. It supports an immunopathological mechanism for his intracranial hypertension.

Effect of cold provocation on vessel density in eyes with primary open angle glaucoma: An optical coherence tomography angiography study.

The cold pressor test (CPT) induces a cardiovascular response, which may affect ocular blood flow and neuronal function. This study assessed whether optical coherence tomography angiography (OCT-A) can be used to evaluate CPT-induced changes in healthy eyes and in eyes with primary open-angle glaucoma (POAG). Twenty-two healthy subjects and 23 subjects with POAG and retinal fibre layer defects in only one hemifield were included in this study. The CPT was performed by submerging a subject's hand in cold water (0-4 °C) for 1 minute. The results showed that baseline peripapillary and macular ves

Street-level ambient fine particulate matter and tuberculosis: A machine learning and Bayesian spatiotemporal analysis.

Tuberculosis (TB) remains a leading cause of death from infectious diseases globally, yet evidence linking short-term ambient fine particulate matter (PM2.5) exposure to TB incidence at fine spatial scales is limited. We investigated the street-level associations between short-term PM2.5 exposure and TB onset in Jinan, China, involving a total of 8291 newly diagnosed active TB cases from 2015 to 2019. Daily street-level PM2.5 concentrations were estimated using an extreme gradient boosting (XGBoost) model integrated with geographic information systems (GIS). A Bayesian hierarchical spatiotempo

Neuronal Activity in the Hibernating Brain.

Hibernation is a natural phenomenon in many species which helps them to survive under extreme ambient conditions, such as cold temperatures and reduced availability of food in the winter months. It is characterized by a dramatic and regulated drop of body temperature, which in some cases can be near 0°C. Additionally, neural control of hibernation is maintained over all phases of a hibernation bout, including entrance into, during and arousal from torpor, despite a marked decrease in overall neural activity in torpor. In the present review, we provide an overview on what we know about neuronal
